Despite the fact that HPV infection is widespread among the population around the world, more than 150 strains of HPV viruses are known. At the present stage it is still unclear, why HPV-associated oral papillomas in dental patients either spontaneously undergo involution or become malignant. We investigated the quantitative dynamics of Langerhans cells depending on localization in the structure of such papillomas: in the peripapillary space, in the tissue of the oral mucosa in the presence of HPV infection, without a clinical picture of papillomas and in healthy tissue. It has been established that with long-term existing papillomas, antigen presentation is disturbed, and Langerhans cells can be an indicator of papilloma malignancy.
В стоматологической практике проблемы дентальной имплантации у пациентов не только с сахарным диабетом 2 типа, но и со сниженной толерантностью к глюкозе остаются предметом острых дискуссий, так как остеоинтеграция дентальных имплантатов у данной категории больных на современном этапе остаётся малоизученным вопросом. С помощью иммуногистохимических методов изучены особенности распределения эффекторных иммуноцитов в составе дифферонов эпителиальной пластинки слизистой оболочки рта в динамике дентальной имплантации и их роль в прогнозировании исхода дентальной имплантации у пациентов со скрытой формой сахарного диабета 2 типа. Было установлено, что экспрессия маркеров CD68 и CD163 достоверно коррелирует с уровнем интеграции и отторжением имплантатов. Анализ полученных результатов показал, что у пациентов с высокой экспрессией CD163 значительно хуже показатели интеграции имплантатов (р < 0,01), что косвенно свидетельствует об их потенциальном прогностическом значении в прогнозировании исходов и эффективности применения дентальной имплантации при различных состояниях локального иммунного гомеостаза слизистой оболочки рта у пациентов со скрытой формой СД2 и сниженной толерантностью к глюкозе. Research and Education Center, Niigata, In dental practice, the problems of dental implantation in patients with type 2 diabetes remain a topic of acute debate, since the osseointegration of dental implants in this category of patients remains a poorly understood issue at the present stage. The purpose of this study was to study the dynamics of dental implantation and clinical implantation results in patients with type 2 diabetes mellitus. It was studied with the help of immunohistochemical analysis, the features of the distribution of effector immunocytes in the composition of the epithelial membranes of the epithelial plate of the oral mucosa in the dynamics of dental implantation and their role in predicting the outcome of dental implantation in patients with a latent form of type 2 diabetes mellitus are studied.. It was found that the expression of markers CD68 and CD163 significantly correlates with the level of integration and rejection of implants. The analysis of the obtained results showed that in patients with high CD163 expression, the integration of implants is significantly worse (p <0,05), which indirectly indicates their potential prognostic value in predicting dental implant outcomes. MORPHOLOGY OF THE ORAL MUCOSA IN THE DYNAMICS OF DENTAL IMPLANTATION IN PATIENTS WITH TYPE 2 DIABETES MELLITUS
Analysis of the HPV positive papilloma structures at different levels of pathological process development was carried out. Classical staining of preparations with hematoxylin and eosin was used for immunohistochemical determination of Ki67-positive cells and phenotyping of CD positive cells. We found that the process of papilloma formation begins with a local increase in the proliferative activity of keratinocytes which contributes to the formation of a local epithelial convex above the surface of the oropharyngeal mucosa (OPM) in the form of papilloma. The connective tissue of the OPM adjacent to the epithelium grows at the second stage. At the same time here is a decrease in immunocytes/macrophages number of the oral epithelium not only in the growth zone of the papilloma but in the adjacent neoplasm tissue. The third stage is characterized by the destruction of the basal membrane of the OPM. Apoptotic cells in the cambial layer and forming leukocytes infiltrate the OPM lamina propria. Virus-infected keratinocytes are phagocytized by macrophages or exfoliated from the surface of the epithelial layer. The emerging in the middle layers defect of tissue of the epithelial layer and the absence of Langerhans cells indicate a relationship between the migration of antigen-presenting cells expressing CD68 with impaired differentiation and specialization of keratinocytes. The conclusion is based on the analogy of "leukemic failure" in leukemia and on the absence of differentiating epithelial cells between the cambium and the specialized surface layer in the long-existing papilloma. The disappearance from the epithelium of CD68-positive cells specific to the epithelial layer is a prognostic sign of malignization in the mucous membrane of the oropharynx.
На современном этапе имеются только единичные исследования, посвященные местным факторам иммунной защиты в динамике дентальной имплантации. Нами были изучены срезы биоптатов СОР пациентов в динамике дентальной имплантации с помощью классических и иммуногистохимических методов исследования с последующим анализом на микроскопе Olympus Bx52. В результате морфологического мониторинга структур слизистой оболочки рта в контрольной группе пациентов была представлена модель, отражающая изменения локального иммунного гомеостаза у людей без сопутствующей патологии, отмечено, что с возрастом отмечается снижение регенераторного потенциала кератиноцитов и пластичности эпителия СОР, уменьшение количества эффекторных иммуноцитов, количества функционирующих капилляров и снижение плотности МЦР в собственной пластинке СОР. Предложены дополнения к стандартному алгоритму обследования пациентов в клинике челюстно-лицевой хирургии, с морфологической картиной иммунного гомеостаза СОР с помощью фазово-контрастных микроскопических исследований. При выявлении отклонений цитологических показателей иммунного гомеостаза в сравнении с предложенной моделью, отражающей среднестатистические нормальные возрастные показатели пролиферативной активности кератиноцитов и соотношение эффекторных иммуноцитов, а также плотность МЦР в собственной пластинке слизистой рта, следует планировать тактику дальнейших мероприятий с предварительной иммуномодулирующей корректировкой метода лечения. At the present stage, there are only single studies devoted to local factors of immune defense in the dynamics of dental implantation. We have studied biopsies sections of the oral mucosa (OM) on the background dental implantation, with using the classical and immunohistochemical methods of investigation with the further held analysis on the Olympus Bx52 microscope. As a result of morphological monitoring of the structures of the oral mucosa, a model reflecting changes in local immune homeostasis in people without concomitant pathology was presented in the control group of patients, it was noted that with age, the regenerative potential of keratinocytes and ductility of the COP epithelium decreased, the number of effector immunocytes, the number of functioning capillaries and a decrease in the density of the vascular bed in in its own plate from the lining of the mouth. Offered apps for the standard analysis of patients in the clinic of maxillofacial surgery, and diagnostics of the OM local immunity homeostasis state. If cytological signs of deviation in the indices of the homeostasis are found in comparison with the proposed model, reflecting the average statistic indices, as well as the density of the MCR in its own sheet of oral mucosa, further actions should be planned with a prior immunomodulatory correction of the method of treatment.
