Lichen sclerosus is a chronic skin condition with extragenital and/or genital involvement. The disease may develop in women and men of any age, but most frequently affects postmenopausal women. The aetiology of lichen sclerosus remains unknown. Genetic, infectious, environmental, autoimmune, and hormonal factors are presumed to influence its development. In most cases, lesions are located in the anogenital area. It is associated with sexual morbidity and is associated with an increased risk of cancerogenesis. The first-line treatment of lichen sclerosus includes ultrapotent corticosteroid ointments. Topical calcineurin inhibitors are considered second choice treatment. In recalcitrant cases alternative treatment methods (phototherapy and photochemotherapy, oral retinoids, methotrexate, cyclosporine, photodynamic therapy, adipose-derived stem cells and platelet-rich plasma, surgical management) are used. Because of a possible 3-8% risk of malignant transformation of genital lichen sclerosus to squamous cell carcinoma, early diagnosis is required, and long-term follow-up is recommended.streszczenie Liszaj twardzinowy jest przewlekłą chorobą skóry. Występuje w postaci pozagenitalnej i/lub genitalnej. Choroba jest stwierdzana u kobiet i mężczyzn niezależnie od wieku, ale najczęściej dotyczy kobiet w okresie po menopauzie. Etiologia liszaja twardzinowego jest nieznana. Przypuszcza się, że w jego rozwoju pewną rolę odgrywają czynniki genetyczne, zakaźne, środowiskowe, autoimmunologiczne oraz hormonalne. W większości przypadków zmiany są umiejscowione w okolicy anogenitalnej, co wiąże się z zaburzeniem funkcji seksualnych i możliwością kancerogenezy. W terapii pierwszego wyboru u pacjentów z liszajem twardzinowym stosuje się bardzo silnie działające maści z glikokortykosteroidami. Terapia drugiego wyboru opiera się na miejscowych inhibitorach kalcyneuryny. W przypadkach opornych na leczenie stosowane są alternatywne metody terapeutyczne (fototerapia i fotochemioterapia, doustne retinoidy, metotreksat, cyklosporyna, terapia fotodynamiczna, leczenie komórkami macierzystymi pochodzenia tłuszczowego i osoczem bogatopłytkowym, a także zabiegi chirurgiczne). Ze względu na ryzyko złośliwej przemiany postaci genitalnej liszaja twardzinowego w raka płaskonabłonkowego (3-8% przypadków) konieczne jest wczesne rozpoznanie i długoterminowa obserwacja pacjenta.
The present study retrospectively analyses the prevalence of late diagnosis in patients with newly-diagnosed HIV infection in Lodz, Poland from January 2009 to December 2016, and assesses the predictive factors associated with late presenters. Late presentation is defined as a diagnosis of HIV with a CD4 count<350 cells/μL, or the occurrence of an AIDS- defining event, regardless of the CD4 cell count. Two hundred and fifty-nine (62.86%) patients were late presenters, 178 of whom (68.72%) were advanced late presenters (CD4 cell count below 200 cells/μL). Multivariate factors associated with late HIV presentation included referral from physician for HIV testing (OR: 3.95, 95% CI 2.42-6.46), older age (OR: 1.81, 95% CI: 1.38-2.38) and route of HIV transmission. Heterosexual patients (OR 1.98, 95% CI: 1.01-3.90), active drug users (OR: 3.49, 95% CI: 1.63-7.48) and patients who did not report the route of transmission (OR: 4.29, 95%: CI 1.45-12.62) were more likely to present late than MSM subjects. In conclusion, the majority of HIV-infected patients are still diagnosed late. There is a need for expanded testing not only in MSM group, in which HIV prevalence is the highest, but also in intravenous drug users, or among subjects who are heterosexual or from a higher age group.
Excessive sweating (hyperhidrosis) is a common condition developing regardless of age, sex, and race, which often leads to a decrease in the quality of life. Depending on the cause, hyperhidrosis can be divided into the primary and secondary type (triggered by comorbidities, drugs, stress). Based on the degree of body involvement, hyperhidrosis may be either focal or generalised. Primary hyperhidrosis limited to certain body areas accounts for a vast majority of cases. According to the Canadian Hyperhidrosis Advisory Committee primary hyperhidrosis can be diagnosed if the symptoms persist for at least 6 months, and at least 4 of the following 6 criteria are fulfilled: bilaterally symmetric sweating, impairment of daily activities, more than 1 episode per week, age at onset < 25 years, positive family history, and absence of symptoms during sleep. First-line therapy of primary hyperhidrosis is based on topical medications. If no improvement is noted, systemic or surgical options are considered (e.g. iontophoresis, botulin toxin injections). In secondary hyperhidrosis, the focus is on the treatment of the underlying cause. streszczeNie Nadmierna potliwość jest chorobą, która występuje u osób wszystkich ras, płci, w różnym wieku i wielokrotnie jest przyczyną obniżenia jakości życia. Uwzględniając przyczynę, hiperhydrozę dzieli się na pierwotną oraz wtórną (wywołaną przez choroby współistniejące, leki, stres). Ze względu na zajmowany obszar ciała wyróżnia się hiperhydrozę uogólnioną oraz ograniczoną. W większości przypadków hiperhydroza ma charakter pierwotny i jest ograniczona do pewnych obszarów ciała. Według Canadian Hyperhidrosis Advisory Committee, aby rozpoznać hiperhydrozę pierwotną, należy potwierdzić utrzymywanie się objawów przez co najmniej 6 miesięcy oraz stwierdzić cztery z sześciu wymienionych kryteriów: występowanie obustronne i symetryczne, wpływ na codzienną aktywność, więcej niż 1 epizod w tygodniu, początek przed 25. rokiem życia, dodatni wywiad rodzinny, brak objawów podczas snu. Terapię hiperhydrozy pierwotnej rozpoczyna się od zastosowania preparatów miejscowych, a w przypadku ich nieskuteczności włączane jest leczenie ogólne, zabiegowe (np. jontoforeza, iniekcje toksyny botulinowej) oraz metody chirurgiczne. W terapii hiperhydrozy wtórnej nadrzędne jest leczenie przyczynowe.
Female genital lichen sclerosus is an underdiagnosed, distressing, chronic dermatosis affecting the well-being of women. The aim of this retrospective case-control study was to assess whether the disease is connected with work productivity and activity impairment, depression and decreased sexual quality of life. Fifty-one female patients with genital lichen sclerosus and forty-five healthy women were enrolled to the study and filled out an online survey including: Work Productivity and Activity Impairment: General Health (WPAI:GH), Patient Health Questionnaire-9 (PHQ-9) and The Sexual Quality of Life-Female (SQOL-F) questionnaires. The results showed that women with genital lichen sclerosus are at risk of having a diminished work productivity, are more often screened for depression and have a decreased sexual quality of life. The study highlights the importance of a multidisciplinary approach to treating female genital lichen sclerosus.
