PurposeIn this study, we evaluated the effect of preputial type on bacterial colonization and wound healing in boys undergoing circumcision.Materials and MethodsThis study consisted of 78 boys consecutively admitted to our clinic for circumcision between 2009 and 2011. Preputial status was classified into five types on the basis of preputial retractability. One sterile culture swab was swept circumferentially once around the surface of the glans starting just proximal to the urethral meatus. Three weeks following circumcision, control swabs from the same regions of the same patients were taken and inoculated. Thus, the same patients formed the control group. Patients were evaluated on days 1 and 7 after the operation to assess whether the preputial type affected healing.ResultsThe mean age of the children was 46.3 months. In our study, the growth rate was 71.8% in pre-circumcision patients, whereas the rate was 10.25% in the post-circumcision group. Types 1, 2, 3, 4, and 5 had 100%, 93.8%, 71.4%, 44.4%, and 53.6% colonization, respectively. A significant difference was observed among these types in terms of colonization. The most common agent was Enterococcus species (33%). When postoperative patients were evaluated, all had local swelling and hyperemia on postoperative day 1, whereas there was a significant difference on day 7.ConclusionsThere was a significant correlation between preputial type and bacterial colonization, and the preputial type affected post-circumcision wound healing. Practitioners should keep in mind that the healing period will be longer in patients with type 1, 2, and 3 preputium.
Objective: This study was conducted to research the factors determining biochemical recurrence (BCR) in low-risk localized prostate cancer patients who underwent radical prostatectomy (RP). Material and methods:We retrospectively analyzed the data of 504 patients who had undergone RP between 2003 and 2013 at our clinic. One hundred and fifty-two patients who underwent RP for low-risk prostate cancer were included in the study. Results:The mean follow-up period for patients was 58.7 (21-229) months. The mean age of the patients was 63.7±7.2 years (49-79). The mean prostate specific antigen (PSA) value was 5.25±4.22 ng/mL (3.58-9.45). The BCR rate after the operation was 25% (38/152). In the univariate analysis, recurrence determining factors were shown to include extracapsular involvement (ECI) (p= 0.004), capsular invasion (CI) (p= 0.001), age (p= 0.014), and tumor size (p= 0.006). However, only CI was found to be significant in multivariate analysis (p= 0.001). Conclusion:Capsular invasion is an independent risk factor in low-risk prostate cancer patients who underwent RP for BCR.
Our study results showed that PSA value during initial diagnosis as well as Gleason score were independent factors in predicting BCR following radical prostatectomy.
Risk stratification should be considered in patients with prostate cancer while selecting the patients who would undergo pelvic lymphadenectomy. In addition, ePLND should be performed to patients undergoing lymphadenectomy.
ÖZET AMAÇ: Ciddi yanık travması olan hastalarda hastane kaynaklı idrar yolu enfeksiyon sıklığı ve risk faktörlerini araştırmayı amaçladık. GEREÇ VE YÖNTEM: Bu çalışmada Ağustos 2009-Nisan 2012 tarihleri arasında Bozyaka Eğitim ve Araştırma Hastanesi YanıkMerkezi'nde %20 ve üzerinde yanık yaralanması nedeni ile tedavi gören hastaları kapsayacak şekilde geriye dönük olarak değerlendirme yapıldı. Çalışmaya 69 hasta alındı. Çalışmadaki hastaların 30'u erkek (%43.5), 39'u kadındı (%56.5). Hastaların yaş ortalaması 40.1±16.7 idi.BULGULAR: DM, sondalı kalma süresi ve sonda bakımı ile hastane kaynaklı idrar yolu enfeksiyon arasında istatistiksel olarak anlamlı korelasyon saptandı (p<0.005). SONUÇ:Hastane kaynaklı idrar yolu enfeksiyonlarıın büyük bir kısmı üriner kateterizasyonla ilişkilidir. Bu şartlarda takılan üriner kateterin mümkün olduğunca kısa sürede sonlandırılması hastane kaynaklı idrar yolu enfeksiyonu gelişiminin önlenmesi açısından dikkat edilmesi gereken önemli bir noktadır. GİRİŞCiddi yanık travmalı hastalarda ortaya çıkan hastane kaynaklı enfeksiyonlar en sık gözlenen ölüm nedenlerinden birisidir. [1] Yetişkinlerde toplam vücut yüzey alanı'nın (TVYA) %20'den fazlasında yanık olması, %5'den fazla tam kat yanık olması, yük-sek voltaj yaralanması, bilinen inhalasyon yanığı olması, yüz-el gibi bölgelerde yanık olması ciddi yaralanma olarak tanımlan-mıştır.[2] Deri bütünlüğünün bozulması, yanık sonrası gelişen immün süpresif etki, tanı ve tedavi için girişim yapılması, hastanede uzun süre yatış gerektirmesi nedeniyle yanıklı hastalar hastane kaynaklı enfeksiyonlara daha açıktırlar. Gelişen enfeksiyonlar önemli morbidite ve mortaliteye yol açmaktadır. [2] Yanık hastalarında yoğun antibiyotik kullanımı floranın bozulmasına ve enfeksiyonların oluşmasına neden olabilmektedir. [2] Günümüzde tüm hasta gruplarında da hastane kaynaklı enfeksiyonlar önemli bir morbidite ve mortalite sebebidir. Sağlık hizmetlerinde iyileşme, hastanede bakım hizmetlerindeki gelişme ve yeni antibiyotiklerin geliştirilmesine rağmen hastane enfeksiyonları insidansı azalmış değildir. [3] İdrar yolu enfeksiyonları (İYE) tüm hastane enfeksiyonlarının %30-40'ını oluştururken, yoğun bakımda meydana gelen İYE ise tüm hastane enfeksiyonlarının %8-21'ini oluşturmaktadır. [4,5] Yoğun bakım hastalarında yapılan geniş bir surveyans ça-lışmasında İYE en sık üçüncü enfeksiyon olarak saptanmış. Abstract:Objectives: Ureteropelvic junction (UPJ) obstruction is of critical importance to understand the histopathology of UPJ obstruction in terms of therapy planning and follow-up. For this purpose, our study was conducted with TNF-α and TGF-β markers to investigate possible underlying problems in intrinsic UPJ obstruction. Methods: Of the patients who had undergone surgery in our clinic, 36 UPJ segments of patients who had undergone dismembered pyeloplasty surgery due to UPJ obstruction and 14 UPJ segments of the patients who had undergone nephrectomy were collected to form 2 groups. All histological sections were examined by applying immunohistochemical transforming growth factor beta 3 (TGF-β3) and tumour necrosis factor alpha (TNF-α) monoclonal antibody dyes. Results: The mean staining values for TNF-α in mucosal tissue and mucosa were 0.53±0.84 and 0.58±0.84, respectively in the obstruction group, whereas the values observed in the control group were 0.86±0.36 and 0.93±0.47, respectively. While the mean staining values in the obstruction group in mucosal tissue and mucosa for TGF-β3 were 1.75±0.73 and 2.17±0.77, respectively, the values established in the control group were 1.14±0.66 and 1.43±0.93, respectively. The difference between the obstruction and control groups were statistically signifi cant for both values (p<0.05). Conclusion: Only a limited number of studies have been carried out on this particular issue. Data from the present study indicate that TGF-β3 and TNF-α may play a role in the histopathogenesis of UPJ obstruction (Tab. 1, Fig. 1
