Lesões císticas radiculares, quando próximas a região de seio maxilar, podem decorrer em cistos inflamatórios invasivos, adentrando o seio maxilar. O processo inflamatório pode ser danoso para ambas as estruturas podendo resultar em uma comunicação buco-sinusal de difícil regeneração. O objetivo deste trabalho é discutir, em forma de relato de caso clínico, as condutas terapêuticas utilizadas na resolução de uma extensa lesão cística em região de seio maxilar originada por um tratamento endodôntico malsucedido. A lesão cística radicular inflamatória demonstrou associação à região periapical do dente 27, com rompimento da lâmina dura do soalho do seio maxilar, gerando uma comunicação buco-sinusal. Foi realizada a extração do dente 27, apresentando tratamento endodôntico malsucedido, com obturação intracanal inadequada e aquém do forame apical. Posteriormente, foi realizada punção aspirativa via comunicação buco-sinusal, curetagem do sítio cirúrgico com posterior levantamento de seio fechado, enxertia e utilização de membranas de colágeno. O procedimento de punção aspirativa da lesão cística associada à técnica de levantamento de soalho de seio maxilar com posterior enxertia e uso de membranas de colágeno, possibilitaram a eliminação do cisto radicular inflamatório instalado no seio maxilar, demonstrando o fechamento da comunicação buco-sinusal e ganho de volume ósseo local. Em adição, a permanência de um tratamento endodôntico inadequadamente instrumentado e obturado pode desencadear no desenvolvimento de um cisto inflamatório, possibilitando acometer estruturas adjacentes, levando injurias adicionais a estas estruturas.
