Врожденные аномалии развития дыхательной системы представляют собой довольно редкий вариант нехромосомной патологии (4,01-4,15‰ по различным данным). Но, несмотря на этот факт, вопросы фетальной хирургии таких аномалий сохраняют свою высокую актуальность, так как, во-первых, они часто сочетаются с другими аномалиями и пороками развития, во-вторых, при тяжелом течении возникает угроза гибели плода или новорожденного. Целью настоящей обзорной статьи является рассмотрение вопросов формирования, диагностики и внутриутробного хирургического лечения кистозно-аденоматозной мальформации легкого и бронхолегочной секвестрации, осложненных массивным фетальным гидротораксом. В качестве основных, наиболее распространенных в мировой практике методов хирургического лечения в сравнительном аспекте рассмотрены лазерная абляция питающей мальформированный участок артерии и торакоамниотическое шунтирование. Более целесообразным, на основе данных многочисленных исследований, проанализированных и приведенных в данной обзорной статье, признано применение лазерной абляции питающей артерии как современного, радикального и высокоэффективного метода фетальной хирургии, обеспечивающего больший процент живорождения, выживаемости (около 100% относительно 59-68% при торакоамниотическом шунтировании), позволяющего увеличить гестационный возраст при рождении (до 39,1 нед.), снизить процент опасных осложнений и оперативных вмешательств в постнатальном периоде (до 18,2-20%), необходимость интенсивной терапии и кислородной поддержки новорожденных. Лазерная абляция дает возможность в 91,7% случаев полностью прекратить кровоснабжение измененного участка легочной паренхимы и почти во всех описанных случаях приводит к его дальнейшему некрозу и лизису в постнатальном периоде жизни ребенка.
