Вступ. Традиційна методика секторальної резекції передбачає стягування країв рани, при цьому візуально виявляється ефект втягнення тканини до середини органа, що в деяких випадках призводить до значної тканинної деформації. Матеріали методи. Проаналізовано результати лікування 67 пацієнток із доброякісними захворюваннями молочних залоз за період 2010–2019 рр., яким виконано операції на молочній залозі в об’ємі до 1 квадранту. Перед хірургічними втручаннями проводили розрахунок меж деепідермізації для виконання мастопексії. Ефективність методу оцінювали за показниками якості життя. Результати. Встановлено, що запропонована методика, яка передбачає передопераційне планування з наступною мастопексією, не збільшує число ранніх післяопераційних ускладнень, а у віддаленому періоді покращує якість життя. Обговорення. На сьогодні існують лише поодинокі повідомлення про застосування методів пластичної хірургії в лікуванні пацієнток із доброякісними захворюваннями молочної залози. Також не достатньо вивчений вплив операцій на якість життя жінок. Висновки. На етапі передопераційної підготовки хворих необхідно проводити ультразвукове дослідження молочної залози із розрахунком об’єму тканин для резекції та можливості виконання мастопексії.
У роботі представлений огляд участі пролакти-ну у розвитку патології молочної залози. Актуаль-ність проблеми визначається загальним збільшен-ням випадків захворювання на рак молочної зало-зи. Порушення гормональної регуляції є важливим фактором розвитку раку молочної залози та пух-лин інших локалізацій у репродуктивній системі людини. Значну роль в цьому дисбалансі відіграє саме гормон пролактин.Метою роботи є проведення аналізу літератури для встановлення ролі пролактину у розвитку пато-логії молочної залози.Для патології молочної залози основне значен-ня має підвищений рівень пролактину у біологічних рідинах та тканинах організму. Загалом, гіперпро-лактинемія може впливати не тільки на патологію молочної залози, а й викликати порушення у репро-дуктивній системі, викликаючи ановуляторні цикли, аменорею та розвиток безпліддя. Патологічні про-цеси в молочній залозі виникають не тільки за раху-нок пролактину, який циркулює в крові, але й про-лактин, який секретується власне залозистою ткани-ною. Це є одним із факторів канцерогенезу та має велике значення у розвитку раку молочної залози.Отже, подальше дослідження патогенетичних механізмів впливу пролактину на тканину молочної залози може сприяти розробці нових методів про-філактики та лікування раку молочної залози.Ключові слова: рак молочної залози, про-лактин, експресія рецепторів пролактину, гіперпро-лактинемія.Зв'язок роботи з науковими програмами, планами, темами. Патологія молочної залози (МЗ) є важ-ливою сучасної проблемою через значну пошире-ність і тенденцію до зростання [1]. Особливо небе-зпечним є невпинне зростання випадків раку мо-лочної залози (РМЗ) у країнах Північної Америки, Європи, і в тому числі в України. За даними Націо-нального канцер-реєстру, у 2014-2015 роках було зареєстровано 13641 випадків захворювання [2]. Ця проблема має значне соціально-економічне значення для суспільства, тому дослідження при-чин, механізмів розвитку злоякісних пухлин МЗ можна віднести до пріоритетних завдань наукових пошуків.Останнім часом підвищилась увага дослідників до ролі гормону пролактину (ПРЛ) у розвитку пато-логії МЗ [1,2].Пролактин -це поліпептидний гормон, який синтезується та виділяється зі спеціалізованих клітин лактотрофів передньої частини гіпофіза. Зв'язуючись з мембранним рецептором пролакти-на (ПРЛ-Р), регулює через сигнальні шляхи репро-дуктивну, імунну та метаболічну функції [3]. Поза гіпофізом ПРЛ cекретується ендометрієм, дециду-альною оболонкою, лімфоцитами, центральною нервовою системою, молочною залозою і проста-тою, де виступає в якості цитокіна [3,4]. У молочній залозі ПРЛ існує, як циркулюючий гіпофізарний гормон та у вигляді гормону, який продукується протоковим епітелієм. В клітинах МЗ пролактин зв'язується з клітинною поверхнею рецептора ПРЛ-Р та активує сигнальні шляхи Jak2 / Stat5, Map Kinase, PI3K-Akt [...
Вступ. Проліферативна доброякісна дисплазія (ДД) грудної залози (ГЗ) належить до передпухлинних станів. Хірургічне втручання з приводу ДДГЗ є основним методом лікування. Мініінвазійні хірургічні технології є затребуваними. Матеріали і методи. Проаналізований 3-річний досвід мініінвазивного хірургічного лікування хворих з приводу протокового папіломатозу ГЗ. Проведений порівняльний аналіз ступеня проліферації дисплазії та експресії альфа–рецепторів естрoгену (EsRα). Результати. Встановлено ефективність запропонованого способу лікування протокового папіломатозу ГЗ. Доведено залежність між ступенем проліферації проток ГЗ і експресією EsRα. Обговорення. Сьогодні універсального способу хірургічного лікування протокового папіломатозу ГЗ немає. Використання сучасних методів імуногістохімії та індивідуалізація способів втручання сприятимуть покращенню результатів лікування. Висновок. Мініінвазивні методи хірургічного лікування забезпечують радикальне лікування протокового папіломатозу ГЗ, естетичне задоволення пацієнток і лікаря.
