Hiperkolesterolemia akibat mengkonsumsi makanan tinggi lemak mampu meningkatkan radikal bebas sehingga dapat memicu terjadinya gangguan reproduksi maskulina. Daun pepaya Jepang (Cnidoscolus aconitifolius) mengandung bioaktif berpotensi sebagai antikolesterol dan antioksidan. Tujuan penelitian ini untuk mengetahui efek pemberian ekstrak daun pepaya Jepang dalam menurunkan kadar kolesterol, memperbaiki morfometri, dan struktur histologi testis mencit jantan akibat diet aterogenik. Penelitian ini menggunakan Rancangan Acak Lengkap terbagi dalam kelompok kontrol negatif (pakan pur), kontrol positif (pakan pur + diet aterogenik), dan perlakuan ekstrak daun pepaya Jepang (dosis 11,2, 16,8, 22,4, dan 28mg/20gBB + pakan pur + diet aterogenik) dengan 4 pengulangan. Data warna, bentuk, permukaan testis dianalisis deskriptif. Data kadar kolesterol, panjang, diameter, berat, dan perlemakan testis dianalisis menggunakan uji ANOVA. Data histologi testis dianalisis menggunakan uji Kruskal-Wallis. Hasil penelitian menunjukkan warna, bentuk, dan permukaan testis pada kelompok kontrol negatif maupun kelompok perlakuan tampak normal, kecuali bentuk testis mengalami atropi pada kelompok kontrol positif dan perlakuan dosis 11,2mg/20gBB. Pemberian ekstrak daun pepaya Jepang berpengaruh dalam penurunan kadar kolesterol sangat signifikan (P<0.000), perbaikan morfometri signifikan pada panjang testis (P<0.027), diameter testis (P<0.042), sangat signifikan pada berat testis (P<0.000), perlemakan di sekitar testis (P<0.002), dan penurunan tingkat kerusakan testis (P<0.000). Dosis ekstrak terbaik menghasilkan penurunan kadar kolesterol, perbaikan morfometri, dan struktur histologi testis mencit akibat diet aterogenik adalah 16,8mg/20gBB. Disimpulkan bahwa pemberian ekstrak daun pepaya Jepang dapat menurunkan kadar kolesterol, memperbaiki morfometri, dan kerusakan struktur histologi testis mencit jantan akibat diet aterogenik. Konsumsi daun pepaya Jepang dapat menurunkan kadar kolesterol dan memperbaiki kerusakan testis akibat hiperkolesterolemia.
