Background/aim: Atopic dermatitis (AD) is a chronic, inflammatory skin disorder characterized by dry skin, pruritus and eczematous lesions. The aim of this study is to evaluate the skin moisture and sebum content of the infants with AD and its relationship between the disease severity. Materials and methods: For this cross-sectional and case-control study we included 49 infants with AD aged between 2 and 24 months and 34 healthy infants with the same age and sex as a control group. Skin moisture and sebum content were measured by the bioimpedance method and recorded. Skin prick tests, total IgE levels and food-specific (milk, egg) IgE levels were performed. Results: There was a significant difference between the groups in terms of mean skin moisture and sebum content percentage (P < 0.001, both). The mean skin moisture and sebum content were correlated with CRP in patients with AD (P < 0.01, r =-1.00, both), but we did not find any correlation between these parameters with the disease severity. Conclusion: We found that skin moisture and sebum content were low even in unaffected areas in AD. The skin structure of these patients may contribute to the pathogenesis of the disease.
Aim: A thrombosis and inflammation marker, Signal peptide-CUB (complement C1r/C1 s)-EGF (epidermal growth factor)-like domaincontaining protein 1 (SCUBE-1) is expressed in endothelial cells and activated platelets. We aimed to investigate the levels of this marker in patients with Thalassemia major (TM), a disease which causes a tendency to thrombosis. Methods: Twenty patients with TM and age and gender-matched twenty healthy controls were included in this prospective case-control study. Other thrombosis markers such as D-dimer and homocysteine were studied in addition to SCUBE-1 in both groups. Results: The SCUBE1 and D-dimer levels were higher in the TM group compared to the control group (P=0.004, P=0.023, respectively). A significant positive correlation was detected between platelet count and SCUBE1 (r=0.342, P=0.031). Conclusion: We detected elevated SCUBE1 levels in children with TM compared to the healthy controls, which correlated with platelet count. SCUBE1 may be used as a thrombosis marker in TM in the early stages of the disease.
Aim: Anemia is an important public health problem that concerns human and community health. In this study, we aimed to determine the etiology of anemia in different age groups and create social awareness. Methods: The study (retrospective cohort study) included anemia patients between 6 months and 18 years old who applied to the Pediatric Hematology and Oncology Outpatient Clinic of KSU Medical Faculty between January 2015 and January 2018. Results: 1120 patients were included in the study. There were 873 with iron deficiency anemia, 184 with thalassemia minor, 25 with vitamin B12 deficiency anemia, 13 with autoimmune hemolytic anemia, 10 with hereditary spherocytosis and the other etiology of anemia. While 16.4% of all patients and 33.8% of patients aged 6-12 years had thalassemia minor. On the other side, 81.3% of patients aged 6 months-2 years, 74.3% of patients aged 2-6 years, 60.4% of patients aged 6-12 years and 85.0% of patients aged 12-18 years had iron-deficiency anemia. Iron deficiency anemia was common in males (60.2%) in the 2-6 years age group (P=0.002), but it was common in females (80.6%) in the 12-18 years age group (P<0.001). Conclusion: As a result of this study, it has been concluded that screening and iron supplementation should be done in societies at risk.
Bulgular: Çalışmaya dahil edilen 266 (89'u kız, 177'si erkek) Suriyeli mülteci çocuğun 111'inde (%41.7) anemi tespit edildi. 6 ay-59 ay yaş grubundaki çocukların 80'ninde (%54.4), 5-11 yaş grubundaki çocukların 22'sinde (%29.3), 12-14 grubundaki çocukların 6'sında (%24.0) ve 15-18 yaş grubundaki çocukların 3'ünde (%15.8) anemi tespit edildi (p<0.001). Anemi prevalansı erkek çocuklarda %48.0 (n=85) iken kız çocuklarda %29.2 (n=26) idi (p=0.003). Çocukların %14.4'ünde (n=16) hafif, %55.9'unda (n=62) orta ve %29.7'sinde (n=33) şiddetli anemi görüldü (p<0.001). Anemili çocukların %64.9'unda demir eksikliği anemisi (DEA), %16.2'sinde talasemi minör, %8.1'inde vitamin B12 eksikliği, %7.2'sinde Glukoz-6-fosfat dehidrogenaz (G6PD) eksikliği, %1.8'inde herediter sferositoz, %0.9'unda talasemi majör ve %0.9'unda otoimmün hemolitik anemi vardı. En yaygın görülen anemi tipi DEA (%64.9, n=72; p<0.001) idi. DEA'sı olan çocukların %73.6'sı (n=53) 6-59 ay grubunda idi (p<0.001).Sonuç: Bu çalışmanın sonuçları Suriyeli mülteci çocuklarda anemi prevalansının özellikle de demir eksikliği anemisi prevalansının yüksek olduğunu ortaya koymuştur ve bu çocuklarda anemi taraması ve demir takviyesi yapılması gerektiği sonucuna varılmıştır.
