Цель. Исследование физических свойств составляющих биологических тканей и их роли в процессе электрической сварки мягких биологических тканей, а также измерение температуры в зоне электросварки. Материалы и методы. Исследования провели с использованием созданной системы для измерения температуры, состоящей из усилителя термо-ЭДС, внешнего модуля АЦП Е14-440 “Л-Кард”, ноутбука с программным обеспечением “PowerGraph”. Использовали также биполярный электрохирургический зажим и ВЧ-электрокоагулятор ЕК-300М1. Для имеющегося при сварке биологических тканей диапазона температур 40 – 200 °С выбраны термопары К–типа диаметром 70 мкм. Результаты. Проведенные исследования позволили установить, что сопротивление электролитов уменьшается, а проводимость - увеличивается с повышением температуры. Это объясняется тем, что с повышением температуры для слабых растворов увеличивается их диссоциация и, следовательно, число положительных и отрицательных ионов. Электропроводность цельных органов в 2 - 5 раз ниже, чем электропроводность биологических жидкостей. Поэтому при «полном» обезвоживании ткань практически теряет электропроводность, тепло в ней перестает выделяться и происходит резкое падение напряжения, силы тока и сопротивления, что предотвращает глубокие деструктивные изменения мягких тканей в зоне электросварки. Эти данные получены нами в процессе выполнения эксперимента по изучению температурных режимов тканей кишечника, мышечной ткани, легкого и печени. Выводы. Данными, полученными в ходе выполнения эксперимента, доказана безопасность высокочастотной электросварки мягких тканей в пределах температурных режимов от 45 до 95 °С без явлений некроза и обугливания.
The study of the exosomal effect on the paracrine secretion of peripheral blood mononuclear cells of patients with chronic heart failure (CHF) in vitro demonstrated gender-associated changes in the synthesis and secretion of the key factors of endothelial inflammation by leukocytes.Under the exosomesinfluence, the synthesis and accumulation of the cytokine monocyte chemoattractant protein-1 (MCP-1) in the leukocytes of healthy donorsincreased by 26%; while in women by 18%, in men by 38% was observed. Incubation of patients’cells with suspension of exosomes increased intracellular synthesis of MCP-1 in women by 16%, in men by 19%.Interestingly, in activated cells of control women group the synthesis of MCP-1 under the exosomes influence was 2.7 times higher than in the group of female patients and in leukocytes of male donors 4 times higher than in the CHFmale group. Thus, under the exosomes influence the synthesis of the cytokine MCP-1 in cells increaseswhereasits paracrine secretion decreases, which may be considered as one of the potential physiological mechanisms of “restraining” the progress of inflammation. Our findings suggest that the level of cytokinetumor necrosis factor-alpha (TNFα) synthesis in activated leukocytes of CHF and control groups was almost the same, while in CHF group the TNFα level increases only in the culture medium. Incubation with exosomes did not cause significant changes in cytokine production, but in the CHF group there was a tendency to decrease the “medium/cell” index by more than 1.3 times. Subsequent future studies of the exosomal corrective effects would enable to establish algorithms for potential therapeutic solutions on initiative mechanisms of endothelial inflammation and development of acute and chronic ischemia. Our primary experimental work demonstrates the need for further study of the exosome’s influence on peripheral blood leukocyte function both in vitro and in vivo experimental models.
