Проблема острого повреждения почки (ОПП) после кардиохирургических вмешательств по-прежнему остается не решенной, не смотря на высокий уровень развития хирургической техники, защиты органов и тканей в условиях искусственного кровообращения. Частого развития осложнения по данным разных авторов достигает до 42%, а необходимость проведения методов заместительной почечной терапии до 2-8% Цель: оценить диагностическое и прогностическое значение определения уровня матриксной металлопротеиназы 8 и ее ингибитора 1 типа (ММР-8, TIMP 1) у пациентов с острым повреждением почек до и после АКШ. Материалы и методы: сыворотка крови пациентов с ишемической болезнью сердца (ИБС) (n = 120), планируемых на операцию АКШ, которых ретроспективно разделили на 3 группы: с ренальными осложнениями (I гр.), с ОПП и сахарным диабетом 2 типа (II гр.), без осложнений в послеоперационном периоде (III гр.). Результаты: зарегистрировано увеличение концентрации ММР-8 в сыворотке крови при развитии ОПП в I гр. (p<0,05), выявлен низкий уровень ММР-8 до операции во II группе с последующим его нарастанием в 2 раза (p<0,05), увеличение уровня TIMP 1 после оперативного вмешательства зафиксировано во всех группах на 7е сутки (p<0,05). Ключевые слова Матриксная металлопротеиназа 8 типа, тканевой ингибитор 1 типа острое повреждение почек, аортокоронарное шунтирование Summary The problem of early postoperative complications after coronary artery bypass surgery remains unresolved, one of these complications is acute kidney injury (AKI), the prevalence of which according to different authors reaches 30%. Aim: to evaluate the diagnostic and prognostic value in determining the level of matrix metalloproteinase 8 and their type 1 inhibitor (MMP-8, TIMP 1) in patients with acute kidney injury before and after cardiac surgery. Materials and methods: serum of 120 patients with coronary heart disease before and after cardiac surgery, divided into 3 groups: with AKI (I gr.), with AKI and diabetes mellitus (II gr.), without renal complications (III gr.). Results: an increase in serum MMP-8 concentration was registered during the development of AKI in I gr. (p<0,05), low MMP-8 level was detected before the operation in group II (p<0,05), an increase in the level of TIMP 1 after surgery was recorded in all groups at 7 day (p<0,05).
