Bedah kuku termasuk tindakan yang tidak rutin dilakukan pada praktik kedokteran. Tindakan ini menjadi tantangan bagi para klinisi karena memerlukan tindakan bedah rapi yang teliti. Bedah kuku dapat dilakukan untuk penegakan diagnosis maupun terapi berbagai kasus tumor jinak atau ganas, kasus-kasus inflamasi, dan trauma pada kuku. Beragam jenis tindakan bedah kuku memiliki prinsip yang berbeda sesuai dengan patologi kelainan kuku dan letak pada unit kuku yang terkena. Pembedahan pada kuku terutama pada matriks kuku sangat berisiko menimbulkan komplikasi seperti distrofi kuku. Tujuan pembedahan seringkali tidak tercapai pada pengambilan jaringan biopsi, karena spesimen tidak memadai untuk dilakukan pemeriksaan histopatologis yang disebabkan tindakan bedah yang tidak tepat. Oleh karena itu, penting bagi para klinisi untuk dapat memahami berbagai teknik bedah kuku.
Vaginal hygiene kemungkinan memiliki peranan penting dalam timbulnya vaginitis. Cara mencuci vagina yang tidak benar dapat menyebabkan timbulnya vaginitis pada remaja. Tujuan penelitian ini adalah untuk mengetahui hubungan antara cara mencuci vagina dengan timbulnya vaginitis pada pelajar SMA.Penelitian ini merupakan penelitian observasional analitik dengan pendekatancross sectional.Penelitian dilaksanakan pada di SMA Muhammadiyah 1 Palembang. Pengambilan sampel dilakukan dengan teknik total samplingsesuai kriteria inklusi dan eksklusi. Data mengenai cara mencuci vagina dan vaginitis diperoleh dari kuesioner.Sebanyak 83,3% subjek termasuk kategori cara mencuci vagina yang buruk, 73,5% berusia 17 tahun, dan 66,7% memiliki riwayat vaginitis. Hasil uji Chi-Square mengenai hubungan cara mencuci vagina dengan timbulnya vaginitis pada penelitian ini menunjukkan nilai p sebesar 0,000 (p<0,05). Hal ini kemungkinan akibat faktor usia remaja, kelembaban Indonesia yang tinggi, serta tingkat pengetahuan mengenai vaginal hygieneyang masih rendah.Kesimpulan,terdapat hubungan yang sangat bermakna antara cara mencuci vagina yang buruk dengan timbulnya vaginitis pada pelajar SMA.
Psoriasis pustulosa generalisata (PPG) adalah salah satu varian psoriasis pustulosa akut. PPG dapat dipicu berbagai faktor, salah satunya adalah kehamilan. Penyakit ini pada kehamilan dapat menyebabkan komplikasi maternal dan mengancam keselamatan janin. Penanganan kasus PPG pada kehamilan memerlukan pemilihan terapi efektif yang juga aman bagi janin dan bayi saat fase menyusui, salah satunya adalah siklosporin. Dilaporkan sebuah kasus PPG yang diinduksi kehamilan pada kehamilan kedua seorang wanita berusia 21 tahun yang telah mengkonsumsi kortikosteroid jangka panjang dan mengalami hipokortisol. Manifestasi klinis timbul sejak usia kehamilan memasuki trimester kedua dan berlanjut setelah melahirkan hingga masa nifas berakhir. Pasien memiliki riwayat PPG berulang sejak enam tahun yang lalu. Siklosporin diberikan pada masa postpartum dengan dosis awal 0,8 mg/kgBB/hari setelah mengalami rekalsitran terhadap pengobatan kortikosteroid sistemik dosis tinggi saat kehamilan. Perbaikan klinis terjadi setelah pemberian siklosporin dengan dosis 2,5 mg/kgBB/hari selama satu minggu. Pemberian kortikosteroid tetap diberikan dengan penurunan dosis secara bertahap dan dihentikan pada pengamatan hari ke-101. Perubahan hormonal dan imunitas selama kehamilan berperan penting dalam mencetuskan PPG. Selama kehamilan terjadi perubahan rasio estrogen dan progesteron yang akan mempengaruhi keadaan sistem imunitas selama kehamilan. Kortikosteroid sistemik menjadi pilihan terapi utama pada kasus PPG berat dalam kehamilan, namun siklosporin dapat digunakan pada kasus refrakter terhadap kortikosteroid dosis tinggi. Pada pasien ini PPG mengalami perbaikan setelah pemberian siklosporin 2,5 mg/kgBB/hari dan kortikosteroid sistemik tetap diberikan karena adanya kondisi hipokortisol. Penggunaan kortikosteroid jangka panjang dapar memberikan efek samping berupa moon face, hipertrikosis, atrofi kulit, dan osteoporosis. Selain itu gangguan maternal dapat terjadi, bayi pada kasus ini lahir dengan berat badan lahir rendah karena penggunaan kortikosteroid selama kehamilan.
One of the success indicators of the World Health Organization (WHO) leprosy eradication program is the decreasing number of new cases of pediatric leprosy with a grade 2 disability. A case of borderline lepromatous (BL) leprosy with partial claw hand in a 13-year-old boy was reported. On physical examination, we found claw fingers on the fourth and fifth fingers of the left hand accompanied by hypoesthetic erythematous plaques on both cheeks. The patient also presented with the enlargement of bilateral great auricular, ulnar, and peroneal nerves. The bacteriological examination showed the bacterial index 3+ and morphological index 35%. The results of histopathological and serological anti-phenolic glycolipid-I examinations supported the diagnosis of BL type of leprosy. Genotyping of Mycobacterium leprae by variable number tandem repeat of the patient showed 24 copies thymine-thymine-cytosine that were similar to his father, who had been diagnosed with leprosy 12 years before, without adequate therapy. The result indicated the possibility of leprosy transmission from the father to a son. This case report revealed the presence of leprosy in children with a multibacillary infection who have been living with leprosy family members. Genotyping seems to be feasible for epidemiological analysis of leprosy transmission.
