A combination of explicit solvent molecular dynamics simulation (30 simulations reaching 4 µs in total), hybrid quantum mechanics/molecular mechanics approach and isothermal titration calorimetry was used to investigate the atomistic picture of ion binding to 15-mer thrombin-binding quadruplex DNA (G-DNA) aptamer. Binding of ions to G-DNA is complex multiple pathway process, which is strongly affected by the type of the cation. The individual ion-binding events are substantially modulated by the connecting loops of the aptamer, which play several roles. They stabilize the molecule during time periods when the bound ions are not present, they modulate the route of the ion into the stem and they also stabilize the internal ions by closing the gates through which the ions enter the quadruplex. Using our extensive simulations, we for the first time observed full spontaneous exchange of internal cation between quadruplex molecule and bulk solvent at atomistic resolution. The simulation suggests that expulsion of the internally bound ion is correlated with initial binding of the incoming ion. The incoming ion then readily replaces the bound ion while minimizing any destabilization of the solute molecule during the exchange.
This publication focuses on skin toxicity and skin manifestations of a chronic graft versus host disease (chGVHD). The article provides analysis of contemporary literature on these pathologies, presents new approach to preventing its development and also introduces pathogenetically determined patterns of non-drug correction of skin manifestations.
Хроническая реакция трансплантат против хо-зяина (РТПХ) представляет собой мультисистемное алло/аутоиммунное заболевание, характеризующе-еся иммунной дисрегуляцией, иммунодефицитом и поражением внутренних органов.Актуальность темы обусловлена тем, что РТПХ -частое осложнение трансплантации гемопоэтиче-ских стволовых клеток (ТГСК). По данным зару-бежных коллег [1], РТПХ возникает у половины больных после аллогенной трансплантации крове-творных стволовых клеток и является одной из ос-новных причин смертности этих пациентов.Хроническая РТПХ развивается спустя 100 дней после аллогенной ТГСК. Через 2 года после ТГСК РТПХ регистрируется у 25-80% пациентов. Лими-тированная кожная форма составляет 30-50%, по данным разных авторов, а 5-летняя общая выживае-мость при экстенсивных формах данного осложне-ния не превышает 40%.Биологические механизмы формирования хро-нической РТПХ не столь очевидны по сравнению с острой РТПХ. К главным органам-мишеням РТПХ относят ко-жу, кишечник и печень. Хроническая РТПХ в коже представлена мононуклеарным воспалительным инфильтратом, лимфоцитарной деструкцией саль-ных и потовых желез, неравномерным акантозом, гиперкератозом или атрофией эпидермиса, фибро-зом и склерозом дермы. Помимо этого, происходит деструкция слезных и слюнных желез, эпителия ре-спираторного тракта, кишечника, желчных прото-ков. Повреждение эндотелия сосудов в период кон-диционирования, т.е. подготовке к ТГСК с целью создания иммуносупрессивного состояния, приво-дит к стимуляции синтеза клеток-предшественни-ков эндотелиоцитов и васкуляризации. Васкуляри-зация в свою очередь облегчает распространение иммунных клеток к органам-мишеням. Затем обра-зовавшиеся сосуды сами становятся мишенью для аллореактивных донорских T-клеток, что приводит к фиброзу кровеносных сосудов. Алло-аутоиммун-ный характер процесса, нарушение как гуморально-го, так и клеточного звеньев иммунитета объясняют полиморфизм клинической картины [2].

Благодаря современным протоколам лечения выживаемость больных в ранние сроки после ТГСК увеличилась. Вместе с тем возросло количество пациентов с позд-ними осложнениями ТГСК. Одно из таких осложнений-хроническая реакция «трансплантат против хозяина» (хрРТПХ)-алло-и аутоиммунный процесс, главной мишенью которого является соединительная ткань. По данным разных авторов, кожа вовлекается в патологический процесс у 70-80% пациентов; у 50-70% из них заболевание протекает в экстенсивной форме [1-4].
