Penyakit komorbid Diabetes Melitus (DM) yang umum dan paling sering adalah hipertensi. DM dan hipertensi terdapat secara bersamaan pada 40%-60% penderita DM tipe 2. Tujuan penelitian ini adalah untuk mengetahui unmodifiable factors dan modifiable factors pada penderita DM tipe 2 sebagai faktor risiko hipertensi. Desain penelitian ini adalah cross sectional. Sampel penelitian adalah pasien DM tipe 2 yang berobat di poli penyakit dalam RSUD dr. Chasbullah Abdulmadjid Kota Bekasi pada tanggal 30 September-19 Oktober 2019 dengan total sampel sebanyak 292 responden. Unmodifiable factors meliputi gender, umur, pendidikan, status perkawinan, lama menderita DM, hereditas DM, hereditas hipertensi dan golongan darah. Sedangkan modifiable factors terdiri dari indeks massa tubuh, pekerjaan, aktifitas fisik dan merokok. Hipertensi adalah keadaan tekanan darah sistolik ≥ 140 mm Hg dan/atau tekanan darah diastolik ≥ 90 mm Hg. Analisis data dengan Cox regression menggunakan Stata versi 15. Persentase hipertensi pada penderita DM tipe 2 adalah 46,57%. Dari analisis multivariat faktor risiko hipertensi yang signifikan untuk unmodifiable factors adalah faktor umur > 50 tahun (Pv= 0,02; PR= 1,93) dan kelompok dengan hereditas DM yang berasal dari kakek/nenek (Pv= 0,04; PR= 1,86) dan orang tua (Pv= 0,04; PR= 1,54). Sedangkan dari modifiable factors, Indeks Massa Tubuh berat badan lebih (Pv= 0,01; PR=1,81) dan obesitas (Pv=0,02; PR=1,81), merupakan faktor risiko hipertensi yang signifikan. Disarankan agar terhadap pasien DM tipe 2 terutama bila disertai dengan berat badan berlebih atupun obesitas perlu diberikan informasi lengkap tentang faktor risiko hipertensi.
Coronavirus Disease 2019 (COVID-19) merupakan jenis penyakit menular baru yang sampai saat ini masih ditetapkan sebagai pandemi. Berbagai studi menyebutkan risiko kematian COVID-19 lebih tinggi pada kelompok lanjut usia terutama dengan riwayat penyakit penyerta. Ketersediaan rumah sakit dan laboratorium pemeriksa COVID-19 yang tidak adekuat juga dapat memberikan pengaruh terhadap tingginya angka kematian COVID-19. Studi ini bertujuan untuk membandingkan tingkat kematian COVID-19 di wilayah Indonesia dengan mengontrol variabel usia serta didukung dengan analisis ketersediaan rumah sakit dan laboratorium pemeriksa COVID-19. Penelitian ini menggunakan desain studi potong lintang (cross sectional) dengan data sekunder laporan COVID-19 nasional di Kementerian Kesehatan periode Maret 2020 – Mei 2021. Analisis dilakukan dengan metode standardisasi langsung terhadap variabel usia sehingga dihasilkan nilai Age Standardized Mortality Rate (ASMR). Hasil penelitian menunjukkan bahwa kematian COVID-19 di Indonesia tertinggi yaitu pada kelompok lanjut usia (>65 tahun). Setelah dilakukan standardidasi, pulau Jawa (3,82 per 100 kasus) dan pulau Sumatera (3,76 per 100 kasus) menjadi wilayah dengan rate kematian tertinggi. Berdasarkan provinsi, rate kematian tertinggi yaitu provinsi Sumatera Selatan (6,14 per 100 kasus), Jawa Timur (5,93 per 100 kasus) dan Aceh (5,59 per 100 kasus). Ketersediaan rumah sakit berbanding terbalik dengan jumlah kematian COVID-19 yang dilaporkan. Sedangkan, ketersediaan laboratorium pemeriksa COVID-19 berbanding linier dengan jumlah kasus konfirmasi COVID-19 yang dilaporkan. Ketersediaan rumah sakit dan laboratorium pemeriksa COVID-19 harus didukung dengan sarana penunjang operasional. Penelitian ini menjadi penting terutama bagi pemerintah baik pusat maupun daerah untuk menentukan prioritas kelompok intervensi serta mendukung upaya percepatan penanggulangan pandemi COVID-19.
Prediabetes adalah masalah kesehatan global. Prevalensi prediabetes meningkat signiikan di seluruh dunia dan umumnya tinggi di masyarakat serta merupakan keadaan risiko tinggi untuk DM. Obesitas memiliki peranan penting dalam patoisiologi prediabetes. Penelitian ini bertujuan untuk mengetahui apakah obesitas umum dan obesitas abdominal secara bersama-sama berhubungan terhadap kejadian prediabetes pada kelompok usia 20-65 tahun di Kecamatan Bogor Tengah berdasarkan faktor riwayat DM keluarga, jenis kelamin, umur, merokok, hipertensi, aktiitas isik dan stress. Desain studi yang digunakan adalah potong lintang dengan Cox Regression untuk analisis multivariable. Data analisis merupakan data baseline dari studi kohort faktor risiko penyakit tidak menular tahun 2012. Ada 3244 responden yang dipilih dengan teknik sampel acak. Hasil analisis menunjukkan bahwa obesitas terhadap kejadian prediabetes setelah mengendalikan faktor umur adalah obesitas umum PR 1,56 (95% CI: 1,15-2,11), obesitas abdominal PR 1,43 (95% CI; 1,09-1,85) dan obesitas umum dan obesitas abdominal secara bersama-sama PR 1,93 (95% CI; 1,62-2,28). Untuk itu, obesitas umum dan obesitas abdominal secara bersama-sama berkontribusi paling besar terhadap peningkatan prevalensi prediabetes. Peningkatan kesadaran dan skrining prediabetes pada kelompok risiko tinggi berdasarkan pengukuran IMT bersama dengan lingkar pinggang penting untuk dipertimbangkan sebagai bagian dari upaya untuk mengurangi epidemi prediabetes di masyarakat. Chronic kidney disease is the progressive loss of kidney function over months or years. The significant increase in new cases of chronic kidney disease is in line with the increasing number of patients undergoing hemodialysis as kidney replacement therapy in an effort to survive. Comorbid cardiovascular disease is a major risk factor for morbidity and mortality with chronic kidney disease. The study was conducted to determine the survival of hemodialysis patients in the group with comorbid cardiovascular disease and the group without comorbid cardiovascular disease. This study used a retrospective cohort design. The location of this study was conducted at Persahabatan Central Public Hospital, DKI Jakarta, and used secondary data from the hospital information system data from 2015 to 2019. The variables significantly related to the survival of patients undergoing hemodialysis with comorbid cardiovascular disease were age, complications of anemia, diabetes mellitus, and hypertension. The age variable has a p-value of 0.029 with an HR of 1.54 (95% CI OR 1.043-2.262). The anemia variable has a p-value of 0.013 with an HR of 1.60 (95% CI 1.117-2.515). The diabetes mellitus variable has a p-value of 0.000 with HR2.71 (95% CI 1.780-4.11). The hypertension variable has a p-value of 0.004 with HR1.79 (95% CI 1.208-2.646). In conclusion, patients undergoing hemodialysis with comorbid cardiovascular disease have a risk of death of 0.76 times compared to patients undergoing hemodialysis with the comorbid non-cardiovascular disease. This study's internal validity was not good due to selection bias and non-differential misclassification information bias. Thus, the results of this study cannot be generalized.