Extracellular small vesicles – exosomes, attract the attention of researchers as a promising tool for regulating intercellular communication. At the same time, the therapeutic effects achieved through the use of mesenchymal stem cells (MSCs) are traditionally considered by medicine as a time-tested, multi-vector strategy for the treatment of various pathologies. In particular, in addition to the application of MSCs directly, it is important to study the products of their secretion. Aim. To study characteristics of the influence of exosomes isolated from blood plasma of healthy donors and conditioned medium of placental mesenchymal stromal cell (MSC) culture on paracrine secretion of peripheral blood mononuclear cells (PBMCs) in patients with chronic heart failure (CHF) in vitro. Materials and methods. The characteristics of paracrine secretion of PBMCs in patients with CHF by the content of proteins VEGF-A, MCP-1, ICAM-1 in their culture medium were studied in two directions: under the influence of exosomes isolated from plasma of healthy donors and exosomes isolated from placental MSC culture medium. Results. Incubation of PBMCs with plasma exosomes increased VEGF-A secretion in the group of healthy donors by 2.73 times (P ≤ 0.05), in patients with CHF – by 2 times (P ≤ 0.05); but there were a multidirectional effect on the content of ICAM-1 protein: it was 1.8 times (P ≤ 0.05) increased in the group of donors and 1.4 times (P ≤ 0.05) decreased in the group of CHF patients; MCP-1 secretion was insignificantly 10 % reduced in the donor group and did not change significantly in patients. Incubation of РВМС with exosomes isolated from MSC conditioned medium did not cause significant changes in paracrine secretion of PBMCs: there was a decrease in secretion of VEGF-A by 25 %, ICAM-1 by 17 %, MCP-1 by 22 % in the group of healthy volunteers; secretion of these proteins was 19.7 %, 22.0 % and 25.0 % decreased, respectively, in the group of CHF patients. The effects observed in the incubation of PBMCs with exosomes isolated from blood plasma of healthy donors and conditioned medium of placental MSC culture have provided valuable information for the design of future studies in this area of cell biology. Conclusions. Our studies have demonstrated in vitro the effects of exosomes isolated from donor plasma and conditioned medium on the functional properties of human PBMCs.
The aim – based on the analysis of the scientific literature focused on understanding the role of exosomes in the mechanisms of inflammation development and application of stem cells for cellular therapy in different pathological conditions, to identify and substantiate the prospects of using the exosomes as prognostic markers of a disease progression and application of their therapeutic potential in cardiovascular pathology. Global trends in the study of stem cells of different origins from the perspective of morphofunctional, molecular-genetic, cytogenetic, immunogenetic and cytological characteristics contribute significantly the development of regenerative medicine in the context of developing new methodological solutions for the use of stem cells and their components, particularly exosomes, for cell therapy of various pathological conditions. Studies show the indirect effect of exosomes on the immune response activation, coordination of cellular senescence processes and antigen presentation. There are also evidence of their impact on the structural and functional restoration of affected organs and blood vessels. The application potential of exosomes in practical medicine, particularly in the area of new approaches development to synthesize the newer biopharmaceuticals and as markers of multifactorial pathology course in conjunction with studies on the mechanisms of exosome involvement into immune processes is discussed. The study on the exosome-mediated mechanisms of inflammation in atherosclerosis is relevant, given the fact that their main physiological role is to implement the link between immunocompetent cells. Conclusions. Improving knowledge of the molecular biological mechanisms of the exosome influence on immunological processes in patients with cardiovascular pathology allows to expand the range of diagnostic and prognostic criteria for the formation of immuno-inflammatory reactions and endothelial dysfunction and to outline ways to personify the choice of therapeutic programs, which, in turn, can open approaches to develop fundamentally newer pharmaceuticals.
Мета. Аналіз сучасного стану правового забезпечення хірургічної корекції статевої належності в Україні. Матеріали і методи. Проаналізовано Постанову Верховної Ради України «Про затвердження положень про паспорт громадянина України та про паспорт громадянина України для виїзду за кордон», статті 42, 51 Основ законодавства України про охорону здоров’я, Наказ Міністерства охорони здоров’я України від 3 лютого 2011 р. №60 «Про удосконалення надання медичної допомоги особам, які потребують зміни (корекції) статевої належності» та Наказ Міністерства охорони здоров’я України від 5 жовтня 2016 р. №1041 «Про встановлення медико-біологічних та соціально-психологічних показань для зміни (корекції) статевої належності та затвердження форми первинної облікової документації й інструкції щодо її заповнення», (далі – Наказ МОЗ України від 5 жовтня 2016 р. №1041) Результати. Досі залишається актуальним питання правового супроводу корекції (зміни) статевої належності в Україні і немає єдиного принципу дотримання певного алгоритму дій лікаря-хірурга під час лікування хірургічних хворих, хоча відповідно до Наказу МОЗ України від 5 жовтня 2016 р. №1041 процедура отримання дозволу для корекції статевої належності стала дещо спрощеною. Однак положення цього наказу й дотепер викликають дискусії і можуть мати наслідки правового характеру для лікаря-хірурга, який виконує свої службові обов’язки. Висновки. Діяльність МОЗ України, спрямована на приведення у відповідність до світових стандартів правової регламентації зміни (корекції) статевої належності, потребує не лише державної підтримки та запровадження відповідних програм, а й подальшої законодавчої ініціативи з метою закріплення даних положень на державному рівні щодо осіб, стать яких є невизначеною.
