Mở đầu: Nhiễm khuẩn huyết là một hội chứng lâm sàng do kết quả của phản ứng viêm mất kiểm soát đối với nhiễm khuẩn dẫn đến rối loạn chức năng cơ quan. Sử dụng kháng sinh thích hợp cần thiết để đảm bảo kết quả điều trị tích cực. Mục tiêu: Khảo sát và so sánh việc sử dụng kháng sinh, tính hợp lý của kháng sinh và kết quả điều trị ở bệnh nhân nhiễm khuẩn huyết hoặc sốc nhiễm khuẩn trước và sau khi ban hành hướng dẫn kháng sinh, triển khai chương trình quản lý kháng sinh tại bệnh viện Thống Nhất (ASP). Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang mô tả, so sánh 2 giai đoạn được tiến hành trên bệnh nhân được chẩn đoán nhiễm khuẩn huyết hoặc sốc nhiễm khuẩn. Bệnh nhân được chọn ở 2 giai đoạn trước và sau khi triển khai ASP: giai đoạn 1 - từ tháng 01/2018 đến 6/2018 và giai đoạn 2 – từ 10/2019 đến 3/2020. Sự hợp lý của kháng sinh được đánh giá dựa trên phác đồ của Bộ Y tế năm 2015, Hướng dẫn sử dụng kháng sinh Bệnh viện Thống Nhất năm 2019 và Sanford guide năm 2020. Kết quả: Có 213 bệnh nhân được đưa vào nghiên cứu, trong đó 107 bệnh nhân ở giai đoạn 1 và 106 bệnh nhân ở giai đoạn 2. Beta-lactam và fluoroquinolone là hai nhóm kháng sinh được chỉ định phổ biến nhất. Tỷ lệ lựa chọn kháng sinh kinh nghiệm hợp lý chung tăng từ 49,5% lên 63,2% (p = 0,044). Tỷ lệ sử dụng kháng sinh kinh nghiệm hợp lý theo kháng sinh đồ của 2 giai đoạn lần lượt là 43,9% và 41% (p =0,752). Tỷ lệ bệnh nhân có kết quả điều trị thành công là 85%. Kết luận: Chương trình quản lý sử dụng kháng sinh giúp cải thiện tỷ lệ sử dụng kháng sinh hợp lý ở bệnh nhân nhiễm khuẩn huyết hoặc sốc nhiễm khuẩn tại Bệnh viện Thống Nhất.
