Resistance to antibiotics is a worldwide increasing problem. A well-known example is methicillin resistant Staphylococcus aureus, MRSA. What is the relevance of MRSA on a surgical ICU? On a 20 bed academic SICU/intermediate care ward 14,976 patients were treated in a seven-year period. We identified only 98 MRSA-positive patients. 56 (57 %) of them were merely colonised, 42 (43 %) suffered from an MRSA infection. A control group comprised 56 similar patients without MRSA detection. Patients with MRSA infection had a higher mortality rate (OR 4.18; p = 0.002), but only 4 out of 20 patients died due to the MRSA infection. APACHE 2 score of more than 20 was predictive for being colonised with MRSA (OR 3.08; p = 0.04), but it was not a risk factor for developing an MRSA infection (OR 1.03; p = 0.95). Patients with MRSA colonisation did not have a higher mortality rate than patients without. Outcome depended on severity of the disease, but not on the MRSA colonisation status. Patients with MRSA infection were more likely to die, but the reason of death rarely was MRSA.
SummaryLymphatic complications are minor complications after varicose vein surgery. The frequency of lymphatic complications following an operation on varicose veins is very rarely discussed in literature. Patients, method: We investigated their frequency in a prospective study. Results: Between January 2004 and December 2005 in the cases of 3565 varicose vein operations we found 345 or 9.7% lymphatic complications, i.d. 123 cysts (3.4%), 24 fistulas (0.7%), 7 lymphorrhoe (0.2%) and 5 lymphoedema (0.14%). Conclusion: Lymphoedema is the only longlasting complication of the varicose vein surgery.
Zusammenfassung Ziel, Patienten, Methoden: Von Januar 2002 bis Dezember 2003 musste bei 96 Patienten ein Revisionseingriff wegen einer klinisch und farbduplexsonographisch relevanten Rezidiv-Varikosis der Fossa poplitea vorgenommen werden. Das entspricht einem Anteil von 10% der Vena-saphena-parva(V.s.p.)-Operationen. Mit der uneingeschränkt reproduzierbaren Farbduplexsonographie geschieht sowohl die präoperative Markierung der hämodynamisch relevanten Refluxe am stehenden Patienten als auch die Festlegung der operativen Strategie. Alle Patienten wurden intraoperativ klassifiziert; nach 6 Wochen, 6 Monaten und 1 Jahr nachuntersucht. Ergebnisse: Entsprechend ihrem intraoperativen Befund und ihrer Wertigkeit teilten wir die poplitealen Rezidive in vier Gruppen ein: I: übersehene V.s.p.-Anteile oder zu lang belassene V.s.p.-Stümpfe (71%), II: insuffiziente Kniekehlenperforantes (14,6%), III: insuffiziente Gastroknemiusvenen (12,5%), IV: neovaskuläre Varikosen (2,1%). Bei etwa 84% fanden sich Defekte der poplitealen Faszie, die bei 51% so immens waren, dass wir zur Rekonstruktion ein Vicrylnetz implantierten. Intraoperative Komplikationen wurden nicht beobachtet. Die Quote der Nervus-suralis-Läsionen betrug nach einem Jahr 5,2%. In 13,5% traten Flächenhämatome auf, in ca. 5% konservativ beherrschbare Wundheilungsstörungen. Etwa 12% der Patienten benötigten eine prolongierte Lymphdrainagebehandlung. Schlussfolgerung: Ein Großteil der poplitealen Rezidive kann durch eine differenzierte Diagnostik und standardisierte operative Technik mit exakter poplitealer Krossektomie beim Ersteingriff vermieden werden. Eine strenge Indikationsstellung stellt die Voraussetzung für eine erfolgreiche operative Behandlung der poplitealen Rezidive dar. Wir sehen bei sehr großen Fasziendefekten mit der Implantation von Vicrylnetzen eine Möglichkeit, die Faszie zu rekonstruieren.
