El higroma subdural (HSD) es una colección de líquido cefalorraquídeo (LCR) en el espacio sub-dural que puede ocurrir posterior al trauma cra-neoencefálico (TCE). Rara vez ocurre posterior a la realización de una craniectomía descompresiva (CD), éste se puede presentar de manera ipsilate-ral o contralateral. Se han propuesto varios meca-nismos fisiopatológicos para explicar la forma-ción del HSD posterior a la CD, incluyendo la pér-dida normal de la pulsatilidad del LCR al compro-meterse la integridad de la bóveda craneana. Aun-que el manejo quirúrugico de ésta colección es su drenaje, se ha demostrado que la craneoplastía es el manejo definitivo. Presentamos un caso de un paciente con TCE severo quien desarrolló un HSD contralateral al sitio de la CD, que requirió varias intervenciones de drenaje y que finalmente resol-vió definitivamente mediante la craneoplastía
