Цель исследования. Проанализировать биохимические показатели (уровень активности щелочной фосфатазы (ЩФ) и уровень содержания ионов кальция Ca2+)) ротовой жидкости у лиц с травматическими переломами нижней челюсти при наличии в ротовой полости третьих моляров и без них и определить направленность влияния последних на гомеостаз и минеральный обмен в ротовой полости. Объекты и методы исследования. Обследовали 82 человека в возрасте 18–60 лет. Группа 1 – 43 пациента с переломом нижней челюсти, у которых имелось более двух третьих моляров. Группа 2 – 18 пациентов с переломом нижней челюсти, у которых не было третьих моляров. Группа 3 (контрольная) – 21 здоровый человек, у них было более двух третьих моляров. Группы не имели различий по возрасту, полу, соматическому и стоматологическому статусу. Ротовую жидкость для исследования собирали в утренние часы. Уровень активности ЩФ и уровень содержания Ca2+ определяли на полуавтоматическом анализаторе «Clima MC-15» со стандартным набором реактивов. Полученные данные обрабатывали с помощью пакета прикладных программ «Statistica 10.0». Результаты исследования и их обсуждение. Выявлены значимые различия групп 1 и 3 с помощью критерия Краскела-Уоллиса z=3,372 (H = 12,218, p = 0,002) по уровню активности ЩФ и уровню содержания ионов Ca2+ z = 2,672 (H = 8,767, p = 0,013). Заключение. Травма челюсти вызывает ишемию тканей, способствует активизации анаэробов, создавая благоприятные условия для их развития, что важно, когда в ротовой полости находятся два и более третьих моляров, которые за счет патологических процессов, связанных с этими зубами, нарушают гомеостаз рта и являются причиной роста уровня эндогенной интоксикации, что убеждает в необходимости более широко использовать дифференцированный подход к сохранению в ротовой полости третьих моляров.
Цель исследования. Анализ иммунологических показателей (уровней содержания интерлейкинов в ротовой жидкости) у пациентов с травматическими переломами нижней челюсти. Объекты и методы исследования. Группа 1 включала 61 человека в возрасте 18-58 лет с диагнозом «травматический перелом нижней челюсти». Данные здоровых соматически и стоматологически лиц взяты из специальной литературы и учтены как группа 2 (контрольная). Определение уровней содержания интерлейкина-1β (IL-1β), интерлейкина-6 (IL-6), интерлейкина-8 (IL-8) в ротовой жидкости выполняли при помощи иммуноферментного анализа, результат выражали в пг/мл. Полученные данные подвергали статистической обработке при помощи пакета прикладных программ Statistica 10.0. Результаты исследования и их обсуждение. Уровень содержания IL-1β в ротовой жидкости группы 1 составил 14,15 (9,85-23,30), уровень IL-6 был 16,95 (7,05-85,00), уровень содержания IL-8 равнялся 1705,0 (690,0-1965,0). При сравнении показателей групп 1 и 2 по критерию Краскела-Уоллиса для множественных сравнений были выявлены достоверные различия уровня содержания в ротовой жидкости IL-6z = 4,16 (H = 20,23, p = 0,00) и уровня содержания IL-8z = 3,32 (H = 11,82, p = 0,03). Заключение. Профиль уровня содержания интерлейкинов в ротовой жидкости информативно отражает как наличие травматического повреждения нижней челюсти, так и изменение гомеостаза полости рта при указанном заболевании, а также характеризует влияние последнего на организм в целом. Это дает основание для использования указанных показателей при оценке влияния отдельных негативных факторов полости рта на клиническое течение травматического повреждения челюсти.
