Background. The modern concept of quality of life research creates opportunities for studying various aspects of patients’ lives, assessing the patient’s condition in dynamics and raises new questions about developing further approaches to a comprehensive assessment of the quality of life of patients with various neurological pathologies, in our case, the patients with post-traumatic neuropathy and plexopathies. Materials and methods. Seventy-three men with neuropathies and plexopathies were examined, who were divided into two groups. Group I included 44 patients with post-traumatic neuropathy and plexo-pathy. The second (control) group included 29 patients with compression-ischemic neuropathies and plexopathies without manifestations of chronic neuropathic pain. Patients underwent clinical and neurological examination, electroneuromyography, ultrasound. DN4 and PainDetect questionnaires were used to determine the neuropathic nature of the pain, and a visual analog scale was used to assess pain severity. The quality of life was assessed according to the MOS SF-36 questionnaire. Results. The study showed a significant reduction in the quality of life of patients with post-traumatic neuropathy and plexopathy accompanied by chronic neuropathic pain. The quality of life of patients compared to the control group is significantly lower on the scales of physical functioning, role functioning due to physical condition, the intensity of pain, mental health. In both groups, patients with neuropathies and plexopathies according to the MOS SF-36 questionnaire had reduced quality of life. In group I, chronic neuropathic pain was diagnosed in 32 patients (72.7 %). The indicators of pain corresponded to VAS 7.85 ± 1.52 points, according to the questionnaire DN4 — 7.83 ± ± 1.06, PainDetect Test — 23.20 ± 3.55.
In this review discussion was focused on edema -the common presentation of internal diseases, neurological disorders and surgical problems. Edema is defined as abnormal accumulation of interstitial fluid, which can be either due to excessive capillary filtration or ineffective lymphatic drainage. There are systemic and local causes of edema, which should be revealed promptly, because specific etiology has significant impact on the approach to treatment. Among common causes of edema chronic venous insufficiency, heart failure, nephritic syndrome, effects of drugs, deep venous thrombosis and etc. were analyzed. Some attention was paid to specific conditions like idiopathic edema and lipidema (which is abnormal fat distribution). Diagnostic procedures and treatment options became the features of consideration regarding its effect on the management of patient with peripheral edema. In addition to the descriptive part of the article, the multi-choice questions were proposed for self-assessment.
The following lection is dedicated to one of the most important problems in modern neurology, namely, cell research and myasthenia gravis treatment. Great attention was given to the special characteristics of etiology, pathogenesis, clinical implications, diagnosis by exclusion and contemporary ways of treating myasthenia. Disease classifications, as well as diagnostic criteria of myasthenia, while taking age and gender aspects into account, are given here. New methods to evaluate the dynamics, prognosis of the disease, modern pathogenetic and symptomatic treatment of myasthenia are described.
Regional Clinical special clinic for radiation protection of the population * The aim of the study was to investigate the characteristics of cerebrovascular disorders in the remote period in liquidators of the Chernobyl accident on the basis of a complex examination and dynamic observation of patients from 1996 to 2015. 68 patients were examined -men aged from 50 to 65 years. We studied the clinical and instrumental performance (clinical neurological examination, EEG, REG, Doppler ultrasound of the neck vessels and brain vessels, brain MRI, etc.). Dynamic observation averaged 10-15 years. It is confirmed on the basis of clinical and paraclinical examinations that leading pathology in liquidators is encephalopathy, with a specific set of neurological symptoms, which is characterized by progressive course, the early development of cognitive, psycho-emotional disorders, the development of cortical atrophy. This shows the rapid progression of cerebrovascular disease, the early development of atherosclerosis manifestations of premature aging of the brain at the liquidators of the Chernobyl nuclear power plant.Key Words: cerebrovascular disorders, the liquidators of the Chernobyl accident, the clinical and neurophysiological examination, premature aging of the brain.Стаття надійшла до редакції 03.01.2017 р.
Myofascial pain syndrome in railway workersAbstract. Effect of proprioceptive neuromuscular facilitation and injection of anesthetic (0.5% novocaine or 2% lidocaine solutions) into the trigger points in combination with standard therapy was studied in 64 railway workers suffering from myofascial pain syndrome manifested by pain, muscle tension, decrease of amplitude of muscle movements, active trigger points in affected muscles, sensory and autonomic disorders in the area of referred pain. Ap-plication of this combination was accompanied by fast decrease of pain syndrome, increase in the amplitude of muscle movements and disappearance of active trigger point in them, normalized indicators of electromyography.
