Simvastatin treatment is associated with moderate clinical improvement in patients with active RA. Immunological changes produced by simvastatin in peripheral blood are complex and may reflect both its anti- and pro-inflammatory properties.
We studied safety and clinical efficacy of transplantation of autologous bone marrow cell in complex therapy of 158 patients with chronic hepatitis and cirrhosis of the liver. The efficiency of cell therapy was assessed in 12 months after single injection of the cells. The positive response (alleviation of liver cirrhosis or stabilization of the pathological process) was observed in 70% cases. The efficacy of therapy correlated with the severity and etiology of the disease and was maximum in patients with Child-Pugh class A (in 82.5% cases) and class B liver cirrhosis (in 79% cases); in patients with class C liver cirrhosis, the positive response was achieved in 42.5% cases. In 39 patients, ultrasonic examination performed in 3 years after transplantation revealed no focal lesions or ectopic ossification foci.
Recurrent herpes simplex labialis caused predominantly with herpes simplexvirus 1(HSV-1) is a major problem, for which various treatments have minimal impact. Given the important role of the immune system in controlling virus infection, an activation of virus-specific immune responses, in particular,using dendritic cell (DCs) vaccines, seems to be a promising approach for the treatment of patients with frequent recurrences of herpes labialis. The current paper presents the results of a pilot study of the safety and efficacy of DC vaccines in 14 patients with recurrent HSV-1 infections. DCs were generated in presence of GM-CSF and IFN-alpha and were loaded with HSV-1 recombinant viral glycoprotein D (HSV1gD). DCs cells were injected subcutaneously as 2 courses of vaccination during 9 months. Immunotherapy with DCs did not induce any serious side effects and resulted in more than 2-fold reduction in the recurrence rate and significant enhancement of the inter-recurrent time during the 9 months of treatment and subsequent 6-month follow-up period. An obvious clinical improvement was accompanied with an induction of an antigen-specific response to HCV1gD and a normalization of reduced mitogenic responsiveness of mononuclear cells. According to long-term survey data (on average 48 months after the beginning of therapy), 87% of respondents reported the decreased incidence of recurrent infection. At this time, most patients (85.7%) responded to HCV1gD stimulation. The data obtained suggests that dendritic cell vaccines may be a promising new approach for the treatment of recurrent labial herpes.

Статины используются для снижения уровня холестерина и профилактики сердечно-сосудистых заболеваний. В последние годы были открыты иммуномодулирующие и противовоспалительные свойства статинов, обосновывающие возможность их применения при аутоиммунных заболеваниях. Нами изучалась эффективность и безопасность симвастатина в дозе 40 мг/сут. при активном ревматоидном артрите (РА) на фоне приема стабильных доз болезнь-модифицирующих препаратов. После 12 недель терапии симвастатином было зарегистрировано статистически значимое снижение индекса активности болезни на 0,89 баллов, улучшение качества жизни и уменьшение уровня боли. Для подтверждения эффективности и безопасности симвастатина при активном РА требуется проведение крупномасштабных контролируемых рандомизированных испытаний.

Хроническое рецидивирующее течение инфекции, обусловленной вирусами просто-го герпеса (ВПГ) 1 и 2 типа, представляет серьезную медико-социальную проблему. Учитывая важ-ную роль иммунной системы в контроле за вирусом, перспективы лечения рецидивирующих форм ВПГ-инфекции связывают с активацией антигенспецифического иммунного ответа, в частности с использованием вакцин на основе дендритных клеток (ДК). В настоящей работе представлены ре-зультаты открытого пилотного исследования эффективности и безопасности дендритноклеточных вакцин у пациентов с рецидивирующей ВПГ-инфекцией. В исследование были включены 29 человек, включая 14 пациентов с лабиальным и 15 с генитальным герпесом. ДК генерировали в присутствии GM-CSF и IFNα, нагружали рекомбинантными вирусными белками (HSV1gD или HSV-2gD) и про-водили 2 курса вакцинаций в течение 9 мес. Иммунотерапия ДК не вызывала серьезных нежелатель-ных эффектов и сопровождалась более чем двукратным снижением частоты рецидивов и увеличе-нием длительности ремиссии на фоне 9 мес. лечения. В период последующего 6-месячного периода наблюдалось дальнейшее снижение частоты рецидивов и трехкратное возрастание длительности ре-миссии. Клинический эффект на фоне лечения и в период 6 мес. наблюдения отмечался как в группе с лабиальным, так и генитальным герпесом, регистрировался у большинства пациентов, ассоции-ровался с индукцией антигенспецифического пролиферативного ответа и нормализацией исходно сниженной митогенной реактивности мононуклеарных клеток в ответ на стимуляцию КонА. Анализ отдаленных результатов лечения на основе анкетирования пациентов с давностью наблюдения более 24 мес. показал, что положительный эффект иммунотерапии в виде снижения частоты рецидивов че-рез 48 мес. (медиана наблюдения) сохранялся у 77,8% опрошенных. Антигенспецифический проли-феративный ответ в эти сроки сохранялся у 66,7%. Полученные результаты позволяют рассматривать ДК-вакцины в качестве нового перспективного подхода к лечению рецидивирующих форм лабиаль-ного и генитального герпеса. Опыт при-менения симвастатина при ревматоидном артрите (РА) ограничен