Summary. Introduction. Over the past two decades, such traditional surgical procedures as crossectomy and stripping have been replaced to a greater or lesser extent by less invasive endovenous methods. Unfortunately, endovenous ablation methods do not allow to completely exclude such complications as: hyperpigmentation, paresthesias, hematomas, thermal damage to the skin.
Purpose.To improve the tactics and improve the cosmetic effect of surgical treatment of superficially located suprafascial segments of the great saphenous vein in patients with varicose veins of the lower extremities.
Materials and methods. An analysis of the treatment of 107 patients with VDLE at the University Clinic of ZDMU in 2019-2021 was carried out. The first (main) group – 65 (60.75 %) patients – received surgical treatment according to the proposed method – RFA, miniphlebectomy of the suprafascially located main trunk, miniphlebectomy of the tributaries along the Varada. The second (comparison) group – 42 (39.25 %) patients – received standard treatment – RFA of the GSV trunk, miniphlebectomy.
Research results. According to the control duplex scan in all patients, the deep vein passage was occluded in 106 (99.1 %) patients.
The overall frequency of postoperative complications in the main group was 18.4 % (12 out of 65 patients), in the comparison group – 35.7 % (15 out of 42 patients, mainly due to hyperpigmentation) and was significantly different (р ˃ 0.05).
According to the data of CIVIQ-2, the proposed method of surgical intervention made it possible to improve the quality of life from (31.72 ± 1.56) points in patients of the comparison group to (27.49 ± 1.45) in patients of the main group.
Conclusions. The proposed tactic of surgical correction of the varicosity of the trunk of the GSV – RFA of the intrafascial area of the GSV plus miniphlebectomy of the suprafascial segment – allows to reduce the probability of the development of hyperpigmentation, which contributes to a better cosmetic effect, subjective satisfaction of patients with the results of the operation, and a better quality of life in patients with VDLE.
Известный способ радикального лечения варикозной болезни нижних конечностей заключается в одномоментном выполнении флебэктомии ствола большой и малой сафеновых вен, коллатералей с субфасциальной перевязкой перфорантных вен, однако характеризуется высокой травматичностью. С целью разработки малотравматичного способа хирургического лечения обследовали 97 пациентов с варикозной болезнью нижних конечностей класса С2-С6 по CEAP. Объем предложенной операции: кроссэктомия, короткий стриппинг ствола большой сафеновой вены с удалением перфоранта Boyd, минифлебэктомия по Müller. Установлено, что способ отличается меньшим травматизмом и продолжительностью операции, позволяет сохранить неизмененные подкожные вены, отличается лучшим косметическим эффектом, что свидетельствует о целесообразности использования предложенного способа лечения.
Цель работы-изучить у больных варикозной болезнью нижних конечностей (ВБНК) взаимосвязь показателей эндотелиальной дисфункции с клиническим классом заболевания до и после операции. Материалы и методы. Обследовали 69 больных варикозной болезнью нижних конечностей. Методом иммуноферментного анализа определяли уровни гомоцистеина, нитротирозина и eNOS до и в отдаленные сроки после операции. Результаты. Изучили динамические изменения показателей эндотелиальной дисфункции у 69 больных варикозной болезнью нижних конечностей, обследованных до и после операции. Определена достоверная взаимосвязь средних значений показателей гомоцистеина, нитротирозина и eNOS с клиническим классом ВБНК. Выводы. Отмечено достоверное снижение показателей гомоцистеина и нитротирозина и повышение eNOS в отдаленные сроки после операции в сравнении с исходными значениями. Установлено, что значения гомоцистеина относительно выше коррелируют с возрастом пациента, чем с клиническим классом ВБНК, а нитротирозина и eNOS-относительно выше коррелируют с классом ВБНК, чем с возрастом пациентов.
Abstract.The results of the examination and surgical treatment of 132 patients with CEAP class C2-C6 varicose disease of the lower extremities have been analyzed. A combined diagnostic method that allows detecting the peculiarities of the functioning of the perforating vein venous system of the shin, namely, allows establishing the uncompetency of the perforating veins of the shin objectively, has been developed. In view of the examination results obtained, a method of radical surgical treatment has been developed that allows individualizing, standardizing and synchronizing the extent of the surgical intervention depending on the stage of the disease, which results in a decrease in the injury rate and the duration of the operative intervention, ensuring the best results in the near and remote postoperative periods. A change in the CEAP classification has been proposed in order to adapt it to the surgical approach with a need to single out three clinical variants of stage С6: С6a, С6b and С6c, as well as the need to verify the competent "re-entry" perforating veins and the uncompetent perforating veins of the shin while designating them with "Apc" and "Apun" symbols respectively. In view of the innovations proposed an algorithm of the approach and selection of the extent of the surgical treatment of varicose disease depending on its clinical course has been developed.
У роботі наведені результати амбулаторного малоінвазивного лікування пацієнтів із варикозною хворобою нижніх кінцівок, ускладненою тромбофлебітом. Встановлено, що радіочастотна абляція в поєднанні з мініфлебектомією є ефективним методом лікування за рахунок радикального видалення патологічного вогнища, одночасної ліквідації загрози рецидивуючого плину захворювання, прискорення термінів реабілітації, має гарний косметичний ефект та може бути альтернативою відкритих хірургічних втручань.Ключові слова: варикозна хвороба, тромбофлебіт, радіочастотна абляція, флебектомія.
