INTRODUÇÃO: A infecção pela Síndrome Respiratória Aguda Grave por Coronavírus 2 (SARS-CoV-2) geralmente se manifesta por sintomas de acometimento respiratório, mas também pode ter repercussões extrapulmonares, como complicações cardíacas. Por outro lado, biomarcadores são entidades mensuráveis que podem apontar a existência ou gravidade de certas doenças. Nesse sentido, a alteração dos níveis de troponina I, um biomarcador de lesão cardíaca, torna-se relevante na avaliação de pacientes acometidos pela COVID-19. OBJETIVO: Investigar a relação da troponina I com prognósticos de COVID-19 e fatores associados. METODOLOGIA: Trata-se de uma revisão integrativa da literatura indexada nas bases de dados Embase, PubMed, Scielo, LILACS, Cochrane e Google Scholar. Realizou-se a pesquisa em março de 2022 utilizando os descritores “COVID-19”, “Biomarcadores” e “Troponina I”, os quais foram cruzados pelo operador “AND”. Incluiu-se metanálises e ensaios clínicos randomizados e controlados publicados em inglês, português e espanhol durante os últimos 3 anos. Excluiu-se artigos incompletos, indisponíveis e os não correspondentes ao objetivo do estudo. Encontrou-se 6029 artigos, dos quais 458 se adequaram aos critérios de inclusão e 18 aos critérios de exclusão. RESULTADOS: Notou-se nos estudos que pacientes com troponina I elevada na admissão e durante a internação apresentaram índice de mortalidade maior quando comparados a pacientes com níveis séricos normais. (1,92 vezes em estudo com 10.449 pessoas, podendo chegar a 4.07 vezes como foi evidenciado em outra pesquisa). Além disso, constatou-se que a elevação dos níveis de troponina I se relacionou à gravidade dos casos de COVID-19, estabelecendo assim associação com admissão na unidade de terapia intensiva. Destaca-se, ainda, que os estudos demonstraram que o aumento do biomarcador em questão foi maior em homens idosos e em pacientes com comorbidades preexistentes como hipertensão, doença arterial coronariana e outras doenças cardiovasculares, assim verificou-se aumento no tempo de internação desses pacientes em cerca de 2,5 dias. CONCLUSÃO: Concluiu–se que a Troponina I é um preditor de mortalidade e desfechos adversos, principalmente, em pacientes com doenças cardiovasculares preexistentes como hipertensão e doença arterial coronariana e em homens idosos. Dessa forma, estudos sobre a relação da troponina I com prognósticos ruins devem continuar sendo realizados para detecção precoce de eventos trágicos.
