The analysis of the maintenance of a metabolite of vitamin D (25dihydroxyvitamin D) and vitamin D-binding protein in the blood serum of children under 3 years, patients with pneumonia. Found that a child with pneumonia is characterized by a significant decrease in blood serum concentrations of 25-dihydroxyvitamin D and vitamin D-binding protein. Additional risk factors for pneumonia in young children are adverse for the 2nd half of the pregnancy, the risk of miscarriage, the acute respiratory infections during pregnancy, a small body weight at birth; asphyxia at birth, a prematurity, the artificial ventilation in the neonatal period , artificial nutrition in the first year of life, the disease rickets, anemia, delayed motor development.
Резюме. Проведено комплексне обстеження 30 дітей, хворих на позалікарняну пневмонію, віком від 2 місяців до 3 років, серед яких у 18 дітей захворювання було викликано Streptococcus pneumoniae, а в інших 12 пацієнтів -грамнегативною флорою. Всім дітям проводилася оцінка тяжкості стану за шкалою PRESS, за результатами якої встановлено, що перебіг пневмонії пневмококової етіології в більшості випадків був тяжким. Проведений аналіз показав, що розвиток пневмококової пневмонії у дітей на відміну від пневмоній, етіологічним чинником яких були грамнегативні патогени, відбувався на фоні зниження вмісту в сироватці крові метаболітів вітаміну D та активності антимікробних пептидів. В сироватці крові дітей з пневмококовою пневмонією встановлено зниження вмісту β 1 -дефензину в 2,6 раза, LL-37в 3,7 раза та hBPI -в 2,8 раза порівняно з контрольною групою (р < 0,05). Доведено, що недостатня активація антимікробних пептидів на фоні дефіциту метаболітів вітаміну D у дітей раннього віку, хворих на пневмонію, викликану Streptococcus pneumoniae, є однією з патогенетичних ланок, що призводить до тяжкого перебігу захворювання.
У статті розглядається сучасний стан проблеми антибактеріальної терапії бактеріальних інфекцій дихальних шляхів у дітей. Науково обґрунтоване застосування при патології органів дихання препарату з групи цефалоспоринів ІІІ покоління — цефподоксиму проксетилу (Цефодокс). Наведено досвід зарубіжних та вітчизняних фахівців, а також власний досвід застосування цефподоксиму проксетилу в терапії респіраторних бактеріальних інфекцій у дітей.
