El factor principal del cáncer de cuello uterino es la infección por virus papiloma humano (VPH). Actualmente existen tres vacunas con alta eficacia y seguridad. En el Perú, el Ministerio de Salud cuenta con un programa de vacunación gratuita para niñas de 9 a 13 años con dos dosis de vacuna tetravalente (0 y 6 meses), cuya cobertura nacional es sumamente importante para conseguir revertir la alta morbilidad y mortalidad que esta enfermedad produce en nuestro país. Las mujeres de mayor edad y los hombres desde los nueve años también pueden ser vacunados por cuenta propia. Esta es la principal forma de prevención primaria que actualmente existe y debe ser complementada con la prevención secundaria.
El cáncer de cuello uterino es un problema de salud pública. La vacuna contra el virus del papiloma humano (VPH) protege contra la infección por el VPH. Ha mostrado ser efectiva para prevenir lesiones premalignas y cáncer de cérvix, así como lesiones de la vulva, vagina, canal anal, pene y orofaringe. Forma parte del calendario nacional de vacunación, es costo efectiva en la introducción de la estrategia nacional de vacunación y es la herramienta ideal ante sistemas de salud donde la prevención secundaria no ha dado resultado a lo largo del tiempo. La implementación del programa de vacunación en Perú se inició en el 2011. Actualmente, la indicación de la vacunación es con la vacuna tetravalente a niñas del 5° grado de primaria de los colegios públicos y privados, en 2 dosis a los 0 y 6 meses. En el 2019, la cobertura fue de 87% (234 535 niñas) para la primera dosis y 78% (211 339) para la segunda dosis.

La Sociedad Peruana de Obstetricia y Ginecología ha contribuido con el desarrollo de la ginecología en el Perú, incluyendo la especialidad de oncología ginecológica, mediante artículos de la especialidad publicados en la Revista Peruana de Ginecología y Obstetricia, congresos, cursos pre congresos, jornadas nacionales e internacionales, simposios, cursos para residentes, entre otros. El tema más desarrollado en la Revista de la Sociedad ha sido cáncer de cérvix, que constituye un problema de salud en nuestro país, seguido de cáncer de ovario y mama.
En este simposio tratamos algunos puntos de actualidad en la ginecología oncológica, como son las vacunas contra el virus papiloma humano (VPH), la cirugía mínimamente invasiva en cáncer ginecológico y la valoración preoperatoria de la masa anexial. Además, se revisan los síndromes genéticos más importantes. La Organización Mundial de la Salud el 2017 se ha pronunciado respecto a las vacunas contra el VPH, declarándolas muy seguras y recomendando la vacunación universal en niñas de 9 a 14 años(1). Ello exige a los médicos en general y especialmente a pediatras, ginecólogos y ginecólogos oncólogos conocer bien la información de la seguridad y eficacia de dichas vacunas y su correcto manejo, más aun contando con un programa nacional de vacunación contra el VPH gratuito a cargo del Ministerio de Salud. Es importante que la población reciba la información veraz y clara del conocimiento médico actual sobre estas vacunas, despejando los temores infundados que aún existen en nuestro medio. Solo aunando las mejores estrategias de prevención primaria y secundaria será posible revertir la alta morbimortalidad del cáncer de cuello uterino en nuestro país; de ahí la importancia de esta revisión. Se trata de un nuevo estándar en la práctica médica(2).
