Цель работы-оценить целесообразность применения анти-VEGF терапии перед выполнением факоэмульсификации катаракты (ФЭК) для предотвращения прогрессирования диабетического макулярного отека (ДМО). Материал и методы. В ретроспективное исследование включено 15 пациентов (15 глаз) с сахарным диабетом 2-го типа, которым было выполнено хирургическое вмешательство по поводу катаракты. В 1-ю группу включено 8 пациентов, получавших анти-VEGF терапию перед оперативным лечением. Последняя инъекция препарата Афлиберцепт выполнялась не позднее чем за 2 нед до операции независимо от степени увеличения толщины центральной зоны сетчатки (ТЦЗС). Во 2-ю группу вошли 7 пациентов, не получавших анти-VEGF терапию перед оперативным лечением. Все пациенты прошли стандартную предоперационную подготовку. Оценивали изменение остроты зрения и ТЦЗС на 5-е и 14-е сутки после ФЭК. Результаты. На 5-е сутки среднее повышение остроты зрения составило 0,34 ± 0,05 в 1-й группе и 0,275 ± 0,070-во 2-й группе. Среднее снижение ТЦЗС в 1-й группе составило 47 мкм, при этом во 2-й группе отмечено увеличение ТЦЗС в среднем на 135 мкм. На 14-е сутки в 1-й группе острота зрения была выше исходной на 0,49 ± 0,10, а во 2-й группе ниже достигнутой на 5-е сутки после ФЭК на 0,21 ± 0,07. Среднее снижение ТЦЗС в 1-й группе составило 80 мкм, среднее увеличение во 2-й группе-165 мкм. Заключение. У пациентов с ДМО перед оперативным лечением катаракты необходимо проводить анти-VEGF терапию при наличии увеличения ТЦЗС независимо от остроты зрения, так как ориентироваться на визометрию не представляется возможным из-за мутных хрусталиковых сред. Применение афлиберцепта у пациентов с ДМО по крайней мере за месяц до хирургического лечения катаракты позволяет получить положительный результат в отношении повышения остроты зрения и улучшения анатомических показателей.
