В обзоре представлены результаты исследований свойств рецептора витамина D и его генетического полиморфизма, который отличается вариабельностью в различных популяциях, а также зависит от возраста и пола. Этот полиморфизм определяет ассоциации различных уровней обеспеченности витамином D с возможностью развития бронхиальной астмы или хронической обструктивной болезни легких и, соответственно, различную эффективность фармакологической коррекции дефицита витамина D. Однако научные данные отличаются противоречивостью, молекулярные механизмы связи витамина D с тонусом бронхов или аллергическими реакциями остаются неясными, что подчеркивает важность исследований для уточнения роли витамина D в воспалительных, иммунных нарушениях при бронхообструктивном синдроме.
Резюме. Актуальність. Коморбідна патологія, притаманна особам літнього та старечого віку, призводить до поліпрагмазії лікарських засобів (ЛЗ). Провідну роль у метаболізмі ЛЗ відіграє система СYР450. Особливе значення має застосування вітаміну D у геріатричних хворих з урахуванням їх вікових особливостей метаболізму. Метою огляду був аналіз міжнародного сучасного інформаційного контенту щодо взаємодії вітаміну D із системою метаболізму ЛЗ. Матеріали та методи. Проведено аналіз сучасних наукових джерел. Результати. На сьогодні описано понад 11 500 білків системи CYP, серед яких у метаболізмі медикаментів беруть участь такі шість: CYP1A2, CYP2C19, CYP2C9, CYP2D6, CYP2E1, CYP3A4, що забезпечують біотрансформацію ЛЗ через окиснення. CYP450-гемопротеїн, що забезпечує зв'язування молекули субстрату з активуванням оксигенів, призводячи до утворення окислення, більш гідрофільного продукту та молекули води. Недостатність гідроксилюючої здатності печінки й нирок може призводити до D-гіповітамінозу в організмі хворих. За метаболізм вітаміну D відповідають CYP11А1, CYP27А1, CYP27В1, CYP24А1. Проведені дослідження свідчать про те, що через ці цитохроми метаболізується ряд інших ЛЗ, які можуть виступати як їх інгібіторами, так і індукторами. Висновки. Система цитохрому P450 впливає на формування метаболітів вітаміну D. З урахуванням фізіологічних шляхів його метаболізму формуються різноспрямовані результати взаємодії.
The review presents information on variants of Vitamin D receptor’s genetic polymorphism, ensuring the direct physiological effects of the Vitamin via stimulation of nuclear cellular mechanisms. The article was aimed at raising awareness of the global scientific advances in the field of Vitamin D receptor’s genetic polymorphism and its association with bronchopulmonary pathology in various regions of the planet. The search of scientific references was carried out in the Scopus, Web of Science, The Cochrane Library, Pubmed, ResearchGate, Russian Science Citation Index (RINC) information databases. The regulatory potential of the Vitamin D active hormonal effects in the bronchopulmonary pathology, especially in bronchial asthma (BA), remains unclear in terms of its pathogenetic links. Individual alleles inherent in the receptor genetics were studied, primarily in children with BA across the world. The results were compared as to levels of Vitamin D supplementation, BA symptoms and course The divergences were found in the four variants of alleles: Fok1, Apal, BsmI, TaqI. Those divergences prevail in the individual ethnic populations, limiting our capacities of drawing unambiguous conclusions, although the relationship between the course of BA and the deficient Vitamin’s status remains predominant. It is necessary to widen the database prospectively, to clarify the genetic variants of all the components involved in the metabolism and the Vitamin’s effects (transporter proteins, cytochrome P450 and vitamin D receptor) while the research geography is also expanding in the world.
