Met belangstelling lazen wij het artikel van collega J. Dam omtrent een voorstel voor een meer gestructureerde methode van consultvoering: WASUDOM.1 Goede structurering van een consult en duidelijke schriftelijke vastlegging daarvan is zeer belangrijk. Evenals het artikel over 'Het transparante spreekuur' van collega's Hoedeman et al.2 kan het voorstel van collega Dam daaraan zeker een positieve bijdrage leveren. De volgorde waarin hij de diverse aspecten in een consult wil belichten, is, ons inziens, echter niet logisch. Daarnaast ontbreken een aantal aspecten. In deze reactie willen wij kort onze bezwaren toelichten. Daarnaast willen wij een voorstel doen dat meer past bij onze praktijkvoering.Collega Dam stelt voor de UWV-status te bespreken na de sociale anamnese. Wij denken dat het logischer is de UWV-status evenals de gegevens over functie, uren, werkgever als eerste te noteren, alvorens aan de anamnese te beginnen.Het vermelden van medicatiegebruik als allerlaatste stap vo´o´r stellen van conclusie en beleid, komt o.i. als mosterd na de maaltijd. Dit hoort thuis in de anamnese.In die anamnese horen naast de nadere precisering van de klachten, informatie over reeds ingezette diagnostiek, de gestelde diagnose, ingestelde behandeling en het effect daarvan, medische voorgeschiedenis en medicatiegebruik. Na de sociale anamnese is een overzicht van beperkingen die in het sociale leven en in werk worden ondervonden, nodig. Eventuele belemmeringen worden opgespoord door betrokkene te vragen naar de mening over het eigen werk en de kwaliteit van de werkrelaties. Dit levert veel nuttige informatie op die handig is bij het opstellen van adviezen t.a.v. reı¨ntegratie.
