Éxito anestésico de 1,8ml de Lidocaína 2% en exodoncia de dientes mandibulares. Estudio piloto. Anesthetic success of 1,8ml lidocaine 2% for mandibular tooth extraction. A pilot study.Resumen: Objetivo: Determinar el efecto anestésico de un cartucho de 1,8ml de anestesia lidocaína al 2% con epinefrina 1:100.000 en la técnica troncular al nervio alveolar inferior (NAI) para la exodoncia de dientes mandibulares. Material y método: Estudio piloto de carácter analítico. Participaron pacientes voluntarios del servicio Urgencia Dental de Valdivia-Chile con indicación de exodoncia en dientes mandibulares entre Mayo y Julio del año 2010. La técnica anestésica fue realizada por un solo dentista usando sólo un cartucho de anestesia al NAI. Luego de 15 minutos, se evaluó el efecto considerándose efectiva cuando no fue necesario el refuerzo anestésico durante la exodoncia de los dientes. Se analizó la relación entre el éxito anestésico con el sexo, edad, tipo y diagnóstico del diente y el nivel de dolor observado (chicuadrado y regresión logística; p< 0,05). Resultados: 62 pacientes fueron seleccionados, de los cuales solo en 47 (75,8%) se logró el éxito anestésico. No hubo asociación estadística con el sexo, edad, tipo ni diagnóstico dental y dolor percibido. Conclusión: El uso de un cartucho de 1,8ml de anestesia resultó efectiva en tres de cada cuatro pacientes atendidos por exodoncia de dientes mandibulares. Se sugiere nuevas investigaciones clínicas en relación a la efectividad de otros anestésicos con la misma dosis en el bloqueo del NAI.Palabras claves: anestesia local, lidocaína, nervio alveolar inferior, exodoncia, estudio piloto Aravena P, Bustos N, Cerón A, Castillo V & González C. Introducción.La técnica anestésica troncular al nervio alveolar inferior (NAI) o también llamada técnica a la língula mandibular o técnica de Spix, es la técnica de excelencia para el procedimientos quirúrgicos y rehabilitadores en dientes mandibulares 1 . Consiste en el depósito de anestésico local en la fosa pterigomandibular para el bloqueo nervioso del NAI y el nervio lingual, permitiendo anestesiar los dientes inferiores del tercer molar hasta la línea mediana, el tejido blando vestibular desde los premolares hacia anterior, el cuerpo de la mandíbula, ligamento periodontal y encía, los tejios subcutáneos del mentón y el labio inferior ipsilateral 2 . Es la inyección troncular con mayor frecuencia empleada en odontología y, a su vez, la con mayor tasa de fracaso anestésico reportada entre un 15% al 20% 1 .Dentro de los anestésicos locales, la lidocaína posee un inicio de acción mucho más rápido, produce una anestesia más profunda y una duración de acción más larga, por consiguiente la utilización de lidocaína al 2% con epinefrina 1:100.000 reduce el flujo sanguíneo en la zona de inyección, aumenta la duración de la acción y la zona de hemorragia también es menor 1 .La literatura describe la utilización desde 1,5ml 2,3 a 1,8ml de anestesia local para lograr el bloqueo del NAI, con una tasa de éxito que bordea el 80 -85% 1 , sin emb...
