BackgroundAs health workforce policy is gaining momentum, data sources and monitoring systems have significantly improved in the European Union and internationally. Yet data remain poorly connected to policy-making and implementation and often do not adequately support integrated approaches. This brings the importance of governance and the need for innovation into play.CaseThe present case study introduces a regional health workforce monitor in the German Federal State of Rhineland-Palatinate and seeks to explore the capacity of monitoring to innovate health workforce governance. The monitor applies an approach from the European Network on Regional Labour Market Monitoring to the health workforce. The novel aspect of this model is an integrated, procedural approach that promotes a ‘learning system’ of governance based on three interconnected pillars: mixed methods and bottom-up data collection, strong stakeholder involvement with complex communication tools and shared decision- and policy-making. Selected empirical examples illustrate the approach and the tools focusing on two aspects: the connection between sectoral, occupational and mobility data to analyse skill/qualification mixes and the supply–demand matches and the connection between monitoring and stakeholder-driven policy.ConclusionRegional health workforce monitoring can promote effective governance in high-income countries like Germany with overall high density of health workers but maldistribution of staff and skills. The regional stakeholder networks are cost-effective and easily accessible and might therefore be appealing also to low- and middle-income countries.

Ziel des Projekts "CaRe-Laufbahnentwicklung und Retention Management in der Pflege im Kooperationsverbund von Spitälern, Alters-und Pflegeheimen und Spitex" ist die Bereitstellung von Informationen und Umsetzungsinstrumenten zur Förderung der Verbundenheit des Pflegepersonals mit Beruf und Betrieb. Das Projekt erhebt in einem 1.Teil die nötigen Daten und entwickelt in einem 2.Teil Instrumente. Die Erhebung fragt nach Bedingungen der Verbundenheit (Commitment) des Pflegepersonals mit Beruf und Betrieb und wertet diese Ergebnisse in Bezug auf Lebensphasen, auf Führungsfunktionen und weitere Faktoren aus. Im Fokus der Auswertung steht die Verbundenheit mit dem Beruf, differenziert nach Institutionstypen (Spitex, Alters-und Pflegeheim und Spital) und nach Lebensphasen, worunter Altersphasen, Familienphase aber auch Merkmale wie Quereinstieg und Wiedereinstieg, erfasst werden. Dazu gehört ausserdem die Frage nach der Laufbahn in der Branche, d.h. dem Verbleib bzw. Wechsel zwischen den drei Institutionstypen Alters-und Pflegeheim, Spitex und Spital. Methode Die Datenerhebung wurde zwischen Juni und Oktober 2012 durchgeführt. Der gewählte Mixed-Method-Ansatz umfasst einen quantitativen Teil (schriftliche Befragung) und einen qualitativen Teil (Gruppendiskussionen). Die schriftliche Befragung basiert auf einer Vollerhebung, an der sich 1029 Pflegende aus den genannten Institutionen beteiligt haben. Damit wurde eine Rücklaufquote von 25 % erreicht. 46 Pflegende aus den drei Institutionstypen Spitex, APH und Spitäler haben an den Gruppendiskussionen teilgenommen. Ergebnisse Die Resultate zeigen, dass die emotionale Verbundenheit (affektives Commitment) mit dem Betrieb und dem Beruf insgesamt hoch ist. Das Pflegepersonal identifiziert sich über alle Institutionen hinweg sowohl mit dem eigenen Beruf als auch mit dem aktuellen Betrieb und fühlt sich emotional stark zugehörig. Je stärker die Verbundenheit ist, desto geringer ist die Absicht, den Betrieb bzw. Beruf zu verlassen. Führungsqualität, Entwicklungsunterstützung, Rollenklarheit, emotionale Belastung, subjektiv wahrgenommene Lohngerechtigkeit und Pflegequalität beeinflussen diese Verbundenheit mit Betrieb und Beruf massgeblich; sie sind zentrale Faktoren für Personalbindungsmassnahmen.

In Germany there is an increasing importance of home health care and nurses that are employed in the home care sector often have to face ethical issues. The purpose of this ethnographic study was to explore moral problems in the daily practice of these nurses. The method used was qualitative interviews with 20 nurses which have been analysed by content analysis. The results showed that the ethical principle of beneficence was the core concept for the participants. Moral problems arise when nurses cannot act in accordance to this principle or when they cannot determine the good in a situation. In particular, there were four types of moral problems: "beneficence vs. autonomy", "beneficence vs. justice", "beneficence vs. loyalty" and "The good cannot be determined". The way nurses in home health care address moral problems should be improved. Some participants lack ethical competencies. Furthermore, appropriate support services for dealing with moral problems have to be designed.
