A 36-year-old man with chronic renal insufficiency secondary to type 1 diabetes mellitus was on hemodialysis via central venous catheter (CVC), newly placed into the right subclavian vein after his arteriovenous fistula became dysfunctional. Seven days after CVC insertion, the patient developed fever and on day 11 echocardiography showed a large nearly occluding thrombus in the superior vena cava (SVC) extending into the right atrium (RA). Emergency surgical thrombectomy was successfully performed and an 11 cm long thrombus extending from the RA cranially into the SVC occupying majority of the vein's lumen was removed. Cultures from the thrombus and CVC were negative, but polymerase chain reaction was positive for Staphylococcus aureus. This particular case was interesting for a marked discrepancy between large SVC occluding thrombosis and a relatively mild clinical presentation with fever, and it highlights the importance of correct timing of echocardiography exam which might prevent potentially fatal consequences such as pulmonary embolism.
Autoři popisují případ 51letého pacienta, který byl odeslán na jejich pracoviště s diagnózou srdeční tamponády. Po odsátí 1 900 ml hemoragického výpotku byl na CT angiogramu zjištěn zdroj krvácení z perforace stěny pravé síně. Pacient byl akutně operován, krevní koagula z perikardiálního vaku byla odstraněna. Po uvolnění srdeční tamponády byla perforace pravé síně ošetřena suturou. Příčinou perforace byl nádor, který prorůstal stěnou pravé síně i pravé komory. Cor Vasa 2011;53:138-140. Cíl: Autoři srovnávají výsledky dvou metod náhrady aortální chlopně v CKTCH v letech 2005-2009. Soubor nemocných a metodika: V souboru 676 pacientů s hemodynamicky významnou aortální vadou bylo 472 pacientů operováno metodou implantace pokračujícími monofi lamentózními stehy, 204 pacientů metodou implantace jednotlivými polyfi lamentózními stehy s podložkami. Byly sledovány následující parametry: délka ischemické srdeční zástavy, velikost implantované chlopně, pooperační komplikace, zejména výskyt časného a pozdního leaku. Výsledky: Porovnáním výsledků obou skupin byl zjištěn statisticky významný rozdíl pouze v délce ischemické zástavy srdeční, v ostatních sledovaných parametrech se oba soubory nelišily. Aim: The authors compare the outcomes of two techniques of aortic valve replacement in a Center of Cardiac and Transplant Surgery in the 2005-2009 period. Patients and methods: In a group of 676 patients with hemodynamically signifi cant aortic valve disease, 472 patients had surgery with continuous monofi lament suture implantation while 204 patients had surgery using single polyfi lament pledglets. The following parameters were assessed: time of ischemic cardiac arrest, implanted valve size, postoperative complications (in particular early and late leak). Results: Comparison of the outcomes of both groups revealed a signifi cant diff erence only in the time of ischemic cardiac arrest, with no diff erence between the groups in the other monitored parameters. Conclusion: Both techniques of aortic valve replacement are comparable if properly indicated and performed fl awlessly in technical terms.
