Objetivo: Analisar o adoecimento por tuberculose entre profissionais de saúde em um município da Região Norte do Brasil. Metodologia: Trata-se de um estudo transversal descritivo, utilizando abordagem quantitativa, realizado a partir do levantamento das variáveis sociodemográficas, clínicas e do tratamento, no Sistema de Informação de Agravos de Notificação, no período de 2015 a 2018, e analisados por meio de distribuição de frequência, após atender aos preceitos éticos. Resultados: Dos 40 casos de tuberculose entre os profissionais de saúde durante o período analisado, a maioria pertencia ao sexo feminino, entre a faixa etária de 31 a 40 anos, autodeclarada parda, com ensino médio completo, não apresentavam doenças e agravos associados, foram notificados em 2016, casos novos, com a forma clínica pulmonar, radiografia de tórax suspeito, baciloscopia de escarro negativa, cultura de escarro e teste molecular rápido para a tuberculose não realizados, teste de sensibilidade em branco/ ignorado, HIV negativo, notificação na referência terciária ou na Atenção Primária à Saúde, com dois contatos identificados e nenhum examinado, não realização do Tratamento Diretamente Observado para a tuberculose, baciloscopias de escarro para controle mensal até o terceiro mês, tempo médio de tratamento de 177 dias, baixo percentual de cura e elevado para o abandono. Conclusão: Tais achados permitem uma reflexão sobre a efetividade das estratégias implementadas de vigilância em saúde do trabalhador e os desafios que permeiam a estrutura dos serviços de saúde para o controle da doença.
A avaliação do Programa de Controle da Tuberculose contribui para a identificação das dificuldades que permeiam a implementação de políticas públicas definidas para o controle da doença. Assim, este estudo teve como objetivo avaliar o desempenho dos Programas Municipais de Controle da Tuberculose em Rondônia e identificar os aspectos associados a tal desempenho. Realizado um estudo ecológico em 39 municípios do estado em 2018. Indicadores de desempenho dos programas municipais foram calculados para posterior análise de cluster, que os agrupou de acordo com o desempenho. A análise de correspondência múltipla foi utilizada para identificar as variáveis (demográficas, sociais, epidemiológicas e relacionadas ao sistema de saúde) associadas aos grupos formados. Os resultados mostraram que o cluster 1, com 25 municípios, apresentou desempenho regular e foi associado à pequeno e grande porte populacional, alta incidência de tuberculose e aids e baixa cobertura do Programa de Agentes Comunitários de Saúde e Estratégia Saúde da Família. O cluster 2, com 10 municípios, apresentou desempenho insatisfatório e foi associado a médio porte populacional, baixa incidência de tuberculose e aids, alta cobertura do Programa de Agentes Comunitários de Saúde e Estratégia Saúde da Família, ausência de áreas indígenas e piores indicadores sociais. O cluster 3, com quatro municípios, apresentou desempenho satisfatório e esteve associado à presença de áreas indígenas e melhores indicadores sociais. É possível concluir a necessidade de uma maior atenção às desigualdades sociais nos municípios com pior desempenho e também de um maior protagonismo da Atenção Primária à Saúde para o enfrentamento da doença.
Objetivo: Analisar os casos de tuberculose (TB) sob o regime do Tratamento Diretamente Observado (TDO) em Porto Velho-RO, entre 2010 a 2017. Método: Estudo descritivo, realizado de forma transversal com abordagem quantitativa, a partir do levantamento dos registros das variáveis sociodemográficas e clínicas no Sistema de Informação de Agravos de Notificação e analisados por meio de distribuição de frequência, após atender os preceitos éticos. Resultados: Foram notificados 3.738 casos, desses 359 (9,6%) realizaram o TDO, cuja média de idade foi de 36,2 anos (dp = ± 16,5). A maioria era do sexo masculino (67,7%), raça/cor parda (75,0%), cinco a oito anos de estudo (23,1%), residiam na zona urbana (84,6%), forma clínica pulmonar (88,3%), caso novo (72,7%), raio-X suspeito de TB (75,7%), baciloscopia de escarro positiva (63,2%), HIV negativo (53,7%), baciloscopia de escarro para o controle mensal não realizada, tempo médio de tratamento de 165,09 dias (dp= ±88,92), baixa taxa de cura (61,3%) e elevada para o abandono (20,6%), transferência (14,8%) e óbito (2,5%). Conclusão: Ressalta-se que o baixo percentual do TDO implica em desfechos desfavoráveis e, consequentemente, em fragilidades nas ações de vigilância e controle da TB.
