Introduction : Le cancer de l’estomac constitue un véritable problème de santé publique. Certaines études ont porté sur les aspects épidémiologiques et anatomo-cliniques de cette affection. Nous avons voulu actualiser les données sur l’épidémiologie et la sémiologie du cancer de l’estomac. Méthodologie : Etude transversale du 07 mai 2017 au 7 juin 2018 à Bamako au Mali. Étaient inclus, les sujets des deux sexes âgés d’au moins 18 ans chez lesquels le diagnostic de cancer de l’estomac était retenu sur l’aspect endoscopique et confirmé à l’histologie. Résultats : Cent cinquante-neuf cas de cancers digestifs ont été recensés dont 37 cas de cancer de l’estomac (23.3%). L’âge moyen était de 59,5 ans ±13,5 ans. Le sex-ratio était de 1,3. Parmi ces patients, 64,9% des provenaient du milieu rural, 89,2% utilisaient la conservation naturelle des aliments, et 86,4% consommaient du poisson fumé. Une consommation de potasse dans les repas était retrouvée chez 83,7% ; 21,6 % ne consommaient pas de fruits et légumes. Une consommation de tabac a été retrouvée chez 21,6% des patients. La consommation d’alcool a été retrouvée chez 8,1% des patients. Les principaux symptômes étaient l’épigastralgie, l’amaigrissement et les vomissements respectivement dans 34% ; 34% et 33% des cas. Les lésions ulcéro-bourgeonnantes étaient plus retrouvées chez 67,6% des patients. La localisation antrale était retrouvée chez 51,35% des patients. L’adénocarcinome a été le type histologique le plus retrouvé dans 91,89% des cas. Conclusion : Le cancer de l’estomac est une affection relativement fréquente, diagnostiquée à un stade tardif. Le rôle permissif des facteurs nutritionnels et le mode de vie sont déterminants dans la genèse de ces cancers. Introduction: Stomach cancer is a real public health problem. Some studies have focused on the epidemiological and anatomo-clinical aspects of this condition. We wanted to update the data on the epidemiology and semiology of stomach cancer. Methodology: Cross-sectional study from 07 May 2017 to 7 June 2018 in Bamako, Mali. Included were subjects of both sexes aged at least 18 years in whom the diagnosis of stomach cancer was retained on the endoscopic aspect and confirmed at histology. Results: One hundred and fifty-nine cases of digestive cancer have been identified, including 37 cases of stomach cancer (23.3%). The average age was 59.5 years ±13.5 years. The sex ratio was 1.3. Of these patients, 64.9% came from rural areas, 89.2% used natural food preservation, and 86.4% consumed smoked fish. Potash consumption in meals was found in 83.7%; 21.6% did not consume fruits and vegetables. Tobacco use was found in 21.6% of patients. Alcohol consumption was found in 8.1% of patients. The main symptoms were epigastralgia, weight loss and vomiting respectively in 34%; 34% and 33% of cases. Ulcerative budding lesions were more found in 67.6% of patients. Antral localization was found in 51.35% of patients. Adenocarcinoma was the most common histological type found in 91.89% of cases. Bottom Line: Stomach cancer is a relatively common. Conclusion: diagnosed at a late stage. The permissive role of nutritional factors and lifestyle are decisive in the genesis of these cancers.

Objectif : contribuer à une meilleure prise en charge de l’ascite à Bangui. Patients et méthodes : nous avions réalisé une étude transversale descriptive pendant 30 mois dans le service d’hépatogastroentérologie du CHUASC (Centre Hospitalo-universitaire de l’Amitié Sino-Centrafricaine de Bangui). Elle portait sur les patients des deux sexes hospitalisés pour ascite chez lesquels le diagnostic étiologique était connu. L’âge moyen était de 39 ans avec des extrêmes de 18 et 78 ans avec sex-ratio de 1,42. Résultats : Pendant la période d’étude, 1210 patients étaient hospitalisés dont 204 présentaient une ascite (16,85%), parmi ceux-ci, 170 (14,9%) répondant à nos critères d’inclusion étaient retenus. L’ascite était jaune citrin dans 136 cas (80%) et pauvre en protéine (˂ 25 g/l) dans 136 cas (80%) et riche en protéine (˃ 25 g/l) dans 34 cas (20%). La moyenne des Globules Blancs dans le liquide d’ascite était de 76,94 éléments blancs/mm3 avec des extrêmes de 39 et 678/mm3. Le portage du VHB (virus de l’hépatite B) était plus fréquent dans 55 cas sur 114 (48.24%). Le foie remanié était fréquent dans 108 cas (63,5%). Les étiologies de l’ascite les plus fréquentes étaient la cirrhose décompensée dans 138 cas (81,2), la carcinose péritonéale dans 15 cas (8.8%), tuberculose péritonéale dans 12 cas (7.1%) et cardiaque dans 5 cas (2.9%). Les sujets de sexe masculin étaient plus exposés à la cirrhose décompensée sur le mode ascitique. La carcinose péritonéale dans 15 cas (8,8%) et la tuberculose péritonéale dans 12 cas (7,1%). La cause plus fréquente de la carcinose péritonéale était le Carcinome Hépatocellulaire dans 13 cas (7,65%). Les principaux facteurs de risque étaient le Virus de l’Hépatite B retrouvé dans 55 cas (32,5%) et la consommation d’alcool dans 130 cas (76,5%). Conclusion : les ascites étaient d’étiologies cirrhotique et associées au virus de l’hépatite B. Les hommes étaient plus concernés, une politique de vaccination universelle contre le VHB est nécessaire et prometteuse. Results: During the study period, 1210 patients were hospitalized, 204 of whom presented with ascites (16.85%), of these, 170 (14.9%) meeting our inclusion criteria were retained. The ascites was citrine yellow in 136 cases (80%) and poor in protein (˂ 25 g/l) in 136 cases (80%) and rich in protein (˃ 25 g/l) in 34 cases (20%). The average of White Blood Cells in ascites fluid was 76.94 white elements/mm3 with extremes of 39 and 678/mm3. The carriage of HBV (hepatitis B virus) was more frequent in 55 cases out of 114 (48.24%). The remodeled liver was frequent in 108 cases (63.5%). The most common etiologies of ascites were decompensated cirrhosis in 138 cases (81.2%), peritoneal carcinomatosis in 15 cases (8.8%), peritoneal tuberculosis in 12 cases (7.1%) and cardiac in 5 cases (2.9%). ). Male subjects were more prone to decompensated cirrhosis in the ascites mode. Peritoneal carcinomatosis in 15 cases (8.8%) and peritoneal tuberculosis in 12 cases (7.1%). The most common cause of peritoneal carcinomatosis was Hepatocellular Carcinoma in 13 cases (7.65%). The main risk factors were the Hepatitis B virus found in 55 cases (32.5%) and alcohol consumption in 130 cases (76.5%). Conclusion: the ascites were of cirrhotic etiology and associated with the hepatitis B virus. Men were more concerned, a universal vaccination policy against HBV is necessary and promising.
