The World Health Organization (WHO) has recognized obesity as one of the top ten threats to human health. It is estimated that the number of obese and overweight people worldwide exceeds the number of those who are undernourished. Obesity is not only a state of abnormally increased adipose tissue in the body, but also of increased release of biologically active adipokines. Adipokines released into the circulating blood, due to their specific receptors on the surface of target cells, act as classic hormones affecting the metabolism of tissues and organs. What is more, adipokines and cytokines may decrease the insulin sensitivity of tissues and induce inflammation and development of chronic complications. Certainly, it can be stated that in an era of a global obesity pandemic, adipokines may gain more and more importance as regards their use in the diagnostic evaluation and treatment of diseases. An extensive search for materials on the role of white, brown and perivascular fatty tissue and obesity-related metabolic and chronic complications was conducted online using PubMed, the Cochrane database and Embase.
In the present study, we have decided to evaluate whether serum interleukin 34 (IL-34) levels may have diagnostic value in predicting the risk of vascular diabetic complications. The study included 49 patients with type 2 diabetes mellitus (T2DM) and 23 high-risk group. The receiver operating characteristic (ROC) curve analysis has shown that IL-34 has more discriminatory power than C-reactive protein (CRP) for the risk of diabetic complications. The cut-off value for IL-34 was established as 91.2 pg/mL. The gist of our research was identification of IL-34 as an additional potential inflammatory biomarker for the prediction of the risk of vascular diabetic complications.
Zorena K. i inni: Ocena stężenia wybranych chemokin (CXCL1/GROα oraz CXCL8/IL8) w surowicy pacjentów z cukrzycą typu 1Vol. 13/2014 Nr 1(46) Endokrynologia Pediatryczna Pediatric Endocrinology 9Celem pracy była ocena stężeń wybranych chemokin (CXCL1/GROα) oraz interleukiny 8 (CXCL8/IL8) w surowicy pacjentów z cukrzycą typu 1 (T1DM). Pacjenci i metody. Badania przeprowadzono w grupie 59 pacjentów z T1DM, których podzielono na trzy grupy. W pierwszej grupie było 23 pacjentów z proliferacyjną retinopatią cukrzycową (PDR), w drugiej grupie -15 młodocianych pacjentów z T1DM oraz nieproliferacyjną retinopatią cukrzycową (NPDR), w trzeciej grupie -21 pacjentów z T1DM, lecz bez objawów retinopatii cukrzycowej. U wszystkich pacjentów z T1DM wykonano badania biochemiczne, całodobowy pomiar ciśnienia tętniczego, badania okulistyczne oraz oznaczono stężenia chemokin CXCL1/GROα oraz CXCL8/IL8 w surowicy krwi metodą immunoenzymatyczną (ELISA). Wyniki. U pacjentów z T1DM oraz PDR wykryto znamiennie wyższe stężenie CXCL8/IL8 w surowicy w porównaniu z grupą pacjentów z T1DM oraz NPDR jak też w porównaniu z grupą pacjentów z T1DM, ale bez objawów retinopatii cukrzycowej (226,9±58,3 vs 142,6±32,2pg/ml vs 89,3±15,9). Natomiast nie wykryto różnic znamiennie statystycznych porównując stężenia CXCL-1/Gro-α w surowicy w grupie pacjentów z T1DM i PDR z grupą pacjentów z T1DM oraz NPDR, a także w porównaniu z grupą pacjentów z T1DM bez retinopatii cukrzycowej (69±17 vs 53±12pg/ml vs 47±13). Analiza modelu wielokrotnej regresji krokowej wykazała, że czas trwania cukrzycy, HbA1c oraz białko CRP są niezależnymi czynnikami wpływającymi na poziom CXCL8/IL8 u pacjentów z retinopatią cukrzycową. Współczynnik determinacji dla tego modelu określono na poziomie R 2 =26%. Ponadto na podstawie analizy krzywych Receiver Operating Characteristic (ROC) dla surowiczego stężenia CXCL8/IL8 określono próg referencyjny AUC ROC na poziomie 218 pg/ml. Wnioski. Nasze wyniki badań wskazują na udział w reakcji zapalnej chemokiny CXCL8/IL8, lecz nie CXCL1/GRO-α u pacjentów z T1DM.
