Objective: Infections caused by bacteria growing in blood cultures are important medical problems that cause high morbidity and mortality. The aim of this study is to examine the changes in antibiotic susceptibility and the bacteria isolated from blood samples from various clinics in the five-year period and to determine the antimicrobials that can be chosen in empirical treatment in blood stream infection in our hospital. Methods: Blood samples from our laboratories were evaluated retrospectively between during 2013-2017. Samples were incubated in an automated system of BACTEC-FX(BD,USA). Conventional methods and automated systems (Phoenix,BD,USA) have been used to identify bacteria. Antibiotic susceptibilities of isolated bacteria were evaluated using the automated systems (Phoenix,BD,USA). Results: A total of 4597 bacterial reproductions that isolated from the blood samples including 1972 (42.9%) gram positive and 2625 (57.1%) gram negative were included in the study. Bu çalışmanın amacı, beş yıllık süreçte çeşitli kliniklerden gelen kan örneklerinden izole edilen bakteriler ve antibiyotik duyarlılıklarının yıllar içerisindeki değişiminin irdelenmesi ve hastanemizde dolaşım sistemi enfeksiyonlarında ampirik tedavide seçilebilecek antimikrobiyallerin belirlenmesidir. Yöntem: 2013-2017 yılları arasında laboratuvarımıza gelen kan örnekleri retrospektif olarak taranmıştır. Örnekler BACTEC-FX (BD, USA) otomatize sisteminde inkübe edilmiştir. Bakterilerin tanımlanmasında konvansiyonel yöntemler ve otomatize sistem kullanılmıştır (Phoenix, BD, USA). Çeşitli klinik örneklerden izole edilen Stenotrophomonas maltophilia suşlarında antibiyotiklere direnç oranı 113 Öz Amaç: Stenotrophomonas maltophilia, (S.maltophilia) özellikle hastanede uzun süre izlenen hastalarda fırsatçı enfeksiyonlara neden olan çoklu ilaca dirençli non-fermentatif gram negatif basildir. Klinik örneklerden izole edilen S.maltophilia suşları rutinde test edilen penisilin, sefalosporin, aminoglikozid, kinolon, karbapenem gibi antibiyotiklere yüksek düzeyde çoğul direnç göstermekte, bu nedenle tedavisi oldukça zor olmaktadır. Çalışmamızda hastanemizde çeşitli klinik örneklerden izole edilmiş olan S.maltophilia suşlarının direnç profilini belirlemeyi amaçladık. Gereç ve Yöntem: Çalışmamızda altı yıllık dönemde servis ve polikliniğe başvuran hastalardan izole edilmiş 202 S.maltophilia suşunun dağılımı ve antibiyotik duyarlılığı retrospektif olarak incelenmiştir. Antibiyotik duyarlılıkları ve suş tanımlama konvansiyonel yöntemler ve BD Phoenix 100 otomatize sistemi (Becton-Dickinson, ABD) kullanılarak belirlenmiştir. Antibiyotik duyarlılığını belirlemede zon çapları 2010-2014 tarihleri arasında CLSI, 2015 yılında EUCAST kriterleri doğrultusunda değerlendirilmiştir. Bulgular: Toplam 202 suşun 126 (%62,4)'sı servisten, 58 (%28,7)'i yoğun bakım ünitesinden ve 18 (%8,9)'i poliklinikten takip edilen hastalardan izole edildi. Bu hastaların 122 (%60,4)' si erkek, 80 (%39,6)'i kadın hastalardan oluşmaktaydı. Bu suşlar en sık solunum yolu örneklerinden (n=69) izole edilmiştir. Bu suşların en duyarlı olduğu antibiyotik %1 (2/193) direnç oranı ile trimetoprim-sulfametoksazol olarak belirlenmiştir. Sonuç: S.maltophilia suşlarında direnç oranlarımız ülkemizde bildirilen oranlarla kıyaslandığında trimetoprim-sulfametoksazolde düşük, levofloksasin ve siprofloksasinde kısmen yüksek, diğer antibiyotiklerde benzer olduğu bulunmuştur. Direnç oranları merkezler arasında farklılık gösterebildiğinden; her merkezin kendi direnç oranlarını belirlemesi, hangi hastada stenotrophomonas enfeksiyonunun düşünülmesi gerektiğinin bilinmesi gibi oldukça önemlidir.
Introduction:Alternaria is a common saprophyte, which is usually not pathogenic in humans. Generally, local wounds infections of Alternaria occur with presence of immunosuppression factors such as HIV infection and renal transplant patients.Case Presentation:We reported a case of wound infection induced by Alternaria spp. in a renal transplant patients. The main interest in this case was the rareness of the cutaneous alternariasis, its clinical aspects and good response to therapy. Recognition of Alternaria spp. as potential opportunistic pathogens is important for differential diagnosis of dermatological lesions, such as granulomatous or ulcerative lesions in immunocompromised patients.Conclusions:Alternariasis or similar cases may be increased due to the increased number of immunosuppressed patients. From this point of view, skin lesions in these patients must be planned and microbiologically evaluated considering the molds.
