Objetivo: determinar la prevalencia de infección por C. trachomatis en mujeres con parto prematuro.Metodología: estudio de corte transversal en el que se analizaron 50 muestras de orina de mujeres que asistieron al Instituto Materno Infantil de Bogotá entre febrero y junio de 2004, quienes presentaron parto prematuro entre las 25 y las 36 semanas de gestación. Para su detección se utilizó la técnica de reacción en cadena de la polimerasa (PCR COBAS AMPLICOR CT, Roche diagnostic Systems).Resultados: se encontraron dos muestras positivas para C. trachomatis, lo que corresponde a una prevalencia del 4% (IC95% 0,5 - 13,7) en mujeres con parto prematuro y del 4,5% (IC95% 0,6 - 15,5) en pacientes con RPM.Conclusión: la C. trachomatis debe investigarse en pacientes con trabajo de parto prematuro o con RPM pretérmino, aún en ausencia del antecedente de flujo genital patológico y particularmente en quienes no se encuentra la causa del trabajo de parto prematuro.
ResumenSe definen como infecciones nosocomiales las que ocurren después de 48 horas de hospitalización. Las Unidades de Cuidados Intensivos neonatales, pediátricas y de adultos son las áreas de mayor prevalencia debido al perfil del paciente, a las terapias y a los procedimientos invasivos a los que son sometidos quienes ingresan. De las infecciones intrahospitalarias, una de las más conocidas y caracterizadas es la neumonía, la cual representa la segunda causa de infección nosocomial, con un aumento en la morbi-mortalidad cuando se encuentra asociada a ventilación mecánica. Su epidemiología está muy bien documentada en adultos, pero existe poca información en niños y neonatos.Palabras claves: broncoscopia, infección nosocomial, neonato, neumonía, ventilación mecánica. AbstractNosocomial infections are those which occur 48 hours after being hospitalized. The newborn, paediatric, and adult ICU's are the most prevalent areas due to the patient profile, therapies and invasive procedures that inpatients have to undergo. Pneumonia is one of the most known and characterized hospital infections. It represents the second cause of nosocomial infection which augments morbid-mortality when it is associated to mechanical ventilation. Its epidemiology is very well documented in adults, but there is very little information on children and newborns.
ResumenChlamydia trachomatis (CT) es uno de los agentes etiológicos más importantes en las infecciones de transmisión sexual. En la mujer CT causa patologías tales como cervicitis, uretritis y enfermedad pélvica inflamatoria, entre otras. Los objetivos de este trabajo fueron determinar la prevalencia de la infección y la concordancia de las pruebas diagnósticas para CT en pacientes sintomáticas de vaginitis o leucorrea inespecífica, que asistieron al hospital Pablo VI de Bosa y al Hospital del Sur de Bogotá, durante los meses de junio y julio de 2004. Se obtuvieron 180 muestras simultáneas de suero y de orina para la determinación de IgG, IgM e IgA anticlamidia y para PCR, respectivamente. La detección de los anticuerpos para CT en suero se realizó mediante el método ELISA (VIRCEL) y la detección del DNA en orina se hizo con la prueba AMPLICOR CT PCR (ROCHE). La prevalencia de la infección fue del 31% (56 casos). La mediana de edad en mujeres con evidencia de infección reciente o activa fue de 23 años; 37 pacientes (66%) positivas para algún marcador tenían infección activa. En 24 participantes (42.8%) con infección activa, de acuerdo con los resultados serológicos, no se detectó el DNA en orina; en 14 pacientes (25% de las pacientes positivas en las pruebas serológicas) se encontró el PCR positivo en las muestras de orina. Una prevalencia del 31% en mujeres sintomáticas indica que la infección por Chlamydia es un problema para la salud pública. Implantar un programa de tamizaje para la búsqueda activa de casos, así como la búsqueda activa de contactos sexuales, reducirá el peso socioeconómico de la infección. La prueba ideal para diagnóstico tanto de cervicitis como de uretritis en el mismo procedimiento deberá utilizar muestra de orina y de cepillado cervical.Palabras clave: Chlamydia trachomatis, Prevalencia, Cervicitis, Uretritis, Chlamydia trachomatis Elisa IgG/ IgM/IgA, AMPLICOR CT PCR.
