Đặt vấn đề: Gen MICA mã hóa cho phân tử MICA đóng vai trò như các phối tử cho thụ thể NKG2D kích thích sinh miễn dịch. Sự biểu hiện của MICA có thể gây ra bởi cảm ứng “stress” ở các tế bào bị biến đổi ác tính hoặc nhiễm virus. Đã có các nghiên cứu chứng minh mối liên quan giữa SNP rs2596542 trên gen MICA với nguy cơ mắc ung thư vòm họng, ung thư biểu mô tế bào gan do virus viêm gan C và virus viêm gan B. Tuy nhiên chưa có nghiên cứu nào được thực hiện trên bệnh nhân u lympho. Mục tiêu: Nghiên cứu này nhằm mục đích xác định SNP rs2596542 trên gen MICA và nguy cơ mắc u lympho. Đối tượng và phương pháp: Nghiên cứu được thực hiện trên 100 bệnh nhân được chẩn đoán mắc u lympho và 100 người tình nguyện khỏe mạnh. Alen của SNP rs2596542 được xác định bằng kỹ thuật realtime PCR. Kết quả: Nghiên cứu đã xác định được tỷ lệ phân bố alen của SNP rs2596542C/T trên gen MICA ở bệnh nhân u lympho lần lượt là 29,5% T và 70,5% C; tỷ lệ phân bố kiểu gen là 53% CC; 35% CT và 12% TT. Nghiên cứu không tìm thấy sự khác biệt có ý nghĩa thống kê giữa tỉ lệ kiểu gen và alen của SNP rs2596542 ở 2 nhóm bệnh và nhóm chứng.
Ung thư phổi (UTP) là một trong 3 ung thư thường gặp nhất và là nguyên nhân gây tử vong hàng đầu do ung thư trên phạm vi toàn cầu. UTP gồm 2 loại chính là UTP tế bào nhỏ (UTPTBN) và UTP không tế bào nhỏ (UTPKTBN). UTP giai đoạn tiến triển thường là di căn não. Đã có nhiều thử nghiệm lâm sàng khác nhau đánh giá hiệu quả xạ trị toàn não đơn thuần trong điều trị UTP di căn não và so sánh hiệu quả với các phương pháp điều trị khác. Kết quả đã chỉ ra rằng xạ trị toàn não đơn thuần giúp kiểm soát nhanh chóng và giảm nhẹ các triệu chứng chèn ép thần kinh, nâng cao chất lượng sống cho bệnh nhân (BN). Đối tượng và phương pháp: 32 BN UTPKTBN di căn não 1-3 ổ, đường kính lớn nhất ≤ 30mm, được xạ trị toàn não 30Gy với phân liều 3Gy/ngày, 5 ngày/tuần. Đánh giá kết quả sau xạ trị. Kết quả: Xạ trị toàn não đơn thuần làm biến mất tổn thương di căn não 6,2%, giảm kích thước ổ di căn (53,2%), kiểm soát ổ di căn ở 96,9% BN. Xạ trị toàn não đơn thuần giúp cải thiện chỉ số PS ở 84,4%; tỷ lệ BN có giảm các triệu chứng cơ năng chiếm 62,5%, trong đó có 12,5% BN hết hoàn toàn triệu chứng. Kết luận: Xạ trị toàn não đơn thuần giúp kiểm soát nhanh chóng và giảm nhẹ triệu chứng chèn ép thần kinh nội sọ, giúp nâng cao chất lượng sống cho BN.
Xạ trị điều biến liều theo thể tích hình cung (VMAT: Volumetric Modulated Arc Therapy) là kỹ thuật cao với hệ thống máy gia tốc hiện đại trang bị collimator đa lá (MLC) được thực hiện rộng rãi khắp các nước trên thế giới trong điều trị ung thư phổi không tế bào nhỏ phối hợp với hoá chất. Với kỹ thuật VMAT, các trường chiếu nhỏ được hình thành từ các hướng của chùm tia. Các trọng số tương đối của liều lượng sẽ được làm tối ưu hóa trên mỗi góc độ chùm tia. Để thực hiện được điều này, người ta sử dụng khả năng tối ưu hóa trực tiếp độ mở của collimator. Nghiên cứu nhằm nhận xét ưu điểm của kỹ thuật xạ trị điều biến liều VMAT so với kỹ thuật xạ trị 3D trong lập kế hoạch điều trị và đánh giá kết quả và các biến chứng sớm, muộn khi áp dụng xạ trị điều biến liều VMAT trong điều trị ung thư phổi không tế bào nhỏ giai đoạn III. Từ tháng 6 năm 2017, tại Bệnh viện K Trung Ương đã điều trị cho 30 bệnh nhân ung thư bằng kỹ thuật VMAT. Kết quả điều trị cho thấy kỹ thuật xạ trị điều biến liều VMAT có nhiều điểm ưu việt hơn so với kỹ thuật xạ trị 3D trong lập kế hoạch điều trị thể hiện qua các chỉ số HI, CI, UI, V95 tại PTV; V105, thể tích các cơ quan nhận liều bức xạ; tỉ lệ đáp ứng một phần chiếm 80%; tỉ lệ sống thêm toàn bộ 24 tháng là 64%, tỉ lệ biến chứng sớm và muộn bao gồm viêm phổi và thực quản với tỉ lệ thấp và chỉ có ở độ 1 và 2.
