Tais preditores orientam a realização de testes adicionais durante a avaliação perioperatória.Na avaliação de risco coronariano recomenda-se a abordagem por passos, com uma boa relação custo efetividade desde as diretrizes publicadas em 1996. Investigações adicionais não estão indicadas em cirurgias de emergência. Em pacientes revascularizados nos últimos cinco anos, ou naqueles estratificados para isquemia nos últimos dois anos e que permaneçam clinicamente estáveis, nenhum teste adicional deve ser realizado. Na presença de preditores maiores de risco a cirurgia deve ser adiada, se possível, até que tais fatores sejam compensados. Nos pacientes com preditores intermediários, mas com boa capacidade funcional, o risco de complicações é pequeno. Um teste não-invasivo para isquemia é recomendado, entretanto, para pacientes com pobre capacidade funcional que serão submetidos a cirurgias de maior risco. Em pacientes com preditores menores e boa capacidade funcional, a cirurgia não cardíaca é segura. Com relação ao método para a estratificação coronariana, além da cintilografia miocárdica com tálio-dipiridamol, o ecocardiograma stress com dobutamina pode ser uma opção.Algumas situações merecem destaque. O paciente hipertenso grave deve ser estabilizado antes da cirurgia, com ênfase ao uso dos betabloqueadores. Dentre as valvopatias, a estenose aórtica é a que confere maior risco cirúrgico. Para os pacientes submetidos à angioplastia percutânea, recomenda-se que seja respeitado um período de pelo menos quatro semanas até que a cirurgia não cardíaca seja realizada. O propósito da avaliação perioperatória não é liberar o paciente para algum procedimento, mas sim realizar uma avaliação geral do estado clínico do paciente, fazer recomendações para o manejo e riscos de problemas cardíacos durante todo o período perioperatório e fornecer um perfil de risco para que sejam tomadas decisões que possam influenciar nos eventos cardíacos a curto e longo prazo.
