The number of patients with nosocomial post-cesarean endometritis is steadily growing. Aim: to study the etiology of nosocomial post-cesarean endometritis and to propose a rationalized antimicrobial therapy. Material and methods. Sixty six puerperas with post-cesarean endometritis were examined. The spectrum of microflora and its resistance to antimicrobial agents were determined using an automatic microbiological analyzer based on mass spectrometry. Results. Enterococci, E. coli, staphylococci and streptococci were the most clinically significant pathogens found in the examined patients with post-cesarean endometritis. The microflora spectrum in the endometrium reflected the previous «antibacterial history»; this association must be taken into account when developing antibacterial therapy. Conclusion. If the new mother was previously treated with antibacterial agents, it is recommended to use a combination of the reserve antibiotics in order to block the entire spectrum of possible multidrug-resistant pathogens. In cases of the resistant Gram-positive microflora (MRSE, MRSA, E. faecium), it is advisable to prescribe vancomycin. For the resistant Gram-negative microflora (Enterobacter spp., Citrobacter spp., E. coli-ESBL+ and Klebsiella spp.-ESBL+), vancomycin should be combined with carbapenems.
The executed pharmacoepidemiological study aimed at assessing stereotypes of perioperative antimicrobial prophylaxis in abdominal delivery in maternity hospitals of the city of St. Petersburg showed that more than in 30% of cases there was an inefficient use of antimicrobial chemotherapy drugs, which in turn contributes to the increase in the frequency of postoperative septic complications.
В современных условиях изменились подходы к тактике родоразрешения при осложненном тече-нии беременности и родов. В целях профилактики тяжелых осложнений со стороны матери и плода все чаще предпринимается оперативное родораз-решение, в частности, операция абдоминального кесарева сечения [5, 21]. Возможность сохранения беременности при тяжелой соматической пато-логии, нейроэндокринных заболеваниях, воздей-ствии повреждающих факторов внешней среды и др. способствовали увеличению частоты дето-рождений у лиц с различными степенями имму-нодефицита, снижением защитных возможностей организма. Все указанное определяет достаточно высокий уровень гнойно-воспалительных ослож-нений послеродового периода, который не может быть полностью ликвидирован профилактически-ми мерами, осуществляемыми при проведении родоразрешающих операций, при выполнении принятых противоэпидемических мероприятий в акушерских стационарах [16, 20, 28].Остается достаточно высокой частота тяже-лых форм послеродовой инфекции -перитони-та, сепсиса, то есть заболеваний, для исхода ко-торых решающее значение имеет своевременная диагностика со своевременным началом лечения.Послеродовый сепсис развивается на фоне особого состояния макроорганизма, обусловлен-ного изменившейся под воздействием различных факторов реактивностью. Течение заболевания во многом определяется характером возбудителей в условиях современной антимикробной химио-терапии [4, 8, 13, 20].Характерным микробиологическим признаком современного течения гнойно-воспалительных заболеваний после самопроизвольных родов и кесарева сечения является высокая частота ас-социаций условно-патогенных аэробных и некло-стридиальных анаэробных микроорганизмов, составляющих нормальную (индигенную) микро-флору нижних половых путей женщины. Важное значение микробных ассоциаций в этиопатогенезе послеродовой инфекции обусловлено тем, что па-тогенность облигатных анаэробов в присутствии факультативно-анаэробных бактерий повышается за счет их синергизма. аэробно-анаэробные ассо-циации, обладая более выраженными патогенны-ми свойствами, вызывают наиболее тяжелые фор-мы заболевания [7, 12, 29].В настоящее время из клиники практически полностью ушли патогенные возбудители. Тем не менее немаловажную роль в развитии по-слеродовой инфекции играют такие облигатные патогены, как Chlamydia trachomatis, и условно-патогенные микроорганизмы -Mycoplasma hominis, Urea plasma urealyticum, Gar dnerella vaginalis, грибы рода Candida и др. Мечникова, Санкт-Петербург, российская Федерация Цель: анализ частоты и структуры послеоперационных инфекционно-воспалительных осложнений после абдоминального родоразрешения у родильниц, находившихся на лечении в условиях специализированного отделения стационара третьего уровня за 2017 г. Материал и методы: проведено обследование и лечение 66 родильниц с инфекционными осложнениями после кесарева сечения. Спектр внутрибольничной микрофлоры и резистентность к антимикробным препаратам определены с использованием автоматического микробиологического анализатора на основе масс-спектрометрии. Результаты: основной регистрируемой нозологической формой явилась инфекция области хирургического вмешательства. В 3% случаев послеоперационный метроэндометрит осложнился несостоятельностью швов на матке с развитием акушерского перитонита, а в 7,6% -сепсисом. Структура изолированной микрофлоры при послеоперационной инфекции различалась после планового и экстренного родоразрешения, что необходимо учитывать при разработке локального формуляра периоперационной антибиотикопрофилактики и терапии. Заключение: в настоящее время имеет место