ÖzDisgerminom, dev boyutlara ulaşabilen overin en sık görülen malign tümörüdür. Genelde hormon salgılamamakla beraber beta-HCG salgılayabilirler. Bizde karında kitle ve karın ağrısı şikâyetleri olan radyolojik görüntüleme ile tespit edilen beta-HCG salgılayan dev boyutlara ulaşmış disgerminom tanılı 3 olguyu inceledik. Üç olguya da salpingo-ooferektomi ameliyatı yapıldı. Bir olguda komsu lenf nodlarından, peritondan ve karşı overden örnekleme yapıldı. Bir olgu da üç haftada bir, dört kür kemoterapi verildi. In general, although they do not secrete hormones, they may secrete hormones, they may secrete beta-HCG. We have studied 3 cases of disgerminoma that have reached giant dimensions that secrete beta-HCG detected by radiological imaging, which caused a complaint of abdominal pain in the abdomen. Salpingo-oophorectomy was performed in all three cases. In one case, neighboring lymph nodes, peritoneum and counter-ovary were sampled. In one case, once in three weeks, four times treatment of chemotherapy was administered and treatment was provided. In another case, four chemotherapy treatments was planned to administer. GİRİŞÇocukluk çağında malign over tümörleri oldukça nadirdir. Malign over tümörlerinden en sık disgerminom görülür. Pediatrik malign over tümörlerinin %26,8 'ini oluşturur (1). Genç yaşta özellikle 10-30 yaş arasında görülür. % 85 i tek taraflıdır. Genç yaş bayanlarda görülmesi nedeniyle fertilitenin korunması önemlidir (2). Germ hücrelerinin anormal çoğalmasıyla oluşur. Tümör kitlesi geniş çaplara ulaşabilir (3). Fibröz septalarla birbirinden ayrılan kistik kirli sarı renkte kitlelerdir. Karında oluşan büyük kitleye bağlı olarak batında distansiyon ve karın ağrısı oluşabilir. Metastaz yapması sonucu yaşamı tehdit eden karaciğer patolojileri saptanabilir (4). Genelde lenfatik yolla metastaz yapar, kan yoluyla metastazı nadirdir ve peritoneal yayılımda yapabilir. Tanı anında % 25 metastaz saptanır. USG kullanımının yaygınlaşması tanı konulan olgu sayısını artırmış tedavinin planlanması ve takibinde yardımcı olmuştur. Disgerminomlar genellikle hormon salgılamazlar ama bazı olgularda Beta-HCG sentezlenir (5). Alfa fetoprotein yüksekliği saptanması disgerminomun başka bir tümörle birlikte olduğunu düşündürmektedir. CA-125 yüksekliği saptanabilir ama tanı için nonspesifiktir (5).Tedavi esas olarak cerrahidir. Evreleme içinde cerrahi gereklidir. Tamamı çıkarılan metastaz olmayan malign tümörlerde başka tedaviye gerek yoktur (6). Tamamı çıkarılamayan ve metastaz yapan tümörlerde cerrahiye ilave sisplatinli kemoterapi rejimleri ile genellikle kür sağlanır. OLGU 1:14 yaşında kız hasta şiddetli karın ağrısı, kabızlık ve karında kitle nedeniyle başvurdu. Özgeçmiş ve soygeçmişinde özellik yoktu.
