Zusammenfassung.Hintergrund: Bei schweren diabetischen Veranderungen kann eine kombinierte Verpflanzung von Bauchspeicheldruse and Pankreas vorgenommen werden.Methoden: In einer retrospektiven Falldemonstration prasentieren wir ein Zwillingspaar, bei dem im Alter von 25 Jahren eine Pankreastransplantation vorgenommen worden war. Bei beiden Patienten lag eine durchschnittliche Erkrankungsdauer der Retinopathie von 5 Jahren vor der Transplantation vor.Ergebnis: Nach erfolgreicher Pankreastransplantation lag bei beiden Zwillingen an je einem Auge eine proliferative diabetische Vitreoretinopathiee (PDVR) Stadium B and am Partnerauge ein Stadium C vor, welche multiple Pars-plana-Vitrektomien erforderten. Beim ersten Zwilling betrug der preoperative Visus 1/50 rechts and 0,05 links welcher postoperativ auf 1/35 and 0,2 verbessert werden konnte. Gleichzeitig wurde die retinale Situation langfristig stabilisiert. Beim zweiten Zwilling war der bestkorrigierte Visus 1/35 rechts and 0,2 links. Nach mehreren Operationen wurde eine stabile Fundussituation erzielt and eine Visusstabilisierung auf 1/35 rechts and 0,2 links erreicht. Zu einer AbstoBungsreaktion des transplantierten Pankreas war es innerhalb der ersten vier postoperativen Jahre nicht gekommen.Schlussfolgerungen: Durch eine verbesserte Operationstechnik bei der Pankreas-Duodenal-Transplantation konnte die Rate an AbstoBungsreaktionen and Transplantatversagen deutlich gesenkt werden. Bei unseren Patienten konnte auch langfristig eine suffiziente Stoffwechsellage and Normalisierung des Glukosestoffwechsels erreicht werden and die Lebensqualitat nachhaltig verbessert werden. Fur eine erfolgreiche Behandlung einer diabetischen Vitreoretinopathie musste die Pankreastransplantation vor dem Auftreten von irreversiblen Veranderungen durchgefuhrt werden. Sollte es gelingen, die Pankreastransplantation zu einer Routineoperation werden zu lassen, sollte der Zeitpunkt fruher gesetzt werden, so dass fiir den juvenilen Diabetiker ein brauchbarer Visus erhalten werden kann. Zusammenfassung. Hintergrund: Die bei diabetischer Glas-k ö r p e r b l u t u n g durchgeführte, zirkuläre periphere Netzhaut -k r y o k o a g u l a t i o n vermag zur Resorption der Blutung beizutragen. Es soll untersucht werden, inwiefern diese auch die Vitrektomienotwendigkeit senken kann.Material und Methode: Wir werteten retrospektiv die Akten aller in den Jahren 1997-2000 operierten Patienten aus, die eine zirkuläre, periphere Netzhautkryokoagulation bei Glaskörperblutung im Rahmen einer proliferativen dia -b e t i s c h e n V i t r e o r e t i n o p a t h i e erhielten. Erfasst wurden neben Alter und Geschlecht des Patienten die resorptive Kraft der Kryokoagulation, ob weiterhin Vitrektomiepflichtigkeit bestand und der Zeitpunkt einer möglicherweise nachfolgenden ppV Der Nachuntersuchungszeitraum wurde mit mindestens 2 Monaten festgelegt.Resultate: Von den im Untersuchungszeitraum bei dia -b e t i s c h e r G l a s k ö r p e r b l u t u n g durchgeführten 240 zirkulären Netzhautkryokoagulationen mussten sich 33,8% keinem glaskörperchirurgischen Eingriff mehr unterziehen. Insgesamt war bei 46,7% der eingebluteten Augen schon nach 8 Wochen eine komplette Resorption erfolgt.Schlussfolgerung: Die zirkuläre Netzhautkryokoagulation stellt eine sinnvolle Behandlung dar, wenn sie bei diabetischen Glaskörperblutungen und peripherer Netzhautanlage durchgeführt wird, da der resorptive Effekt die Vitrektomiehäufigkeit deutlich mindert.
Circumferential retinal cryocoagulation is a useful preparation before pars plana vitrectomy, when done due to vitreous haemorrhage, because improved resorption lowers the vitrectomy rate. In all other cases it can be done during vitrectomy. Adjuvant circumferential cryocoagulation seems to decrease the risk of postoperative retinal detachments and rebleedings as documented in literature comparison.
