To determine the frequency of interleukin-4 (IL-4) (C-589T, rs2243250) single nucleotide polymorphism in children with bronchial asthma (BA) and allergic rhinitis (AR) and with isolated or allergic rhinitis-induced comorbid malocclusion. Materials and methods.Single nucleotide polymorphism of the IL-4 (C-589T, rs2243250) gene was analyzed in 170 children aged between 6 and 17 years, 11 months, 29 days. Group I included 89 children with BA; Group II consisted of 31 children with AR; Group III was composed of 27 children with AR and distal occlusion (DO); Group ІV comprised 23 children with malocclusion. Genotyping was performed using a commercial "SNP-express RT" kit by real-time polymerase chain reaction method (Applied Biosystems, USA) via TagMan ® SNP Genotyping Assay on an amplifier CFX96 TM Real-Time PCR Detection System (Bio-Rad Laboratories, Inc., USA).DNA was isolated using a commercial DNA-express kit ("LLC Research and Production Company LITEKH"). The general, recessive and dominant models of inheritance and the odds ratios with a 95 % confidence interval were used for the analysis. The results analysis was conducted using the Statistica 6.0 RU licensed software package.Results. IL-4 (C-589T, rs2243250) gene polymorphism in children with allergy and malocclusion living in Zaporizhzhia was analyzed for the first time. The frequency of C/C -C/T -T/T genotypes registration was 69.66 % -22.47 % -7.87 % of cases in children with BA; 58.06 % -38.71 % -3.23 % in children with AR; 62.96 % -29.63 % -7.40 % in AR with DO. On the contrary, in children with malocclusion, the C/C (34.78 %) and T/T (4.35 %) genotypes were registered less frequently and the C/T genotype (60.87 %) was recorded more often as a genetic feature of the DO phenotype.Conclusions. The C/C genotype of IL-4 (C-589T, rs2243250) gene was associated with bronchial asthma (OR = 4.31; 95 % CI = 1.63-11.36; P = 0.002) and allergic rhinitis (OR = 4.32 (95 % CI = 1.04-7.81; P = 0.04), in comparison with the fact that the C/T + T/T genotype indicated a predisposition to malocclusion development.Характеристика поліморфізму гена інтерлейкіну-4 (С-589Т, rs2243250) у дітей із бронхіальною астмою, алергічним ринітом і з ортодонтичною патологією, яка ізольована чи коморбідна з алергічним ринітом Т. Характеристика побутових предикторів персистуючого перебігу бронхіальної астми у дітей-підлітків та алергічного риніту у дітей із дистальним прикусом Т. Є. Шумна, С. М. Недельська, О. С. Зінченко Мета роботи -визначення побутових предикторів розвитку бронхіальної астми в дітей-підлітків та алергічного риніту в дітей із дистальним прикусом, що мали персистуючий клінічний перебіг захворювання.Матеріали та методи. Здійснили клініко-анамнестичне та алергологічне дослідження 87 дітей підліткового віку (від 13 до 17 років). У І групу спостереження ввійшли 34 підлітки із персистуючим перебігом бронхіальної астми, ІІ групу -28 дітей із персистуючим алергічним ринітом і дистальним прикусом, які додатково лікували супутню зубощелепо-лицьову патологію в лікаря-ортодонта. Контрольна група спостереження (ІІІ) -25 здорових дітей. Співвідношення хлопчиків і дівчаток у групах спостереження та контролю було однаковим (р > 0,05). За анкетуванням вивчили анамнестичні дані щодо побутових умов проживання, соціально-гігієнічний стан житла: наявність домашніх тварин, частоту та особливості використання певних хімічних мийних і косметичних засобів у дітей. Проаналізували клінічний перебіг захворювань. Під час диспансерного спостереження в періоді ремісії дітям із персистуючим перебігом бронхіальної астми та алергічного риніту за відсутності протипоказань до тестування in vivo з алергенами виконали алергологічне дослідження шляхом постановки шкірних прик-тестів із побутовими й епідермальними алергенами виробництва ТОВ «Імунолог» (м. Вінниця). Порівнюючи статистичні сукупності, використовували непараметричний статистичний метод «2×2 Table», the Chi-square (df = 1) та розраховували відношення шансів (ВШ) з використанням чотирьохпільної таблиці з розрахунком довірчого інтервалу (ДІ) за методом Woolf. З використанням непараметричного методу gamma correlation (рекомендується використовувати, коли в даних є багато значень, котрі збігаються) виконали аналіз зв'язку між показниками гіперчутливості до алергенів. Розбіжності р < 0,05 при порівнянні показників у групах спостереження та контрольній вважали статистично вірогідними.Результати. Персистуючий перебіг хронічного алергічного респіраторного синдрому середнього ступеня тяжкості вірогідно частіше реєстрували в дітей з алергічним ринітом і дистальним прикусом (75,00 %), ніж у підлітків із бронхіальною астмою Purpose. The study of the distribution patterns of allelic genes and genotypes of the C/A polymorphism of the collagen CO-L1A1_1 gene (rs1107946) in children with bronchial asthma, taking into account the indicators of external respiration function. Materials and methods. Molecular-genetic study to determine the C/A polymorphism of the collagen COL1A1_1 gene (rs1107946) was conducted in 125 children from 6 to 18 years, 100 of them with bronchial asthma who were hospitalized in the Allergic Department of the Municipal Non-Profit Enterprise "City Children Hospital No 5 of Zaporizhzhia City Council" and 25 healthy children (control group). There were no differences in age and sex in the comparison groups (P > 0.05).Genotype determination was performed by polymerase chain reaction method according to the instruction (Applied Biosystems, USA) using the samples of total DNA received from whole venous blood using SNP-Screen reagents (manufactured by "Syntol") on amplifier CFX96TM Real-Time PCR Detection Systems ("Bio-Rad laboratories, Inc.", USA). The ventilation function of the lungs was studied by conducting a spirometric study on a computer spirograph "PULMOREM" TU U 33.1-02066769-005-2002 (Kharkiv, Ukraine). To compare the frequencies of alleles and genotypes in different groups, the non-parametric statistical method "2 × 2 Table", the Chi-square (df = 1) was used. Medians and interquartile intervals were also calculated, the two independent groups were compared by the Mann-Whitney criterion, the χ 2 criterion. Non-parametric statistics methods for the licensed software package Statistica for Windows 6.1.RU, serial number АХХR712D833214SAN5, were used to process the obtained study data. Results. Molecular-genetic study of distribution patterns of allelic genes of the C/A polymorphism of the COL1A1_1 collagen gene (rs1107946) in patients with bronchial asthma and in practically healthy children, showed that the allele C was registered with a frequency of 69.5 % and 84.0 %, allele A-30.5 % and 16.0 %; dominant genotype C/C-58 % and 76 %; heterozygous genotype C/A-23 % and 16 %; homozygous genotype A/A-19 % and 8 %, respectively. Children with bronchial asthma with genotype A/A had significantly lower FVC values up to 2.32 (1.55; 3.29), VC max up to 1.69 (1.40; 2.98), FEV 1 up to 1.82 (1.43; 2.98), with genotype C/C-MEF 75 up to 2.34 (1.87; 3.14) when compared with patients with heterozygous genotype A/C, and very low rates of FVC were recorded in 68.75 % of children with bronchial asthma with the A/A genotype against 30.77 % of patients with the A/C genotype and 36.17 % with the C/C genotype (P < 0.05).
