The aim of this research was a study of gene LEPR polymorphism rs1137101 role in diabetes mellitus 2 type development and his influences on the clinical and laboratory data, related to obesity for these patients. 103 patients are inspected, that had the clinically expressed complications. A control group was made by 100 practically healthy persons of corresponding age. The analysis of polymorphic DNA-loci was carried out by the method of polymerase chain reaction in real time (TaqMan® SNP Genotyping Assay). Results showed that allelic polymorphism of rs1137101 (223Gln>Arg) had been associated with diabetes mellitus 2 types development: allele Gln promotes, while allele Arg-reduces (OR=1.5; P=0.039) the risk of its development. Presence allele Gln for diabetic patients assists development of obesity and his displays, namely-to the higher values of body mass index and HOMA-IR, and also to the greater leptin and triglycerides blood levels on a background reduction of high-density lipoproteides maintenance. The risk of developing obesity in these patients is increased by 1.8 times.
ВСТУПу розвитку діабетичної ретинопатії (др) при цукровому діабеті 2 типу (цд2Т) значну роль відіграють генетичні чинники [1][2][3][4]. відповідно до сучасних даних, генетичним факторам відводять до 50% ризику розвитку др. генетичні чинники можна вважати есенціальними факторами виникнення ускладнень цукрового діабету, зокрема -др [5, 6]. рядом досліджень показана наявність спадковості при розвитку др в різних популяціях, незалежно від рівня гіперглікемії та супутніх факторів ризику навколишнього середовища [7, 8].патогенез др включає багато факторів і, в першу чергу -порушення обміну речовин: вуглеводного, ліпідного, білкового та електролітного. Тривала гіперглікемія активує поліоловий шлях метаболізму глюкози, внаслідок чого у клітинах накопичуються сорбітол та фруктоза. при відсутності гіперглікемії перетворення глюкози на сорбітол не перевищує 1%, але при цд2Т цей відсоток збільшується до 7-8%. ключовим ферментом поліолового шляху є альдозоредуктаза, яка перетворює глюкозу на сорбітол. активація цього ферменту спричиняє порушення внутрішньоклітинного гомеостазу, оскільки кінцеві продукти метаболізму глюкози за сорбітоловим шляхом (сорбітол та фруктоза) не проникають через клітинну мембрану і накопичуються в клітинах, спричиняють набряк, що призводе до загибелі клітини через механізм осмотичного лізису [8, 9]. Отже дослідження впливу генетичних поліморфізмів, які впливають на активність альдозоредуктази (rs759853 і rs9640883 гена aKr1B1) є актуальним та сучасним.Мета роботи: з'ясувати можливу роль поліморфізмів rs759853 і rs9640883 гена aKr1B1 на підставі порівняння розподілу генотипів та алелей в контрольній групі та у хворих з цд2Т та др.Матеріал і методи. дане дослідження проведено на базі кафедри офтальмології львівського національного медичного університету імені данила галицького. контрольну групу склали 98 пацієнтів, які не мали цд2Т та іншої офтальмопатології. першу групу склали 76 пацієнтів із i стадією др (без видимих змін на очному дні). другу групу склали 64 пацієнти зі встановленим діагнозом непроліферативної др (днпр), а третю -64 пацієнти зі встановленою проліферативною др (дпр). Relevance. The problem of systematization and refinement of modern concepts of the pathogenesis of endothelial dysfunction (EDF) and its role in the development of microvascular complications of type 2 diabetes mellitus (T2DM) is relevant due to the lack of pathogenetic treatment nowadays, which would include endothelial dysfunction. Objective: to conduct an analytical review of the results of scientific research on the mechanisms of EDF in T2DM, with the aim of proposing an integrated modern concept of the pathogenesis of EDF. Materials and methods. Review of scientific publications in the international electronic scientific databases of PubMed, Embase and Scopus for keywords for the entire available period (1982-2019). Results. The article provides modern data on the modern concept of the pathogenesis of EDF and its role in the development of microvascular complications in T2DM. The pathogenesis of EDF in type 2 diabetes mellitus is based on the following key mechanisms: impaired synthesis of the endothelial fraction of nitric oxide (NO) due to inhibition of the activity of endothelial NO synthase (eNOS); decreased bioavailability of NO because of oxidative stress; activation of the formation of Endothelin-1 (ET1) and expression of endothelin receptors with a predominance of vasoconstriction; inflammation, which is supported by the synthesis of pro-inflammatory cytokines and causes the expression of inducible NO synthase (iNOS), which stimulates the synthesis of a significant amount of NO, which enters into free radical reactions with the formation of cytotoxic products. Conclusions. The pathogenesis of endothelial dysfunction is impaired nitric oxide synthesis. Endothelial dysfunction, as an integral mechanism, underlies in the core mechanisms the development of vascular complications in type 2 diabetes.
Relevance. In diabetes mellitus (DM), a cascade of pathological reactions unfolds in the vascular endothelium with the formation of endothelial dysfunction (EDF). Its main factors are endothelium hormone – endothelin-1 (ET1), nitric oxide (NO), endothelial NO-synthase (eNOS), and also tumor necrosis factor-α (TNFα). But the unexplored question remains whether EDFs can be used as prognostic markers for the development of diabetic maculopathy. Objective: to find out the prognostic value of EDF factors in the development of diabetic maculopathy (DM) in type 2 diabetes. Material and methods. This study included 152 patients (304 eyes) with type 2 diabetes, which were divided into groups according to the degree of diabetic retinopathy (DR): 1st – initial non-proliferative DR, 2nd – moderate or severe non-proliferative DR and 3rd – proliferative DR. DM was determined by the presence of specific diabetic retinal changes in the macular area according to the International Clinical Gravity Scale of the American Academy of Ophthalmology (2002). The control group consisted of 95 people. The content of ET1, eNOS and TNFα was determined by ELISA; the content of the final NO metabolites (NOx) was determined spectrophotometrically. Statistical data processing was performed using Statistica 10 (StatSoft, Inc., USA). Results. With the progression of DR, there was an increase in the frequency and severity level of DM (p=2.0e-06), which was accompanied by an increase in the blood levels ET1 (3.7-4.7 times), NOx (1.4-1.5 times) and TNFα (4.2-6.5 times), with a progressive decrease in the level of eNOS (P<0.01). Multifactorial logistic regression analysis (GLZ package; StatSoft, Inc., USA) showed that pathogenetic factors for the DM development were multiple increase in blood levels of ET1 and TNFα, which was confirmed by building a logistic regression model with satisfactory results of adequacy (p=0.039). Conclusions. This study confirmed the significant role of EDF in type 2 diabetes and proved the prognostic value of ET1 and TNFα blood levels for the development of DM.
