L'injection intrathécale directe de baclofène a été proposée dès 1984 par Penn dans le traitement des spasticités sévères, chroniques et diffuses d'origine médullaire. Cette technique a été introduite en France par Lazorthes et de nombreux travaux ont été publiés démontrant l'efficacité et la fiabilité de ce traitement. Les indications initialement limitées aux hypertonies musculaires et spasmes d'origine spinale s'étendent aujourd'hui aux atteintes cérébrales et reposent sur l'existence d'une spasticité multifocale gênante, résistante aux thérapeutiques classiques. La sélection des patients nécessite une approche pluridisciplinaire rigoureuse et une positivité des tests pré-implantation. Au stade d'administration chronique, au-delà des procédures de suivi, les progrès technologiques autorisent l'adaptation des posologies en fonction des résultats cliniques et des bénéfices attendus des personnes. En regard des résultats observés sur l'hypertonie, les spasmes douloureux, l'autonomie et la qualité de vie, les complications dont la fréquence est variable sont le plus souvent réversibles. Les pompes actuelles présentent une fiabilité certaine et une durée de vie prolongée. Les limitations de cette thérapeutique reposent sur les contraintes organisationnelles et le suivi en ambulatoire des personnes. Ce traitement relève de notre discipline et est accessible à toute équipe de médecine physique et de réadaptation. This technique was introduced to France by Lazorthes, since which time many publications have demonstrated the effectiveness and reliability of this method of treatment. Initially only recommended for muscular hypertonia and spasms of spinal origin, its scope has spread to cover cerebral injuries, based on the existence of intractable multifocal spasticity, which is resistant to classic therapies. Selection of patients requires a rigorous, multidisciplinary approach and positive results in the pre-implantation tests. At the stage of chronic administration, beyond the monitoring procedures, technological advances allow doses to be modified in accordance with the clinical results and the expected benefits to the individual. With regard to the results observed for hypertonia, painful spasms, autonomy and quality of life, complications with variable frequency are those that most often able to be reversed. The current pumps are guaranteed to be reliable and have a long life span. The limitations of this type of therapy are linked to organisational constraints and ambulatory monitoring. This treatment falls within the scope of our discipline and is accessible to all physical medicine and rehabilitation teams.
Objet : La sténose du canal lombaire étroit a été décrite, pour la première fois, par Verbiest en 1954. Elle est, selon les études, associée à des troubles vésicosphinc-tériens dans moins de 10 % des cas avec une variabilité de l'expression clinique des troubles. L'objet de l'étude est d'analyser les principales présentations cliniques et urodynamiques des troubles vésicosphinctériens chez des patients présentant un canal lombaire étroit.Méthode : Nous avons analysé, de façon rétrospec-tive, 49 observations de patients présentant un canal lombaire étroit associé à des troubles vésicosphinctériens sur les données de l'interrogatoire, de l'examen neurologique, périnéal et des explorations urodynamiques.Résultats : Parmi les 49 patients, 29 étaient opérés. Le sex-ratio était de 0,88, la moyenne d'âge de 60 ± 16 ans. L'interrogatoire retrouvait 35 % de rétention, 57 % de dysurie, 49 % de pollakiurie et 51 % d'incontinence urinaire. Les explorations urodynamiques retrouvaient 43 % de détrusors normaux, 29 % de détrusors hypoesthésiques, 16 % de détrusors hypocompliants et 16 % de détrusors hyperactifs. Nous confirmons le polymorphisme de l'atteinte vésicosphinctérienne chez les patients atteints de canal lombaire étroit, avec, d'un côté, une présentation fonctionnelle sans retentissement sur les données urodynamiques, à l'instar de ce que l'on peut voir chez les lombalgiques et, d'un autre côté, des dysfonctionnements vésicosphinctériens neurogènes sur les données urodynamiques, nécessitant un diagnostic topographique, évolutif et un suivi adapté. A wide variety of urinary symptoms may occur in up to 10% of cases. We report here the clinical and urodynamic features in patients with lumbar spinal stenosis.Methods: We carried out a retrospective clinical and urodynamic analysis of urinary disorders in patients with defined lumbar spinal canal stenosis.Results: Forty-nine patients (Age 60 ± 16 years, sex ratio: 0,88) were investigated. Twenty-nine patients underwent surgical decompression. Urinary disorders were as follows: 35% urinary retention, 57% dysuria, 49% pollakiuria, 51% urinary incontinence. The urodynamic testing revealed 43% normoactive detrusor, 29% delayed bladder sensation, 16% decreased compliance, and 16% detrusor overactivity. Two distinct categories of patients may be defined: a first category of functional urinary symptoms with normal urodynamic findings similar to those observed in other low back pain patients, a second category of patients with defined urodynamic findings suggestive of a neuropathic bladder, who need careful diagnostic and therapeutic follow-up.
Avec le bilan d'organicité, l'identification des causes iatrogènes médicamenteuses constitue la première étape du bilan étiologique d'une constipation. Son expression peut aller du simple inconfort passager à une occlusion sur iléus paralytique. L'éviction du médicament iatrogène n'est pas toujours possible, comme lors des traitements antiparkinsoniens, certains traitements antidépresseurs ou l'analgésie aux opiacés. Une analyse systématique des facteurs iatrogènes non médicamenteux est de ce fait d'autant plus indispensable, tout particulièrement chez la personne âgée ou les sujets institutionnalisés. Its expression can range from transient discomfort to an occlusion on paralytic ileus. The eviction of iatrogenic medication is not always possible, as in some Parkinson's treatments, antidepressants, or opiate analgesia. L'hyperactivité vésicale est définie comme un trouble de la continence associant une urgenturie avec ou sans incontinence, et fréquemment associée à une pollakiurie et à une nycturie. Un ensemble d'observations récentes permet en effet, désormais, d'établir que l'hyperactivité vésicale correspond d'abord à une anomalie du traitement sensoriel régulant la continence. Les différents cadres étiopathogé-niques chez l'homme partagent un ensemble de désordres similaires au niveau de l'urothélium, des nerfs et du muscle lisse du détrusor. Ces désordres s'inscrivent pour partie dans le cadre d'une neuroplasticité orchestrée par les facteurs de croissance nerveuse. Recent findings establish that OAB may first refer to abnormal sensory processing of continence. The various pathologic conditions associated with OAB share common features of altered function of urothelium, innervation and detrusor smooth muscle, and may partly rely on growth factors that orchestrate neural plasticity.