недооценка тяжести течения метроэндометрита после кесарева сечения с преждевременной выпиской или запоздалый перевод из родильного дома в специализированное обсервационное отделение многопрофильной больницы. Каждый пятый послеоперационный метроэндометрит вызван полирезистентной госпитальной микрофлорой. BACKGROUND: There is an insufficient number of studies studying the association of postpartum infectious and inflammatory complications with anemia of inflammation, which undoubtedly contributes to the risk of postpartum infection and the severity of its course. AIM: To assess the prevalence of anemia of inflammation in pregnant women, to determine its prognostic value as an independent predictor of the progression of endometritis after caesarean section. MATERIALS AND METHODS: A retrospective study of case histories and data from primary medical records of 648 puerperas delivered by caesarean section has been carried out. 406 of the patients have been diagnosed with endometritis after caesarean section and 242 with a physiological course of the postoperative period. Anemia during pregnancy has been diagnosed with a decrease in hemoglobin 110 g/l (in the first trimester) and 105 g/l (in the second and third trimesters). Primary iron deficiency was determined by the level of serum ferritin. Anemia of inflammation has been diagnosed after exclusion of iron deficiency in the presence of documented signs of an inflammatory process (chronic diseases) and in accordance with the international hematological criteria: normochromic normocytic anemia, normo-/hyperferritinemia (serum ferritin 30 g/l), hypotransferrinemia (transferrin 2 g/l) and the level of C-reactive protein in the blood ( 6 mg/l). RESULTS: Anemia of inflammation can be attributed to the most significant predictors of the development of endometritis. The odds ratio for this factor was 4.12 (95% confidence interval 1.286.19; p 0.001). The study has also shown that significant factors in the development of endometritis after caesarean section are obesity (odds ratio 3.45; 95% confidence interval 1.145.11; p = 0.005), asymptomatic bacteriuria (odds ratio 3.68; 95% confidence interval 1.314.94; p 0.001), carbohydrate metabolism disorders (odds ratio 2.84; 95% confidence interval 1.543.52; p = 0.012) and arterial hypertension (odds ratio 2.14; 95% confidence interval 1.124.04; p = 0.032). CONCLUSIONS: The prevalence of anemia of inflammation in pregnancy is 2.1%. Anemia of inflammation complicates the course of pregnancy 5.5 times more often in puerperas with endometritis after caesarean section (27.4 vs. 4.9%; 2 = 52.1; p 0.001), which makes it possible to classify it as a significant predictor of the development of postoperative infectious and inflammatory complications.
Purpose. To study the pharmacokinetics of -lactam antibiotics in the development of endometritis after cesarean section to select the optimal dosage regimen. Methods. A prospective, randomized, single-center study included 52 women in puerperas with endometritis after a caesarean section, divided into four groups. The patients of the first group (n = 17) received a course of ceftriaxone bolus in a single dose of 2.0 g (n = 10) and in the mode of prolonged perioperative infusion (n = 7). The patients of the second group (n = 10) received cefepim bolus at a dose of 2.0 g 2 times a day (n = 5) and in the extended infusion mode (n = 5). The patients of the third group (n = 14) received amoxicillin / clavulanic acid (Amoxiclav 1000 mg + 200 mg) bolus at a dose of 1.0 g 3 times a day (n = 7) and in the extended infusion regimen (n = 7). The patients of the fourth group (n = 11) received ampicillin / sulbactam (Ampisid 1000 mg + 500 mg) bolus at a dose of 1.0 g 4 times a day (n = 6) and in the extended infusion regimen (n = 5). We have compared the concentration of the studied antibiotics in the uterine cavity in the four groups using high performance liquid chromatography. Results. The effective bactericidal concentration (C 4MIC) was not maintained throughout the entire dose interval in any of the treatment groups. The clinical efficacy and safety of the studied antibiotic regimens were similar. However, prolonged infusion of cefepime and aminopenicillins provided significantly higher concentrations in lochia. Conclusion. Prolonged intravenous infusion of cefepime, ceftriaxone, amoxicillin / clavulanic acid and ampicillin / sulbactam in the treatment of endometritis after a caesarean section improves the pharmacokinetic / pharmacodynamic characteristics of these -lactams in the uterine cavity, compared with the traditional bolus administration.
The objective of the study was to compare the uterus microflora in healthy women and in the patients with postpartum endometritis using the method of quantitative assessment of contamination of the material. Further examination revealed polymicrobial etiology of the postpartum endometritis. The leading role in the etiological structure belongs to opportunistic microorganisms: enterococci, enterobacteria (preferably Enterococcus faecalis and Proteus mirabilis), and non-spore forming anaerobes (predominantly - Peptostreptococcus spp. аnd Bacteroides spp.), located in the uterus, in most cases, a bulk quantity as aerobically-anaerobic associations.
