Background Although there are simple and low-cost measures to prevent healthcare-associated infections (HAI), they remain a major public health problem. Quality issues and a lack of knowledge about HAI control among healthcare professionals may contribute to this scenario. Aim To present the implementation of a project to prevent HAI in intensive care units (ICU) using the quality improvement (QI) collaborative model Breakthrough Series (BTS). Methods A QI report was conducted to assess the results of a national project in Brazil between January/2018 and February/2020. A one-year pre-intervention analysis was conducted to determine the incidence density baseline of the three main HAI: central line-associated bloodstream infections (CLABSI), ventilation-associated pneumonia (VAP), and catheter-associated urinary tract infections (CA-UTI). The BTS methodology was applied during the intervention period to coach and empower healthcare professionals providing evidence-based, structured, systematic, and auditable methodologies and QI tools to improve patients’ care outcomes. Results A total of 116 ICUs were included in this study. The three HAI showed a significant decrease of 43.5%, 52.1%, and 65.8% for CLABSI, VAP, and CA-UTI, respectively. A total of 5,140 infections were prevented. Adherence to bundles inversely correlated with the HAI incidence densities: CLABSI insertion and maintenance bundle (R = -0.50, P-value = 0.010 and R = -0.85, P-value <0.001, respectively), VAP prevention bundle (R = -0.69, P-value <0.001), and CA-UTI insertion and maintenance bundle (R = -0.82, P-value <0.001 and R = -0.54, P-value = 0.004, respectively). Conclusion Descriptive data from the evaluation of this project show that the BTS methodology is a feasible and promising approach to preventing HAI in critical care settings.
Objective: to present an evaluation model for developing effective indicators for the Vigilância Sanitária (VISA) (Sanitary and Health Surveillance) actions in Brazil. Methods: an exploratory, evaluative study from primary sources data - electronic form by key informants and reports on national, international and secondary benchmarking visits - documental analysis. Results: the diversity of VISA management practices made it possible to define the evaluation object: Health Surveillance Actions - as an organized action system. The components: management; regulation; sanitary and health control; sanitary and health risk monitoring and information, communication and health education were derived from the aims of the intervention and composes a central structure of the Theoretical Model and the Logical Model Actions of VISA in the Sistema Nacional de Vigilância Sanitária (SNVS) (National Sanitary and Health Surveillance System) . Conclusions: The Vigilância Sanitária (VISA) (Sanitary and Health Surveillance) accumulates experience with the process indicators for monitoring actions, being incipient are capable of evaluating its impact. The theoretical resources in the evaluation area support the information management in the VISA field and besides contributing for studies on social determinants and the incorporation of analyses in historic series.
Introdução: A avaliação focada na utilização constitui-se em referencial teórico importante a ser observado na proposição de um caminho entre tantos que o campo da avaliação oferece como suporte à qualificação da gestão em saúde. O monitoramento do desempenho é considerado uma estratégia impulsionadora à racionalização da gestão e das decisões. Objetivo: Apresentar o caminho percorrido pela Agência Nacional de Vigilância Sanitária e pelo Hospital Alemão Oswaldo Cruz no processo de construção participativa de mecanismos para o monitoramento do desempenho da gestão em instâncias subnacionais, como parte da implantação da ação – mecanismos estruturais e técnicos – integrante da primeira linha de execução do projeto Institucionalização de Práticas Avaliativas: a gestão estratégica da vigilância sanitária baseada em evidências. Método: Descrição do processo de construção participativa de mecanismos para o monitoramento do desempenho da gestão em instâncias subnacionais, considerando duas dimensões – o envolvimento dos principais interessados na perspectiva do uso do monitoramento e a estruturação do instrumental teórico e operacional e de estratégias de utilização. Resultados: Projeto implantado em quatro instituições de Vigilância Sanitária, permeado pelo caráter participativo, desde a elaboração da estratégia, à modelagem, à formulação dos dispositivos gerenciais e de instrumentos de análise e interpretação dos indicadores. Conclusões: O projeto viabilizou a instituição de um espaço de aprendizado institucional que valorizou não só a apreciação dos resultados, mas, também, o próprio processo de produção da informação, contribuindo, assim, com a instituição de mudanças e inovações na execução das ações.
