Background and objectives. Stroke-induced mortality is the third most common cause of death in developed countries. Intense interest has focused on the recurrent ischemic stroke, which rate makes up 30% during first 5 years after first-ever stroke. This work aims to develop criteria for the prediction of acute recurrent cerebral ischemic hemispheric stroke (RCIHS) outcome on the basis of comprehensive baseline clinical, laboratory, and neuroimaging examinations. Materials and Methods. One hundred thirty-six patients (71 males and 65 females, median age 74 (65; 78)) with acute RCIHS were enrolled in the study. All patients underwent a detailed clinical and neurological examination using National Institutes of Health Stroke Scale (NIHSS), computed tomography of the brain, hematological, and biochemical investigations. In order to detect the dependent and independent risk factors of the lethal outcome of the acute period of RCIHS, univariable and multivariable regression analysis were conducted. A receiver operating characteristic (ROC) analysis with the calculation of sensitivity and specificity was performed to determine the prediction variables. Results. Twenty-five patients died. The independent predictors of the lethal outcome of acute RCIHS were: Baseline NIHSS score (OR 95% CІ 1.33 (1.08–1.64), p = 0.0003), septum pellucidum displacement (OR 95% CI 1.53 (1.17–2.00), p = 0.0021), glucose serum level (OR 95% CI 1.28 (1.09–1.50), p = 0.0022), neutrophil-to-lymphocyte ratio (OR 95% CI 1.11 (1.00–1.21), p = 0.0303). The mathematical model, which included these variables was developed and it could determine the prognosis of lethal outcome of the acute RCIHS with an accuracy of 86.8% (AUC = 0.88 ± 0.04 (0.88–0.93), p < 0.0001).

The aim of the work – to improve the professional training of students by forming the skills of independent educational and research work. The main body. Nowadays, study work of students takes an important place in the educational process. A lot of difficulties among the students appear during the process of enquiring the skills of independent work; in many cases they are connected with the issues of methodological nature. So, the teacher’s task is to help students in choosing the topic, setting the goal and tasks of the study, and also to determine the scale, materials and methods of the research. Doing some kind of educational and research work under the guidance of the teacher expands students’ scientific outlook and enables them to master the essential scientific concepts and understandings. In the process of writing research work students develop their skills to analyze, systematize, compare, generalize and classify scientific literature data and also obtained results of the conducted research.

Мета – розробити інтегральні критерії прогнозування летального виходу гострого періоду спонтанного супратенторіального внутрішньомозкового крововиливу на тлі консервативної терапії, які ґрунтуються на результатах комплексного клініко-нейровізуалізаційно-електроенцефалографічного дослідження. Матеріал і методи. Проведено проспективне когортне порівняльне дослідження 156 пацієнтів у гострому періоді спонтанного супратенторіального внутрішньомозкового крововиливу на тлі консервативної терапії, які були госпіталізовані протягом 24 годин від дебюту захворювання. Клініко-неврологічне дослідження включало оцінку за шкалою коми Full Outline of UnResponsiveness та National Institute of Health Stroke Scale. Візуалізацію церебральних структур здійснювали методом комп’ютерної томографії. Електроенцефалографічне дослідження проводили в перші 48 годин, при цьому окремо для ураженої гемісфери та інтактної гемісфери визначали рівні абсолютної спектральної потужності ритмів дельта-, тета-, альфа-, бета-діапазонів із наступним розрахунком інтегральних коефіцієнтів, які відображають різні співвідношення зазначених вище ритмів. Для розробки критеріїв прогнозування застосовували логістичний регресійний аналіз та ROC-аналіз. Результати. Летальний вихід протягом гострого періоду захворювання був зафіксований в 42 (26,9 %) випадках. На підставі мультиваріантного логістичного регресійного аналізу було встановлено, що з ризиком летального виходу гострого періоду спонтанного супратенторіального внутрішньомозкового крововиливу на тлі консервативної терапії незалежно асоційовані такі клініко-нейровізуалізаційно-електроенцефалографічні критерії оцінки тяжкості ураження церебральних структур у дебюті захворювання: сумарний бал за шкалою коми Full Outline of UnResponsiveness (ВШ (95 % ДІ) = 0,29 (0,13–0,69), р=0,0050), обсяг осередку ураження (ВШ (95 % ДІ) = 1,05 (1,01–1,10), р=0,0270), значення коефіцієнта DTABR в ураженій гемісфері (ВШ (95 % ДІ) = 1,70 (1,14–1,53), р=0,0092) та коефіцієнта DTR в інтактній півкулі (ВШ (95 % ДІ) = 1,60 (1,02–2,53), р=0,0412). Висновки. Розроблена математична модель для прогнозування летального виходу гострого періоду спонтанного супратенторіального внутрішньомозкового крововиливу на тлі консервативної терапії, яка ґрунтується на результатах комплексного клініко-нейровізуалізаційно-електроенцефалографічного дослідження в дебюті захворювання (AUC=0,99, p<0,0001; точність прогнозування 97,4 %).

The aim of this study is to improve the diagnostic approaches in acute period of spontaneous supratentorial intracerebral hemorrhage by detecting the most informative parameters of spectral analysis of the electroencephalographic pattern in the evaluation of cerebral bioelectrical activity changes caused by the lateral shifts of brain midline structures. Materials and methods. We carried out clinical and paraclinical examination of 156 patients (90 men and 66 women, mean age 66,7±0,8 years) with firstly appeared hypertensive spontaneous supratentorial intracerebral hemorrhage. The diagnosis was based on the findings of computed tomography. Lateral shift was evaluated as an average from septum pellucidum and pineal gland displacements. Clinical neurological investigation included the evaluation according to the National Institute of Health Stroke Scale. Computed electroencephalography was carried out within the first 48 hours from the disease onset. Spectral rhythm power, fronto-occipital gradients, interhemispheric rhythm asymmetry parameters were analyzed. Statistical processing included evaluating differences between the groups studied, logistic regression analysis, ROC-analysis. Results. No lateral shift of brain midline structure was found in 57 (36.5%) patients; lateral shift ranged from 1-5mm was diagnosed in 72 (46.2%) patients; the lateral shift over 5 mm was found in 27 (17.3%) patients. We have found out the following values can be used as electroencephalographic criteria of lateral shift of brain midline structures: values of relative spectral power rhythm of alpha band ≤20,5% in affected hemisphere (Se=79,8%, Sp=77,2%) and ≤17,7% in intact hemisphere (Se=71,7%, 75,4%), fronto-occipital rhythm gradients of alpha2 band >–0,085 in affected hemisphere (Se=71,7%, Sp=63,2%) and >–0,266 in intact hemisphere (Se=80,8%, Sp=54,4%), while relative spectral rhythm power of delta band >48,4% in affected hemisphere (Se=88,9%, Sp=74,2%) and >46,8% in intact hemisphere (Se=92,6%, Sp=72,4%), fronto-occipital rhythm gradients of alpha band >–0,001 in affected hemisphere (Se=81,5%, Sp=65,1%). These values demonstrate disturbances in cerebral bioelectrical activity caused by lateral shift over 5 mm. Conclusions. The values of relative spectral rhythm power and fronto-occipital rhythm gradients of alpha band in affected and intact hemispheres are the most informative parameters of spectral electroencephalographic pattern analysis for detection of bioelectrical brain activity deteriorations caused by the lateral shift of brain midline structure due to acute spontaneous supratentorial intracerebral haemorrhage.
