Intravenous iloprost demonstrated no additional benefit over the conventional management of infants with PAH after repair of intracardiac defects. Clinicians may prefer other alternative agents in infants with a high risk of PAH crisis.

ÖZAmaç: Bu çalışmada şilotoraksın insidansı, tedavi protokolleri ve pediatrik kardiyovasküler cerrahi sonrası erken dönem bulgular üzerindeki etkisi değerlendirildi. Ça lış ma pla nı: Haziran 2011 -Haziran 2014 tarihleri arasında kliniğimizde kalp ameliyatı geçiren 1123 pediatrik hastanın kayıtları retrospektif olarak incelendi. Ameliyat sonrası şilotoraks gelişen 35 hasta (19 erkek, 16 kız; ortanca yaş 240 gün; dağılım 5 gün-15 yıl) çalışmaya dahil edildi. Bul gu lar: Şilotoraks gelişimi en sık tek ventrikül tamirlerinde saptandı (n=11, %31). Bunu aortik ark tamiri (n=9, %25), Fallot tetralojisi (n=4, %11) atriyoventriküler septal defekt tamirleri (n= 4, %11), arteriyel switch ameliyatları (n=3, %8), ventriküler septal defekt tamirleri (n=2, %6) ve diğer ameliyatlar (n=2, %6) izledi. Ortanca şilotoraks drenaj süresi dokuz gün (dağılım 2-44 gün) idi. Beslenme yönetimi yağdan fakir diyet, orta zincirli yağ asitlerinden zengin enteral beslenme ve total parenteral beslenmeyi içerdi. On dört hastaya oktreotid, dört hastaya duktus torasikus ligasyonu ve dört hastaya plörodezis uygulandı. Dokuz hastada sepsis gözlendi ve üç hasta kaybedildi. So nuç:Pediatrik kardiyak cerrahi sonrası gelişen şilotoraks nadir olmayan bir komplikasyondur. ABSTRACT Background:This study aims to evaluate the incidence, treatment protocols, and impact of chylothorax on early outcomes after pediatric cardiovascular surgery. Methods: Records of 1,123 pediatric patients who underwent heart operation at our clinic between June 2011 and June 2014 were reviewed retrospectively. Thirty-five patients (19 males, 16 females; median age 240 days; range 5 days to 15 years) who developed postoperative chylothorax were included in the study. Results: Development of chylothorax was detected most frequently in single ventricle repairs (n=11, 31%). It was followed by aortic arch repairs (n=9, 25%), tetralogy of Fallot (n=4, 1%), atrioventricular septal defect repairs (n=4, 11%), arterial switch operations (n=3, 8%), ventricular septal defect repairs (n=2, 6%), and other operations (n=2, 6%). Median duration of drainage of chylothorax was nine days (range, 2 to 44 days). Nutritional management included low lipid diet, enteral feedings enriched with mediumchain triglycerides, and total parenteral nutrition. Fourteen patients were administered octreotide, four patients were administered thoracic duct ligation, and four patients were administered pleurodesis. Sepsis was observed in nine patients and three patients died. Conclusion: Chylothorax developing after pediatric cardiac surgery is not a rare complication. It occurs most frequently after single ventricle repair and has a significant impact on postoperative clinical course and morbidity.

Background Prolonged pleural drainage is a common complication after undergoing the Fontan procedure. Although various protocols have been described, there is no definitive consensus for how to treat this complication. Materials and Methods Our primary aim was to determine the effect of the management strategy protocol on the duration of drainage and length of hospital stay. Our secondary aim was to determine the parameters affecting the need for prolonged drainage after the Fontan procedure. Ninety‐two consecutive patients who underwent the Fontan procedure were retrospectively analyzed. A protocol‐based postoperative management strategy was adopted in July 2018. Group 1 (n = 48) consisted of patients that underwent the procedure before the protocol was implemented. Group 2 (n = 44) consisted of patients that underwent the procedure after the protocol was implemented. Results The mean age was 5 years (interquartile range [IQR], 4.0‐6.9); the mean body weight was 17.3 kg (IQR, 15.1‐21.8). Statistically significant differences were found between the groups in terms of total drainage, duration of pleural drainage, prolonged drainage, and length of hospital stays (LOHS) (P = .05, P = .04, P = .04, P = .04, respectively). The multivariate analysis results showed that the application of the protocol was the only factor impacting prolonged drainage (OR, 2.46, 95% CI lower‐upper: 1.03‐5.86, P = .04). Conclusion Standardization and strict application of the medical treatment within a specific protocol without being affected by doctor‐, nurse‐, or patient‐based factors increases the success rate of this procedure. After implementing the changes in the medical management strategy, total drainage and duration of pleural drainage and LOHS decreased, and the costs associated with these factors also decreased.

ÖZAmaç: Bu çalışmada trakeostomi uygulanan pediyatrik kardiyak yoğun bakım ünitesindeki hastalara ilişkin veriler analiz edildi.Ça lış m a pla nı: Ocak 2010 -Ocak 2015 tarihleri arasında tek bir merkezde pediyatrik kardiyak yoğun bakım ünitesinde yatan ve hastanede yatış döneminde trakeostomi uygulanan toplam 18 hasta (10 kız, 8 erkek; ort. yaş 9.4±10.5 ay; dağılım 2-42 ay) retrospektif olarak incelendi.Bul gu lar: Başlangıçta üç hastada genetik bozukluk saptandı. Kardiyak cerrahi uygulanan 1450 hastanın 10'una total düzeltme ve sekizine palyasyon yapıldı. Trakeostomi uygulanmadan geçen ortalama entübasyon süresi 41.0±12.9 gün ve ekstübasyon girişimi sayısı 4.2±0.8 idi. İki hastada nörolojik hasar vardı. Trakeostomi sonrası hastaların hiçbirinde pnömomediastinum, pnömotoraks, yara yeri enfeksiyonu ya da mediastinit gelişmedi. Bir hastada kanama nedeniyle trakeostomi revizyonu yapıldı. Hastaların hiçbirinde takip döneminde trakeal darlık gibi geç dönem trakeostomi komplikasyonları gözlenmedi. On sekiz trakeostomi hastasının altısı pediyatrik kardiyak yoğun bakım ünitesinde ve biri ise taburculuk sonrasında evde kaybedildi (mortalite %38.8). Taburcu edilen 12 trakeostomi hastasının sekizine dekanülasyon yapıldı. Üç hasta halen ev tipi ventilatör ile evde takip edilmektedir. So nuç:Trakeostomi işlemleri kardiyak cerrahi sonrası entübasyon süresi uzamış olan hastalarda düşük komplikasyon oranı ile uygulanabilir.
