Цель работы — оценка эффективности симуляционных технологий в процессе формирования и поддержания у клинических ординаторов и аспирантов практических навыков сердечно-легочной реанимации и компетенций оказания неотложной помощи. Был проведен проспективный анализ обучения и усвоения теоретических знаний и практических навыков по проведению сердечно-легочной реанимации клиническими ординаторами и аспирантами. Установлено, что симуляционные формы обучения с использованием современных манекенов способствуют быстрому, эффективному и безоп асному формированию у слушателей курса как технических, так и нетехнических навыков оказания неотложной помощи.
Однією з найбільш актуальних галузей застосу-вання стимуляційного навчання є невідкладна до-помога та реанімація, оскільки підготовка лікарів із питань реанімації та інтенсивної терапії відрізня-ється складною організаційно-деонтологічною спе-цифікою.Аналіз ускладнень у медицині продемонстру-вав, що причиною більшості нещасних випадків (70-80 %) є прояв людського фактора або сукуп-ності так званих нетехнічних навичок [11]. Згідно з даними літератури, одним із найбільш ефективних методів зниження частоти ускладнень та смертності є симуляція або клінічне моделювання, що дозволяє не тільки успішно використовувати набуті на трена-жерах навички роботи в клінічній практиці, але й найкраще відпрацювати командну взаємодію (не-технічні навички) [8, 12].Симуляція у медичній освіті -це сучасна техно-логія навчання та оцінки практичних навичок, умінь і знань, заснована на реалістичному моделюванні, імітації клінічної ситуації чи окремо взятої фізіо-логічної системи, для чого можуть використовува-тися біологічні, механічні, електронні та віртуальні (комп'ютерні) моделі [2]. На сьогодні не викликає сумнівів необхідність впровадження цього методу навчання в процес обов'язкової післядипломної підготовки лікарів усіх спеціальностей, які надають невідкладну медичну допомогу. Так, у введеному в 2010 році в рекомендації Американської Асоціації серця (American Heart Association -AHA) з серце-во-легеневої реанімації (СЛР) та невідкладної до-помоги при серцево-судинних захворюваннях роз-ділі «Навчання, впровадження і робота в команді» [10] уперше розглядаються питання, що присвячені оптимальним методам навчання навичок реаніма-ції, впровадженню алгоритму заходів щодо запо-бігання смерті та передових методів, пов'язаних з роботою в команді. Зазначено, що реалістичні манекени можуть бути корисними для інтеграції знань, вмінь та навичок під час навчання інтенсив-ній терапії [10]. Fos immunoreactivity in the intermediolateralWe investigated neuronal activation of the rat intermediolateral (IML) nucleus of the thoracolumbar spinal cord, initiated by Achilles tendon vibration, after intramuscular (m. triceps surae) administration of 7-nitroindazole (7-NI) or L-arginine (LA). The spindle afferent response to vibratory stimuli induced a distinct bilateral increase in the activation of c-Fos immunoreactivity in the spinal neurons in three groups of rats (tendon-vibrated, tendon-vibrated + 7-NI and tendon-vibrated + LA). The T5/T13 segments in tendon-vibrated +7-NI animals showed the highest increase of Fos-immunoreactive neurons. This increase was two times higher than that in tendon only-vibrated control rats and tendon-vibrated + LA animals. The highest mean number of labelled neurons were observed in the IML nucleus and in layers 4 and 7 of the T5-L3 segments in tendon-vibrated and tendon-vibrated + 7-NI animals, and in the IML nucleus and layer 4 in tendon-vibrated + LA rats. The highest mean number of activated neurons was found ipsilaterally in the IML nucleus of the T5/T13 segment. These results indicate that decreased nitric oxide release after injection of 7-NI was accompanied by a potentiation of the early c-fos gene expression induced by muscle proprioceptive activity within the thoracolumbar region of the rat spinal cord. Thus, enhanced c-Fos immunoreactivity in the IML nucleus indicated that the sympathetic nervous system can exert a direct influence on the muscle spindles.
