Атлас явился итогом аутопсийной работы патологоанатомической службы Москвы по посмертной диагностике проявлений и осложнений COVID-19 в 2000 наблюдениях – самого большого в мире объема вскрытий умерших от COVID-19. Это стало возможным благодаря тому, что в Москве в 100 % случаев летальных исходов с подозрением на COVID-19 производятся патологоанатомические вскрытия с детальным клинико-морфологическим анализом. Результаты исследований легли в основу новой методики, которая детализирует рекомендации ВОЗ по выбору причин смерти у умерших при COVID-19. Применение на практике этой методики позволило усовершенствовать не только учет случаев смерти от COVID-19, но и оказало существенное влияние на подходы к лечению таких пациентов. Вкладом в теорию патологии является введение понятия клинических и морфологических «масок» COVID-19. В атласе представлен широкий спектр проявлений и осложнений COVID-19, что позволит наметить пути изучения их патогенеза и морфогенеза, совершенствовать методы диагностики и лечения больных, детальную классификацию причин смерти, прямо или косвенно связанных с COVID-19.
Background: Pathological anatomy, patogenesis and the morphogenesis of manifestations and complications of COVID-19 remain insufficiently studied. The fullest information on structural bases of organs and tissues alterations by new coronavirus disease can be obtained as a result of autopsies. Aims: The aim of the study was to study the morphological changes of lungs and other organs of the autopsies of COVID-19 deceased persons. Results of 2000 autopsies of people who died of a severe form of COVID-19 in Moscow, consisting of 1212 men and 788 women, from March 20 to May 22, 2020 (a ratio 1.54:1) aged from 20 to 99 years (on average 68.515.63 years) were presented. This experience was previously generalized in the Atlas COVID-19 pathology. Autopsies were made in the converted interstationary pathoanatomical offices at strict observance of rules of biosafety according to standard and legal documents of WHO, Russian Ministry of Health and Rospotrebnadzor. Results: Morphological changes of lungs with varying severity and extent were detected in all examined cases; however, damage to other organs was also common, which in some cases prevailed over pulmonary changes and was the cause of death. The main morphological changes in lungs were diffuse alveolar damage and microangiopathy, alveolar hemorrhage syndrome, thrombosis, and thromboembolism. Conclusion: The involvement of the lungs, other organs, and vascular system in the pathological process is a result of multiple factors. It is advisable to implement clinical and morphological masks of COVID-19.
Введение. Успехи современной медицины при оказании экстренной помощи перенесшим тяжелую черепно-мозговую травму, а также реабилитация таких пациентов все чаще заставляют специалистов разного профиля сталкиваться с различными нарушениями в гипоталамо-гипофизарной системе, требующими коррекции. Несмотря на актуальность этой проблемы, а также на многочисленные клинические исследования, несомненно указывающие на нарушения репродуктивной функции у тех, кто перенес тяжелую черепно-мозговую травму, морфологические изменения эпителио-сперматогенного слоя половых желез, а также морфогенез этих изменений остаются мало изученными. В связи с этим целью нашего исследования явилось количественное изучение морфогенеза эпителио-сперматогенного слоя у мужчин в различные сроки после тяжелой черепно-мозговой травмы. Материалы и методы. При помощи разработанного авторами способа морфометрического анализа -цитологического профиля сперматогенеза -проведено изучение эпителио-сперматогенного слоя мужских половых желез на каждой из шести стадий цикла развития сперматогенных клеток у 45 умерших через 1-3 суток после тяжелой черепно-мозговой травмы (n=9), через 6-9 суток (n=10) и через 15 суток и более (n=7). Группу сравнения составили 19 секционных наблюдений практически здоровых лиц, умерших от черепно-мозговой травмы на месте происшествия. Выводы. Результаты проведенного исследования показали, что выраженные морфологические изменения в эпителио-сперматогенном слое выявляются уже на ранних сроках черепно-мозговой травмы. Выраженность данных изменений прямо пропорциональна длительности течения черепно-мозговой травмы и может быть оценена как «пестрая атрофия половых желез» с исходом в герминальную аплазию транзиторного характера. The available literature reports only rare cases of injuries inflicted by the speargun shots. The last publication of this sort of which we are aware dates back to 1960. This article describes an example from the expert practice concerning the accident involving a wound caused by the speargun shot to a member of an amateur fishing party. The authors examined the weapon of injury (a 'Zelinka' speargun), the harpoon, its head, and fragments of the skin with the traces of the punctured wound obtained from the corpse of the victim. The investigative experiment included the underwater shooting from different distances with the use of a pork carcass as the target. The results of the comparative studies gave evidence of the possibility of formation of the wounds caused by exactly that harpoon, its head and cord which served as the weapons of the wound experienced by the victim. Moreover, the distance and the direction of the shot as well as a number of important concomitant circumstances of the accident were determined.
