The consensus was that transdermal buprenorphine has a valuable role to play in the treatment of chronic cancer pain because of its efficacy and good safety and tolerability profile, including a low risk of respiratory depression, a lack of immunosuppression and a lack of accumulation in patients with impaired renal function.
Le profil épidémiolo-gique, les aspects cliniques et thé-rapeutiques avec analyse des facteurs pronostiques des cancers invasifs du col utérin ont été étudiés de façon rétrospective sur une série de 96 patientes prises en charge au CHU Farhat-Hached de Sousse entre 1992 et 2000. Le cancer du col utérin demeure le 2 e cancer génital chez la femme après le cancer du sein dans le centre tunisien. La médiane d'âge était de 54 ans avec une proportion faible de femmes jeunes âgées de moins de 40 ans (12,5 %). Le délai moyen de consultation était de 6,5 mois et 20 % des malades ont consulté 12 mois après le début de la symptomatologie. La taille tumorale médiane était de 4,25 cm et elle était supérieure à 4 cm dans 70 % des cas. La classification clinique basée sur l'examen sous anesthésie générale a montré une proportion de 56,3 % de formes évoluées inopérables. L'étude histologique a révélé une nette pré-dominance des carcinomes épider-moïdes (95 %) par rapport aux adé-nocarcinomes (5 %). Soixante-six patientes (69 % des malades) ont eu un traitement complet et ont été évaluées sur le plan thérapeutique. Le traitement était basé sur l'association radiochirurgicale pour les stades limités (56 % des patientes) et la radiothérapie exclusive pour les stades avancés (44 % des patientes). Le recul moyen était de 36 mois (extrêmes : 4-84 mois). Le taux de rémission complète après le traitement était de 88 %. La survie globale et la survie sans événe-ment à 5 ans tous stades confondus étaient respectivement de 80,5 % et de 75 %. Onze récidives locorégio-nales et 5 métastases sont apparues au cours de la surveillance. Toutes les métastases et près de 2/3 des récidives sont apparues au cours des deux premières années de suivi. En étude univariée, les facteurs ayant un impact péjoratif sur le pronostic de la maladie étaient l'âge jeune, le stade clinique avancé et l'envahissement ganglionnaire. Les résultats étaient meilleurs pour les tumeurs de petite taille et les tumeurs différenciées mais la diffé-rence n'était pas statistiquement significative.

The aim of this investigator-initiated prospective randomized open-label single institutional trial is to evaluate the role of acupuncture in the treatment of acute skin and mucosal toxicity, xerostomia, and perception of taste, pain, and nausea related to curative and adjuvant (chemo)radiotherapy of head and neck cancer. This paper reports pilot data of the first 30 enrolled patients. Methods. Patients were randomized to undergo standard of care radiotherapy ± chemotherapy and support care defined by our institutional standard operating procedures alone or in the combination with acupuncture which was initiated with the first signs of any toxicity. Results. Fifteen patients were enrolled in both arms and all finished the treatment as planned.The median pain was significantly lower in the acupuncture arm (median 1.6 points vs. 2.5 points on a 10-item Likert scale; P=0.035) as well as duration of acute pain (median 31 days vs. 54 days; P=0.031). Patients with acupuncture had significantly shorter duration of acute skin (median 44 days vs. 109 days; P<0.001) and mucosal toxicity (median 34 days vs. 109 days; P<0.001) with no difference in grading of toxicity (median grade 1.6 vs. 1.5; P=0.701 and median grade 1.4 vs. 1.6; P=0.204 for skin and mucosa, respectively). No significant difference was found for other toxicity domains, with the exception of salivation toxicity which was significantly lower in acupuncture arm (median grade 1.3 vs. 1.7; P=0.048). Conclusion. In this interim analysis, acupuncture leads to lower pain andfaster disappearance of skin and mucosal toxicity after (chemo)radiotherapy of head and neck cancer. Description and validation of acupuncture using scientific approaches will further enhance acceptance of this method by both patients and health care providers.
