Due to the modern level of development of medicine, more and more patients survive after severe acute diseases. However, some of the patients remain dependent on life support systems. Therefore, a chronic critical condition is an urgent problem for patients in the intensive care unit. The main syndrome of this complex is maladaptive response to long-term existing stress factors. This review examines the main pathophysiological mechanisms of chronic critical illness, as well as the resulting metabolic disorders. Emphasis is placed on the systemic inflammatory reaction, the reaction of hypermetabolism - hypercatabolism, disorders of carbohydrate, protein and fat metabolism, the development of secondary sarcopenia. In conclusion, the volume of metabolic control and intensive nutritional and metabolic support, which are an important component of a comprehensive approach to the treatment of chronic critical illness, are also considered.
ВВеДеНИеДанная статья посвящена обсуждению саркопении у пациентов после тяжелого повреждения головного мозга (ГМ). В статье представлены результаты сравнительного анализа группы пациентов и группы добровольцев с проведенным ультразвуковым анализом мышечного волокна. АКТ УАльНОСТьСаркопения является тяжелейшим осложнением у пациента в критическом состоянии. Появляется рано и достаточно быстро прогрессирует в течение нахождения пациента в критическом состоянии. С целью диагностики саркопении могут быть использованы как лучевые, так и ультразвуковые методы. Использование ультразвуковых методов является менее трудо-, энерго-и экономически затратным и не сопряжено с повышением лучевой нагрузки на пациента. В работе освещено использование и сравнение данных методов у пациентов после тяжелых повреждений ГМ. Цель ИССлеДОВАНИяОценить степень выраженности саркопении у пациентов после тяжелых повреждений ГМ. МАТеРИАл И МеТОДыВ данное исследование включены 25 пациентов после тяжелых повреждений ГМ в минимальном сознании, средний возраст которых составил 56,75±19,84 года (диапазон от 22 до 82 лет), при оценке по шкале FOUR (Full Outline of Unresponsiveness) медиана 12 (12; 15) баллов. Оценку проводили в первые 3 дня от момента поступления в ФНКЦ РР. С целью сравнения в исследования включены 19 добровольцев в возрасте 35,63±7,02 года (диапазоне от 21 до 47 лет). РезУльТАТыПолученные данные свидетельствуют о том, что пациенты после тяжелых повреждений ГМ имели выраженные нарушения мышечного волокна: его толщину и эхогенность. Толщина бицепса со стороны повреждения ГМ была равна 0,93±0,27 см (min 0,5; max 1,58), а со стороны, противоположной от повреждения ГМ, -0,62±0,2 см (min 0,27; max 0,93) с p=0,0007, статистически значимо. По эхогенности отличия не были статистически значимы (p=0,1). Толщина трицепса со стороны повреждения ГМ составила 0,5±0,17 см (min 0,25; max 0,82), а с противоположной стороны от повреждения ГМ -0,38±0,14 см (min 0,2; max 0,8) с p=0,028, статистически значимо; при этом степень эхогенности по шкале Modified Heckmatt scale со стороны повреждения ГМ -2,5 [2,0; 3,0] (min 2,0; max 4,0), а с противоположной стороны -3,0 [3,0; 4,0] (min 2,0; max 4,0), p=0,01. Толщина плечелучевой мышцы со стороны повреждения ГМ составила 0,59±0,15 см (min 0,39; max 0,92), с противоположной стороны -0,50±0,17 см (min 0,25; max 0,86), p=0,06, статистически значимо; при этом степень эхогенности оказалась равной 2,0 [2,0; 3,0] (min 1,0; max 4,0) -со стороны повреждения ГМ, а с противоположной стороны от повреждения ГМ -2,5 [2,0; 4,0] (min 2,0; max 4,0), p=0,03, статистически значимо. Выраженные статистически значимые отличия также получены по толщине прямой мышцы бедра (p=0,06) и ее эхогенности (p=0,017). При сравнении данных показателей с мышцами здоровых добровольцев по всем указанным показателям p был менее 0,05, в большинстве же случаев p=0,000001 (статистически значимо). BACKGROUND: The resolution of intestinal insufficiency syndrome remains one of the urgent problems of nutritional support for patients in chronic critical condition. Purpose of the study. To determine the effectiveness of intestinal lavage in restoring the basic functions of the gastrointestinal tract and microbiocenosis in patients in chronic critical condition. Materials and methods. A prospective, randomized, single-center study was performed. The study was performed in 56 affected men with TBI of various etiologies in a chronic critical condition. At the beginning of treatment and on the 10th day, the effectiveness of gastrointestinal lavage was evaluated in the complex intensive therapy of intestinal insufficiency syndrome. Results. Patients were randomized into two groups depending on the conduct of intestinal lavage. In the 1st group (27 patients), enteral tube feeding was performed according to the standard method. In the 2nd group (29 patients), intestinal lavage was additionally performed with the addition of an enterosorbent to the saline enterosorbent solution, 3 procedures with an interval of 3 days. In the 2nd group (main), there was a significant positive dynamics of carbohydrate and protein metabolism, restoration by the 10th day of the barrier function of the intestine, motility of the gastrointestinal tract, microbiocenosis compared with patients in the control (group 1). Conclusion. The inclusion of gastrointestinal lavage in the complex intensive therapy of SCI in patients in a chronic critical condition in a short time helps to resolve the main pathogenetic links of SCI - restoration of the barrier function of the intestine, digestive-transport, motor-evacuation processes in the gastrointestinal tract, normalization of microbiocenosis, metabolic parameters and nutritional status.
Introduction. The frequency of post-traumatic hydrocephalus is 3.9%. The incidence of post-traumatic defects of the skull is 10.46 per 100,000 per year. Overdrain syndrome occurs in 1012% of cases in patients with long-term ventricular shunting. The presence of a cranial defect causes a violation of blood flow and cerebrospinal fluid dynamics in the area of the defect, a displacement of brain structures under the influence of gravity and atmospheric pressure, which can cause a deterioration in the patients condition and a significant slowdown in recovery after a severe head injury. Both craniotomy syndrome and excessive shunting can impede the verticalization and rehabilitation of patients with post-traumatic hydrocephalus and post-traumatic defects of the bones of the cranial vault. Description of the clinical case. Clinical case demonstrates an example of a differential approach and an algorithm for deciding on surgical treatment in a patient with post-traumatic hydrocephalus and cranial bones defect in case of deterioration during attempts at verticalization in a complex of rehabilitation measures. Conclusion. The clinical manifestations of trephine skull syndrome and syndrome of shunt overdrain in the patient after severe traumatic brain injury combines post-traumatic hydrocephalus, may be similar. And not always, as demonstrated in this clinical case, narrowed ventricles and the relationship of deterioration to verticalization should be interpreted as a syndrome of excessive drainage of the shunt system. The plastic surgery of the defect of the bones of the cranial vault performed in this case made it possible to improve the patients condition and regress symptoms.