Introduction : l’Ascite est un symptôme majeur fréquent dans beaucoup de pathologies digestives. Elle est la complication majeure de cirrhose décompensée. Objectif : Contribuer à une meilleure prise en charge de l’ascite à Bangui. Patients et méthodes : Il s’agit d’une étude transversale descriptive d’une durée de 30 mois menée dans le service d’Hépato gastroentérologie du CHUASC de Bangui. Nous avions inclus dans l’étude les patients de deux sexes hospitalisés pour ascite chez lesquels le diagnostic étiologique était connu. Résultats : Pendant la période d’étude, 1210 patients étaient hospitalisés dont 204 présentaient une ascite (16,85%), parmi lesquels 170 (14,9%) avaient répondu à nos critères d’inclusion. On comptait 100 hommes (58,8%) et 70 femmes (41,2%). La sex-ratio était de 1,42. L’âge moyen des patients était de 46 ans. Les motifs d’hospitalisation les plus fréquents étaient le ballonnement abdominal dans 155 cas (91,2%), l’OMI dans 87 cas (51,2%), la douleur abdominale dans 84 cas (49, 4%). L’amaigrissement était retrouvé dans 134 cas (78,8%) et la fièvre dans 10 cas (5,9%). L’ascite était de moyenne abondance dans 89 cas (52,35%). Le foie était d’allure cirrhotique à l’examen physique dans 127 cas (74,7%). Conclusion : l’ascite est signe fréquent en hospitalisation souvent associés aux ballonnement abdominal, œdème des membres inférieurs, la douleur abdominale l’amaigrissement et la fièvre. Les hommes étaient les plus atteints, il concerne en majorité les jeunes. Dans les deux sexes.

Introduction: La pathologie digestive haute est riche et variée. L’endoscopie digestive haute constitue l’examen de référence pour le diagnostic des pathologies digestives. Nous voulons déterminer l’importance des lésions rencontrées en fibroscopie digestive haute à Bangui Résultats: Au total 1233 lésions œsogastroduodénales ont été observées. Les anomalies les plus fréquentes étaient l’incontinence cardiale (467 cas soit 37,88%) et les anomalies inflammatoires (443 cas soit 35,93%). Six cas de cancer (0,61%) étaient observés au niveau gastrique. Les lésions étaient observées dans l’estomac dans 980 cas (79,48%), œsophage dans 196 cas (15,9%). Les lésions duodénales étaient observées dans 57 cas (4,62%). Les anomalies gastriques étaient dominées par les gastrites dans 417 cas (42,55%) alors que les lésions œsophagiennes et duodénales étaient dominées respectivement par les varices œsophagiennes dans 139 cas (70,92%) et les ulcères dans 55 cas (96,49%). Conclusion: La pathologie digestive haute est fréquente à Bangui. Les anomalies endoscopiques sont multiples dont les fréquentes sont les varices œsophagiennes et l’incontinence cardiale. L’extension de l’endoscopie dans les autres formations sanitaires sera bénéfique pour l’amélioration de la prise en charge des patients. Introduction: Upper digestive pathology is rich and varied. Upper digestive endoscopy is the reference examination for the diagnosis of digestive pathologies. We want to determine the importance of the lesions encountered in upper digestive fibroscopy in Bangui. Results: A total of 1233 esogastroduodenal lesions were observed. The most common abnormalities were heart incontinence (467 cases or 37.88%) and inflammatory abnormalities (443 cases or 35.93%). Six cases of cancer (0.61%) were observed at the gastric level. Lesions were observed in the stomach in 980 cases (79.48%), esophagus in 196 cases (15.9%). Duodenal lesions were observed in 57 cases (4.62%). Gastric abnormalities were dominated by gastritis in 417 cases (42.55%) while esophageal and duodenal lesions were dominated by esophageal varices in 139 cases (70.92%) and ulcers in 55 cases (96.49%) respectively. Conclusion: Upper digestive pathology is common in Bangui. Endoscopic abnormalities are multiple, the frequent of which are esophageal varices and heart incontinence. The extension of endoscopy in other health facilities will be beneficial for the improvement of patient care.