единичными работами. Целью настоящего исследования являлось изучение эффективности и безопасности симвастатина у больных ак-тивным РА, получавших болезнь-модифицирующие препараты. В открытом сравнительном исследовании 33 пациента с активным РА принимали симвастатин в дозе 40 мг в сутки в течение 12 недель. Группа исто-рического контроля была сформирована из 9 пациентов, получавших плацебо в сочетании с болезнь-модифицирующей терапией. Группы сравнения не отличались по демографическим показателям и ак-тивности болезни. У больных, принимавших симвастатин, в конце лечения выявлялось достоверно более выраженное, по сравнению с контролем, снижение индекса активности болезни с 28-суставным счетом (DAS28) и оценки активности болезни врачом. Размер эффекта по DAS28 составил 0,76 (95% доверитель-ный интервал 0,01-1,5), что можно расценивать как умеренное снижение активности заболевания. Таким образом, применение симвастатина в сочетании с традиционными болезнь-модифицирующими препа-ратами приводит к уменьшению активности РА. Recently discovered immunomodulating and anti-inflammatory properties of statins have resulted in application of these drugs for treatment of autoimmune disorders. There are few studies investigating therapeutic potential of simvastatin in rheumatoid arthritis (RA). In present study, we investigated efficacy and safety of simvastatin in active RA patients treated with conventional disease-modifying antirheumatic drugs (DMARDs). Thirty-three patients were enrolled into an open-label, controlled study. The patients received treatment with 40 mg of simvastatin daily for 12 weeks. A group of historical controls consisted of nine patients taking placebo combined with disease-modifying therapy. No differences in demographic characteristics and disease activity were observed between the two groups. By the end of therapy (12 weeks), simvastatin-treated patients exhibited a significant reduction in disease activity scores with 28-joint counts (DAS28), and according to physician's assessment of disease, as compared with control group. In the present study, we used multicolor flow cytometry assay to measure the numbers of various myeloid suppressor cell subpopulations (Lin-HLA-DR-CD33+, Lin-HLA-DR-CD33+CD66b+, and CD14+HLA-DRlow/-) in peripheral blood of 51 participants, including 33 patients with viral- and 1 8 patients with «nonviral» (alcoholic or biliary/autoimmune) liver cirrhosis. Patients in both groups had increased proportions of Lin-HLA-DR-CD33+, Lin-HLA-DR-CD33+CD66b+, and CD 14+HLA-DRlow/-cells which levels did not depend on the type and replication of the virus in viral liver cirrhosis. In viral liver cirrhosis, the relative numbers of Lin-HLA-DR-CD33+cells directly correlated with the albumin levels (Rs=0.45; p=0.029). In «nonviral» group an inverse relationship was found between these indicators (Rs = -0.56; p=0.02), in addition the proportion of Lin-HLA-DR-CD33+and CD14+HLA-DRlow/-cells directly correlated with disease severity scores (Child-Pugh and MELD) direct correlation of the proportion of Lin-HLA-DR-CD33+and CD14+HLA-DRlow/-cells with the disease severity scores (Child-Pugh and MELD). Liver cirrhosis (LC) is a serious problem with respect to drug therapy. Certain promises are associated with development of stem cell-based technologies, in particular, mesenchymal stromal cells (MSCs). Over recent years, a significant interest was drawn to the usage of MSCs isolated from adipose tissue, which have a number of advantages over bone marrow MSCs. The aim of this study was to evaluate safety and tolerability of transplantation of autologous MSCs derived from adipose tissue (lipoaspirate) when treating the patients with LC of different origin. The study group consisted of 12 patients (4 men and 8 women), aged 33 to 67 years. In six patients, development of LC was due to viral hepatitis; in other subjects it was caused by non-viral factors, including toxic effects (n = 3), primary biliary cirrhosis (n = 2) and cryptogenic cirrhosis (n = 1). The LC severity assessed by the Child-Pugh scale corresponded to class A (6 cases) or class B (6 patients). The MELD scores varied from 8 to 11. All patients were injected once with autologous MSCs (a mean of 12.94 106, 11.3 to 16.0 106) obtained by culturing the lipoaspirate cells for 14 days. Lipoaspiration and injection of MSCs were not accompanied by development of side effects, or serious adverse events. Analysis of clinical efficacy, carried out 6-12 months after therapy, made it possible to distinguish two subgroups: responders to the therapy (subgroup 1, n = 6) and non-responders (subgroup 2, n = 6). In subgroup 1, the MELD score was found to be decreased in 4 cases, and did not change in 2 patients. In contrast, the MELD score increased in the patients from subgroup 2. Both subgroups did not differ in etiology and severity of LC, or concomitant pathology (COVID-19). Positive dynamics in subgroup 1 was revealed for the signs of hepatocellular insufficiency (n = 1), cytolysis (n = 2) and cholestasis (n = 1). In two patients with MELD score stabilization, the scores reached the reference values. After MSC therapy, the majority of patients (8/12) underwent a new coronavirus infection, and the COVID incidence rate was similar in groups with or without response to MSC therapy (4 patients in each group). The results of the study suggest the safety of using MSCs isolated from the adipose tissue of patients with liver cirrhosis, and the perspectives of such approach in order to stabilize or improve liver function.
