ПРОФІЛАКТИКА ТА ЛІКУВАННЯ АБДОМІНАЛЬНОГО СПАЙКОУТВОРЕННЯ Резюме. Представлені результати лікування спайкової кишкової непрохідності за допомогою лапароскопічного адгезіолізису та кишкової симпатектомії, що базується на концепції вродженої або набутої схильності до патологічного спайкоутворення, зумовлену індивідуальною судинною реактивністю, яка створює тривалу ішемію очеревини й, як результат, хронізацію запальної відповіді на альтерацію очеревини. Метою дослідження було вивчення ефективності застосування лапароскопічного адгезіолізису в поєднанні з виконанням лівобічної кишкової симпатектомії для профілактики та лікування патологічного спайкоутворення. Для аналізу результатів було використано клінічні методи дослідження, а саме: терміни відновлення перистальтики та стулу, вираженність больового синдрому, тривалість госпіталізації. Метод апробовано в клінічних умовах на 58 хворих після експериментального вивчення. В якості контролю проаналізовано результати лікування 10 пацієнтів лапаротомічним доступом без виконання кишкової симпатектомії. Отримані результати свідчать про ефективність запропонованого методу профілактики та лікування патологічного спайкоутворення лапароскопічним методом. The aim: To conduct an analysis of the complex treatment of severe forms of rectal abscesses complicated by NF. Materials and methods: The results of treatment in 471 patients with deep forms of RA was performed. In 38(8%) the spread of the process and rotten-necrotic affection of the perineal fascia. Patients were treated at the surgical clinic of the medical faculty №2, VNPMMU, and Vinnytsya Clinical Emergency Hospital in the period from 2010-2018. Results: Total lethality 8(1,7%). Mortality in GF was 8(19,5%). It is worth noting the reduction of the treatment duration against the background of the modern technologies usage in the period from 2016 to 12 days in relation to the total figure of 15 days. Conclusions: Early surgical treatment, adequate necrectomy, fasciotomy and antibacterial therapy stop the necrotic process. The active aspiration reduces the timing of wound cleansing and further ensures the accelerated implementation of reconstructive surgical interventions.
The aim: To evaluate the effectiveness of the proposed method of surgical treatment of patients with acute purulent cholangitis. Materials and methods: The research is based on the analysis of the treatment results in 104 patients with acute purulent cholangitis. The volume of surgical interventions on the biliary tract in patients was as follows: endoscopic papillosphincterotomy (EPST) + naso-biliary drainage; EPST + laparoscopic cholecystectomy; cholecystectomy + choledocholithotomy + drainage of the common bile duct. Results: The APACHE-2 scale assessment for patients in the control group was 12,80 ± 1,73, with the SOFA scale - 4,32 ± 0,31 points. Accordingly, the assessment for patients in the main group with the APACHE-2 scale is 11.76 ± 0.81, with the SOFA scale - 4.33 ± 0.79 points. The results obtained in both the control and the main group indicate that the physiological reactions of the organism to the disease, treatment, development of organ dysfunction are reflected in the integrated severity assessment scales. Conclusion: The performed analysis of clinical and laboratory results of treatment in both groups established the high efficiency of the proposed treatment method in patients with acute purulent cholangitis, which promotes the more rapid medical and social rehabilitation in this category of patients.
