Sclerostin is produced almost exclusively by osteocytes, which also express receptors for 1,25 dihydroxyvitamin D3. The aim of this study was to investigate the effects of vitamin D3 treatment on serum sclerostin levels in young adult females with severe vitamin D deficiency. A total of 26 subjects were treated orally with calcium (1.200 mg/day for 2 months) and vitamin D3 (300.000 IU/week for 1 month). Serum 25-hydroxyvitamin D (25(OH)D) and sclerostin levels were measured before and after treatment. Baseline serum 25(OH)D and sclerostin levels were at 5.7 ± 2.4 ng/mL and 39.1 ± 14.4 pg/mL, respectively. Serum 25(OH)D was significantly increased, to 62.4 ± 18.7 ng/mL, following treatment; serum sclerostin was significantly decreased, to 29.3 ± 8.8 pg/mL. We conclude that serum sclerostin level is decreased following vitamin D3 treatment in patients with vitamin D deficiency.
Spinal disrafizm (SD), embriyogenez sırasında nöral tüpün kapanmasındaki bozukluğa bağlı omurganın kemik ve nöral yapılarının kısmi birleşmesi veya malformasyonudur. Omurganın arka kısmının orta hat boyunca tam olarak kapanmaması söz konusudur. SD sıklıkla torakolomber ve ve lumbosakral seviyelerde oluşur ve servikal SD tüm spinal anomalilerin %1-5'ini oluşturan nadir bir durumdur. SD açık ve kapalı(gizli) olmak üzere iki şekilde ortaya çıkmaktadır. Gizli SD, genellikle düz grafi veya manyetik rezonans görüntüleme sırasında tesadüfen saptanmaktadır. Klinik olarak önemli gizli SD, kas kuvvetsizliği, cilt anormalliği, Klippel-Feil Sendromu, torasik hemivertebra, ayak deformitesi, duysal anormallik, üriner inkontinans, yürüyüş bozukluğu ve skolyoza neden olabilir. Boyun ve sırt ağrısıyla başvuran ve gizli SD ve torakolomber skolyoz saptanan iki erişkin kadın olguyu sunuyoruz. Servikal SD nadiren bildirilmesine rağmen, dermatolojik, nörolojik ve ürogenital hastalıklarla birlikte skolyoz gibi ortopedik bozukluklarla da birlikte ortaya çıkabilir.Spinal dysraphism (SD) is the partial fusion or malformation of bone and neural structures of the spine by errors in the closure of the neural tube during embryogenesis. SD commonly occurs at the thoracolumbar and lumbosacral levels, and cervical SD is an uncommon condition that comprises 1-5% of all spinal anomalies. Occult SD is generally diagnosed incidentally on plain radiography or magnetic resonance imaging. Clinically important occult SD may cause muscle weakness, cutaneous abnormality, Klippel-Feil syndrome, thoracic hemivertebra, foot deformity, sensory abnormality, urinary incontinence, gait abnormality and scoliosis. We report two adult females with occult cervical spinal dysraphism and thoracolumbar scoliosis who presented with neck and shoulder pain. Although cervical SD has rarely been reported, this report highlights that may be associated with orthopedic abnormalities such as scoliosis as well as cutaneous, neurologic and urogenital disorders.
Amaç: Beldeki mekanik problemler sıklıkla karşımıza çıkar ve bazen sinir köklerinde basıya neden olabilir. Buna bağlı olarak fiziksel muayene sırasında değişik nörolojik bulgular saptanabilir. Değişik etiyolojik nedenlerden kaynaklanan bel ağrıları arasında metabolik kemik hastalıkları, özellikle osteomalazi ve osteoporoz da bulunmaktadır. Gereç ve Yöntemler: Çalışmamızda, yaşları 30-65 arasında olan 25 hasta değerlendirildi. Demografik bulgular (yaş, cinsiyet, meslek, eğitim) ve klinik bulgular (ağrı şiddeti, ağrı süresi, vücut ağırlığı, boy) değerlendirildi. Hastalarda, nörolojik bulgular, duyu değişiklikleri, derin tendon refleksleri ve kas kuvvetleri, dual enerji x-ışını absorpsiyometri (DXA) sonuçları ile korele edildi. DXA değerlendirmesinde, L1-L4 total, femur boynu ve femur total bölgelerinde kemik mineral yoğunluğu ve T-skorları kontrol edildi. Bulgular: Ağrı şiddeti VAS'a göre 6'nın üzerindeydi. Kas kuvveti, duyu ve tendon refleks değerlendirmeleri genellikle normaldi. Femur boynunda osteopeni saptandı. DXA sonuçları arasında L1-L4 KMY (kemik mineral yoğunluğu) ortalama değeri ile ağrı şiddeti arasında anlamlı, zayıf ve negatif bir korelasyon saptandı. Sonuç: Bu çalışmanın mekanik bel ağrısı bulunan daha çok sayıda hastada ve karşılaştırmalı, çok merkezli, uzun süreli takipli olarak yapılması gerektiği düşüncesindeyiz. Neurologic findings like decreased deep tendon reflexes may be found during the physical examination. There are many etiologic factors in the pathogenesis of LBP and metabolic bone diseases like osteoporosis and osteomalacia also may cause mecanical LBP. Materials and Methods: In this study, 25 patients between 30-65 years were evaluated. In the patients, sensorial changes, deep tendon reflexes and muscle strength were correlated with dual x-ray absorbsiometry (DXA) results. Demographic findings (age, sex, occupation, education) and clinical results (pain duration, pain severity, height, weight) were evaluated. In DXA evaluation, L1-L4 total, femur neck and femur total bone mineral density (BMD) and T-scores were reported. Results: Pain severity, according to visual analog scale (VAS) was higher than six. Muscle strength, sensation and deep tendon reflex averages were found to be normal. DXA results were found to be osteopenic in femoral neck region of our patients. DXA results and clinical findings showed a weak and negative statistically significant correlation only between BMD at L1-L4 and pain severity. Conclusion: This study should be performed comparatively, multicenter, in a larger number of patients with LBP, for a longer period of follow-up.
